Employee was at the belt press area, they slipped and when they fell, they hit their head on the ground.
Employee slipped, fell on their back and hit their toe on something that was nearby and also hit their head on the ground. Employee said they felt fine but out of precautionary measures we sent them to the clinic for a check up.
Employee was using a hose to spray pump, employee was putting the hose away and their leg went through the hole on the grading causing leg abrasions and sore back.
Employee was driving the haul truck when they hit a bump in the road, felt back getting tight over the next 10 minutes after hitting the bump.
Employee was unloading a truck when they felt pain on right shoulder.
Employee was operating heavy equipment when they started feeling pain in lower back - employee was unsure as to what initially caused it.
The employee was trimming a piece of plastic with a razor knife.
Employee slipped on a wet platform as they stepped off the last step on stairs.
Haul Truck Driver lost control of truck as EE turned a corner. Truck veered left over a dirt berm and into a small ditch nose down and abruptly stopped, thrusting driver hard towards steering wheel/dashboard, possibly hitting head on cab frame. Driver was initially unconscious. EE was later diagnosed with broken ribs, punctured lung and a brain bleed.
The employee was injured while repairing/cleaning the Chute with a high volume of sand on employee's left leg.
Strain, Lower back area
Cutting a bearing with a grinder and a metal piece went into their right eye.
Wasp bite to right arm.

Strained right shoulder holding up a tail pulley while trying to attach bolts back into it.
EE was using a water hose to wash out EE's haul truck. As EE was walking backwards without looking, EE tripped on a cement k-rail and fell over it, in the dirt on EE's back. EE did not report the injury until 10/18/21
Dirt and Debris on the Catwalk in the Wash Plant. It was evening - dark.
Slipped on a FLOC which is a slippery compound.
Employee was walking down metal stairs and slipped, causing EE to fall onto a motor. This accident resulted in contusion and sprain of ribs.
Employee injured right elbow on a metal head box chute while cleaning with a shovel.
High pressure hose slipped out of employee's hand causing the water stream into their left eye.
Employee was washing down concrete with a water hose. EE slipped on the water and twisted right ankle.
EE was at the maintenance shed unloading a piece of pipe, approx. 3' long, 50 lbs. EE dropped one end on the top of right hand, resulting in a laceration.
EE was backing up the water truck towards a berm. EE went too far and started to drive up the berm, shifting the weight of the water tank, causing the truck to roll on its side
Excavator disturbed a bee hive and ee was near it. EE received 5 stings
Employee was bit by a black widow while working in an electrical cabinet
Employee was using a cutting torch to cut a steel beam, approx. 200 lbs., 5" wide, about 1 foot from the ground. As ee was cutting the beam, it shifted and fell across both feet. EE right 2nd toe was fractured and ee sustained contusions to both feet.
EE was climbing down a ladder from the belt press plant. EE was going too fast and slipped on one of the steps which caused EE to fall the distance of 6-7 steps onto a concrete floor. EE hit the back of their head, right knee and wrist.
EE was shoveling dirt away from a hopper using a wood handled shovel. EE was not using gloves that were provided to ee and a wood splinter went into hand which got infected because ee signed a form to waive medical treatment on the day of injury.
While shoveling dirt and mud, EE hit right hand knuckle on a motor bracket, causing a laceration.
Employee states that ee was reaching into the truck bed of personal vehicle to put tools away when ee felt a sharp pain in back
EE was washing out a pulley with a water hose and slipped on the wet concrete which caused ee to fall on right hip. EE felt pain in back.
Employee tripped and fell exiting a portable toilet.
During a routine inspection of the 988 Loader, employee bent down to reach the lever to open the cab, when EE felt a "pop" in lower back. After a short time, EE started feeling pain and numbing in legs.
Unknown -employee was diagnosed with Lymphoma and feels it may have been caused by exposure to diesel fuel over time. PLEASE NOTE: EMPLOYEE DID NOT REPORT INJURY UNTIL 05/09/2016.
EE was repairing the underflow pipe to the thickener tank, slipped on mud and twisted left knee. **PLEASE NOTE: EMPLOYER WAS NOT NOTIFIED OF INJURY UNTIL 05/08/2016
Co-worker was driving a 988H Loader on the outskirts of the property to avoid scrapers. He drove the tractor over a hill, bucket lowered, he did not notice a pickup truck driven by employee, coming towards him. The front left loader bucket, hit the left front of the truck, injuring him. The employee in the pickup was hurt when the steering wheel was pushed into him.
Three employees were replacing screens at our aggregate wash plant where soil and rocks are separated. While replacing the screens, a couple tools and a screen fell down to a lower level. EE went to the lower level to retrieve the fallen items. While he was down there, another screen, approx. 5 lbs. fell on him, hitting him in the face.
Bruised right knee from kneeling too long on screen doing repairs.
EE is an independent sales consultant for TPH. While consulting on the subject of flocculant equipment, he slipped on the wet concrete and banged his knee. The area was marked "slippery surface", "watch your step". EMT called and transported EE and then he was released. He will follow up with his primary doctor and notify us if there are any complications.
Employee was shoveling mud into a wheelbarrel.
Mechanic was moving a full 55 gallon drum on to pallet. When he lifted the drum his neck snapped and he felt pain in his back.
