SLIP OR FALL OF PERSON

Employee was washing the floor deck of a 355 Komatsu. Once finished he stepped down to the track and slipped with his right foot. His left foot became wedged in between the blade push beam and track frame as he fell from the top of the track to the ground level. The employee was immediately taken to the local emergency room and determined that he had broken his left ankle.
