Employees finger was caught between a sliding section of angle iron and a fixed item on the plant. This resulted in a 1" cut and a fractured finger tip.
Miner was shoveling wet material at the entrance to the Lower Mill.
Employee cut a box open with a utility knife and placed the knife on top of the box. When the employee pulled the box open with EE's left hand EE's left index finger make contact with the blade of the utility knife resulting in a cut to EE's left index finger.
Employee was stepping off a staircase from RC-7 onto ground level when EE felt pain in their left foot.
Hearing Shift
Employee was using a hammer to attempt to unclog a chute from outside the chute. After doing so, EE felt pain in EE's right shoulder.
Employee lost footing while cleaning out #4 chute. EE slid about 2ft and regained footing, but with ankle in awkward position causing an Avulsion Fracture.
Employee has been placed on MSHA log for hearing shift. Testing had noise variables due to on-site testing during production due to Covid-19 restrictions for large gatherings and local occupational exams being restricted.

North Railroad switch in North Branford Quarry was frozen. Heat was used to melt the ice on the switch. It freed up but not all the way. Employee used pry bar to see where switch was hanging up. Pry bar slipped and employee fell backward onto right wrist.

Stepping off truck employee went to plant foot on ground and reposition hand on railing, leaning back hand came off railing only one hand left on railing and fell backwards and hit head on ground
Employee was cleaning the feeder chute and smashed right thumb between the tool and the chute. Employee was taken to urgent care and received 2 stitches.
Tight work area, awkward positions, unexpected kickback of grinder
While pulling door closed with left hand, employee placed his right hand on the door jamb. Employee caught his right index finger in the door by the window while seated inside the truck.
Employee slipped while opening railroad car door and got his finger caught in a pinch point resulting in a laceration on his right index finger that required stitches.
Employee was removing new style motor base from it's pedestal when stored energy from the torsion bar broke free striking the employee's right index finger.
While getting off the railroad car, his foot slipped on the icy step. As he grabbed the railing to break his fall, he felt pain in his left shoulder and lower back.

Opening a ballast car door. Door jammed while employee trying to lift it open with bar causing a muscle strain in upper shoulder and neck.

Employee was using a hydraulic jack on a locomotive leaf spring when the jack kicked out causing the spring to strike his right thumb resulting in a crush injury requiring stitches.
While using a pick axe to loosen dirt, the pick struck a cross member causing the pick to change direction and hit the inside of the employee's left foot. Treated for a foot contusion and placed on restricted duty.
The employee was climbing down the ladder of the railroad car and slipped and fell, hitting the edge of the railroad car and fracturing three ribs.
Employee was attempting to climb onto the top of a screen deck when his hard hat struck an overhead beam causing him to lose his balance and fall onto the screen deck.
Employee was trimming a rubber gate guard with a razor knife. The knife pulled free of the rubber, striking his left leg just above the knee.

Employee claims left and right wrists were injured from connecting and disconnecting trains; climbing on and off ralroad cars and locomotives and through throwing track switches.

Employee was attempting to dislodge a broken RR Car door spring. When the spring dislodged it broke and a piece flew out striking the employee in the left thumb.
Employee felt back pain after operating front end loader. The employee assumes condition was caused by breaking up frozen recycled asphalt with loader.
Top drill bar not in carosel properly when it fell onto employee's shoulder
While sitting in his 100-ton CAT haul truck employee was reaching for his lunch pail while getting loaded. When the loader dumped the shot into the truck bed the truck shook violently causing the employee's shoulder to be jarred upwards while he was reaching for his lunch pail.
While lifting a RR Tie employee substained an abdomen hernia.
Employee has his personal pocket knife in his hand and was leaning back in a chair when he lost balance and jolted suddenly and stabbed himself in the thigh.

No accident date. Employee devloped pain in both arms over time asa result of operatinga loader. Employee returned to work in a restricted capacity for a a period of 10 working days unless otherwise instructed by treating phsycian.
When exiting a tug boat, steppped on dock, hyper extended right knee back, then fell.

