Employee was setting the park brake on a JCB 509-42 telescoping forklift, upon exiting the machine EE noticed a strange feeling in the left bicep and that it was swollen.

EMPLOYEE WAS CLEANING ROCKS OUT OF THE TAIL PULLEY OF A CONVEYER BELT. THE BELT WAS ACCIDENTLY TURNED ON DUE TO A MISCOMMUNICATION BETWEEN THE PLANT OPERATOR AND A LABORER. EE WAS TRAPPED IN T HE TAIL PULLEY RESULTING IN THE LOSS OF RIGHT ARM TO HIS SHOULDER, REMOVED SHOULDER BLADE.
