While EE was installing screens in the middle deck of the 6x20 he struck his little finger with a 4lb sledge. Causing a laceration and fracture. EE's aim was deflected when he struck the deck at the beginning of his swing.

Employee was walking across the 8x20 top deck screen and stepped on a soft spot on the screen media with the outisde of his right foot, while the inside of his foot was on a ridgid support beam, causing his foot to roll to the outside and causing an ankle sprain.

While doing a slump test the employee was packing down mud with a tamping device & caught his finger between the cone and the tamper. It caused a small open wound on one finger, and partial removal of a fingernail. The tamper was hand-held and the employee misjudged the distance between his finger and the item he held in his other hand.

At approximately 7:10 AM two employees determined an inundation occurred & one of them immediately called MSHA. Inundation consisted of South Platte River spilling into pond. No equipment or personnel affected.
