Examining engine under hood of haul truck, body shifted, slipped and hand got caught in fan belt pulley. Taken to Renown Regional Medical Center in Reno, NV. Skin was pulled back over finger tips and sown together and released within a few hours.

WHILE GETTING INTO LOADER AND CLOSING DOOR, WIND BLEW DOOR WIDE OPEN. DOOR SWINGING OPEN CAUSED RIGHT ARM TO BE PULLED/YANKED. RESULTED IN INJURY TO RIGHT SHOULDER. EMPLOYEE NEEDED ANTI-INFLAMATORY DRUGS AND AN ARM SLING FOR APPROXIMATELY 1 WEEK.

WHILE DRIVING TRUCK ON MAIN DIRT ROAD, HIT SOFT SPOT IN DIRT CAUSING THE STEERING WHEEL TO JERK. THIS ACTION RESULTED IN A LACERATION TO EMPLOYEE'S LEFT HAND BETWEEN THUMB AND FOREFINGER (RESULTING IN 9 STITCHES) AND CAUSED A SPRAIN TO RIGHT HAND