On ladder aprox 4' up working with torch removing piece of channel from conveyor. Stored energy caused channel to spring toward miner hitting him in face/knocked him off the ladder. Had on fall protection & was harnessed correctly. It was not far enough for lanyard catch him. Fell & hit his knee. Received small scratch face, contusion/abrasion to left knee/shoulder, sprain knee.
Miner reported on 4/25/08 that his knee began to hurt 4/15/08 while operating a haul truck. Using the retarted aggrevates his knee. Left patella sprain/legiment.
Miner was operating a TD-25 dozer that was stuck in the mud. He was in reverse and hooked by a tow chain to another dozer that was pulling him out. The tow chain that was used to free the stuck dozer broke and a link of the chain broke through the back window of the dozer and struck the miner on the shoulder
Miner was moving a compressor. He grabbed a metal bar to get more torque and the bar broke loose smashing his finger against the compressor.
Employee bumped his knee on angle iron.
The fuel hose was strung all the way out. The hose blew off the reel. Diesel fuel splashed the employee in the face and body. 100psi
Our employee was asked to assist Pacific Aggregate Employees to replace a broken shaft on a conveyor. The belt had not been secured correctly to prevent it from rolling backwards. As it rolled it raised the counterweight and pinched our employee's foot between the counterweight and an angle iron support member.
When flange was removed, employee was sprayed with hot water on inner forearm.
Something flew into left eye while he was welding.
Rolled water truck - Right stomach contusion, body bruised, especially where seat belt crossed chest, bump on head.
Employee was removing seats from classifier. He turned in right direction, straining right groin.
Employee lost control of 8" black elbow pipe while turning it, causing lacerations to right fingers #3, #4, #5.
Employee stepped backwards, stepping on edge of belt. Belt gave way approx one foot and ee hit a bracket.
Employee was removing an item that was secured by a pipe wrench and slipped, causing him to move in a direction he was not ready for. This created a sprain/strain to right knee.
EE was going to the main level of Plant 3 to turn on the 2" gate valve. On the way up he saw co-worker, picked up a sand clog and threw it at him hitting him in the left side of the face. Co-worker picked up piece of rubber from a trash pile and threw it at the EE not knowing the object had steel in it. It caused a puncture on his right thigh requiring one stitch.
Two EE's had a grip on the gear box to lower the object down. One EE lost his grip, while the injured EE had his right hand get pinched between a steel framand the gear box. Right hand (thumb and forefinger) badly bruised.
Employee over filled diesel tank getting diesel on his clothes. He stayed in clothes for extended period of time creating rash on skin from red dye in diesel fuel (slept on clothes).
While moving bobcat, backed equipment onto berm & swerved causing the equipment to fall over & break glass. The glass cut right elbow.
While welding, a piece of metal blew into left eye inside welding hood.
EE claims rupture in coolant tank is leaking on exhaust manifold & fumes are entering cab of vehicle causing a variety of symptoms from dizz- ness to illness.
slipped & fell backwards in mud & landed on tail bone, hitting back of head.
WHILE WORKING ON TOP OF SAND PLANT, EMPLOYEE WAS SHOVELING ROCK TO UNCLOG THE SCREEN, A ROCK THE SIZE OF 2 SOFTBALLS BOUNCED FROM THE SCREEN & HIT THE LEFT SIDE OF EE'S HELMET.
Water truck would not stop going in reverse & rolled over edge of cliff into ravine EE jumped out of vehicle on to the groung dropping about 6-8 feet to avoid serious injury. EE broke L heel sprained knee, R shoulder & side received cuts & scrapes from foliage.
Left eye got dirt or residue in it.
Driving ready mix vehicle around an item on jobsite there was an elevation in ground, causing vehicle to raise on one side and roll over. Employee was thrown into driver door causing laceration & bruise to left ankle, bruise to left thigh & abrasion & bruise to left lower back.
BALANCING A PIPE SUPPORTED BY FORKLIFT THAT WAS RESTING ON A BLOCK. THE ITEM ROLLED OFF THE BLOCK AND FELL ON LEFT FOOT.
WHILE WORKING ON ELECTRICAL FOR READY MIX CEMENT TRUCK, BEGAN TO GET DIZZY. HE SAT DOWN FOR AWHILE AND BEGAN SPINNING FROM DIZZINESS, RESULTING IN VOMITING. REPORTED ILLNESS TO NEARBY EMPLOYEE. REPEATED DIZZINESS & VOMITING CAUSED EMPLOYEE TO REQUEST AID FROM PARAMEDICS.
EMPLOYEE SMASHED RIGHT MIDDLE FINGER IN DOOR AT WORK. REQUIRED 2 STITCHES.
MAINTENANCE MECHANIC REMOVING AND INSTALLING AN ELECTRIC MOTOR ON SCREEN DECK ON PORTABLE SCREENING PLANT. ANOTHER EMPLOYEE WAS OPERATING A CRANE TO LIFT THE MOTOR. HE PLACED HIS HAND BETWEEN THE SNATCH HOOK AND THE SCREEN FRAME AND PLUMBING TRIED TO STOP THE SNATCH HOOK AND HIS HAND WAS PUSHED AGAINST THE END OF A BALL VALVE.
