08/18/2022 employee was removing screens from a screen deck. Employee lost grip with left hand causing the screen to twist with it still in EE's right hand. General first aid measures were given and the employee returned to work. Employee was seen by an occupational clinic on 11/23/2022 for continued pain with injury. Physical therapy and restrictions were prescribed on 11/23/2022.

Employee was removing bolts from the conveyor legs. As the bolts came loose, the conveyor leg swung out and hit the ladder he was standing on and he fell.
