Task was to remove screen flare. Employee was loosening bolts, changed work location, dangerously close to excavator, without warning or eye contact with excavator operator. Excavator rotated, bucket of excavator struck employee in the upper chest resulting in death.

Miner had his finger where it should not have been. Miner was treated at the mine site, then taken to the doctor where he recieved a couple of stitches in his thumb. Miner was returned to work the next day, and instructed to do light work that would not effect the injury.