Walking and fell on snow and ice
PULLING CABLE WITH WINCH, PULLED TO FREE JAM INSTEAD OF WALKING TO JAM, IT CAME LOOSE AND STRUCK IN MOUTH.
JUMPED OFF SLOW MOVING TRAIN, TWISTED RIGHT ANKLE.
Bending & twisting moving RR tie, and felt a pop in hip, it gave out.
WAS INSTALLING A DRIVESHAFT INTO EQUIPMENT WHEN EE EXPERIENCED A SHARP PAIN IN HIS BACK, THE NEXT A.M. IT WAS NOT ANY BETTER.

EE WAS EXITING FROM THE BACKHOE LOADER. THE EE SLIPPED ON THE FRESHLY FALLEN SNOW WHICH HAD COVERED THE EXISTING ICE FROM PREVIOUS STORMS. THE EE WAS UNAWARE OF THE ICE BENEATH THE SNOW. EE SL IPPED BACKWARDS FALLING ON HIS LEFT WRIST WHILE TRYING TO BREAK HIS FALL.
EE WAS REPLACING CAM IN SPRING GUIDE. A BENT CASTING WEDGED AGAINST THE SPRING INSIDE THE DEVICE. WHEN THE PUSHER BOLT WAS BACKED OFF PARTIALLY THE LINE UP PIN WAS REMOVED BECAUSE IT WAS LOOSE IN THE BORE. WHEN HE TOUCHED THE END CASTING IT RELEASED THE SPRING TENSION PROPELLING THE HOUSING STRIKING HIS HAND AGAINST A WORKBENCH VISE.
EE WAS GETTING INTO HIS TRUCK AT THE BEGINNING OF HIS SHIFT AND ENTERED THE CAB IN A DIFFERENT MANNER THAN WHAT HE WAS USE TO . IN DOING TO HE TOUGHT HE PULLED A MUSCLE. HE ATTEMPTED TO EXIT T HE TRUCK LATER IN THE DAY AND BEGAN TO FEEL SHARP PAIN IN HIS UPPER LEG. HE WAS EVALUATED BY A CHIROPRACTOR WHO PLACED HIM ON LIMITED DUTY.
EE WAS GETTING READY TO DISCONNECT A STRING OF AMTRAK CARS REQUIRING HIM TO SHUT OFF THE AIR TOTHE 2 AIR BRAKE LINES. HE THOUGHT HE FORGOT TO SHUT OFF THE AIR IN THE LINE WHEN IN REALITY THE Y WERE ALREADY IN THE OFF POSITION. INSTEAD HE TURNED THE VALVES TO THE ON POSITION SENDING 150PRI OF COMPRESSED AIR THROUGH THE HOSES STRIKINGHIM IN THE FACE.
EMPLOYEE WAS CLIMBING FROM THE DOCK TO THE BARGEWHEN HIS RIGHT PLANTED FOOT SLIPPED FROM UNDER HIM CAUSING HIS LEFT LEG TO BE WHIPPED INTO A BIT.
CLEANING LOOSE STONE FROM TOP OF HOLE, HEAD OF DRILL CAME DOWN ONTO EMPLOYEE'S LEFT HAND AND WRIST PINNING IT IN PLACE.
HAUL TRUCK ROLLOVER DUE TO EE NEGLIGENCE.
WHILE REMOVING WATER PUMP FROM TRUCK, A SMALL PIECE OF RUST FELL INTO EE'S EYE. EE GIVEN A PRESCRIPTION MEDICATION AT CLINIC.
WHILE EXITING CRANE EE WAS STEPPING ONTO A CATWALL, AND FELL APPROX. 6' TO THE GROUND.
WHILE CHANGING BELTS ON SCREEN, HAND BECAME CAUGHT BETWEEN PULLEY DRIVE AND FIXED SECTION OF STEEL.
WHILE EE WAS CLIMBING DOWN A LADDER FROM THE #1 - 4' CRUSHER AREA, HIS HAND CAME IN CONTACT WITH SHARP PIECE OF STEEL CAUSING A LACERATON AND REQUIRING STITCHES.
EE CLAIMS HE FELT A "RIPPING" SENSATION IN HIS BACK WHILE PULLING RAILROAD SPIKES. HE IS CURRENTLY LOSING TIME FROM WORK. QUESTION IF WORK RELSATED DUE TO OTHER CIRCUMSTANCES.
