The Employee was performing maintenance tasks on a wash plant. EE went to kneel and EE's right knee contacted a grinder that was plugged-in and lying on the platform. The grinder came on causing it to tear through EE's coveralls and cause a laceration to the knee requiring stitches. The grinder was checked immediately and was in proper working order.

Employee attempted to access area of washplant which was not protected by hand railing. In violation of company safety policies, training, and work practices, employee failed to use supplied fall protection equipment. Employee lost his footing on framework of plant and fell approx. 8 ft. to ground.
