EE WAS WALKING ON A LEVEL HARD SURFACE WHEN HE LOST HIS FOOTING AND FELL DOWN. HE TRIED TO CATCH HIS FALL WITH HIS LEFT HAND AND ENDED UP WITH A FRACTURED LEFT WRIST. INVESTIGATION SHOWS NO TRIP HAZARDS PRESENT.

REPLACING COUNTERWEIGHT IN TELESMITH 48 CONE, MANTLE WAS LAYING ON THE GROUND. HE WAS WORKING NEAR BOTTOM OF COUNTERWEIGHT WHEN ANOTHER EMPLOYEE ROLLED MANTLE TO CONTINUE WORK, MANTLE ROLLED ONTO HIS FINGER.
