CLEANING YARD WITH BOBCAT, WHILE EMPTYING BUCKET, DUST/DEBRIS CAME BACK AT EMPLOYEE. A PIECE OF METAL WAS IN THE DEBRIS AND ENTERED EMPLOYEE'S EYE REQUIRING REMOVAL.

WHILE ASCENDING LADDER TO GET INTO TRUCK EMPLOYEE FELT A POP IN HIS BACK WHILE AT THE TOP OF THE LADDER. TAKEN TO THE E.R. SAME DAY AND WAS RELEASED TO LIGHT DUTY AND BEGAN TO HAVE DISCOMFORT AGAIN 7-26-01. SOUGHT TREATMENT AND WAS TAKEN OUT OF WORK.
