Equipment operator was trimming the face, back, and ribs with an excavator with a road header attachment. He finished trimming and was starting to tram back out. Operator traveled 4 feet and went over uneven floor causing his left arm to activate the swing arm lever to the left. The road header swung to the left striking the injured employee.

Employee was emptying bags of cement in the dry pot machine and had all his PPE on. Employee started to sweat and the sweat ran down thru his goggles and into his eyes. After the task was completed, he took his goggles off and rubbed his left eye irritating it.

Employee picked up a full can of welding rod by grasping the sharp edge of the top. The weight of the can caused the sharp edge to cut through employee's leather glove and into his left index finger. He received 5 sutures.

The injured employee was struck by a wooden post that had come under compression from the scissor deck he was operating. The post was left leaning against the rail of the scissor deck. When the deck was raised, the post became wedged between the deck and the rib, the pressure was relieved suddenly from the base of the post causing it to strike him in the elbow and ribs.
