Employee was walking down steps from lunch room trailer when he rolled his left ankle.
Mud pump on clarifier exploded while EE was relieving preasure from top of pump causing hot rock and mud to splash on EE face and left arm. EE had light burns on face and 2nd degree burns on left forearm.
EE was cleaning concrete floor around clarifier. EE slipped and hit right forearm on metal I beam. Resulting in a muscle bruise.
EE was removing sprocket from drag chain inside clarifier, support chain come-a-long internal mechanism failed causing chain to fall and pinch EE's Left hand between sprocket and drag chain.
AFTER CUTTING POLYLINERS FOR THE GRIZZLY WITH A SKILL SAW, HE RETRACTED HIS BODY OVER THE LENGTHOF THE LAST CUT. THE SAW WAS OUT OF THE CUT, ON ITS SIDE WITH GUARD FULLY EXTENDED. HIS HAND & FINGER WERE PLACED OVER THE TRIGGER BUT THE SAW WAS NOT IN MOTION, AS HE RETRACTED THE SAW WAS ACTIVATED & CUT INTO HIS KNEE.
INSTALLING CONVEYOR, CABLE SMASHED FINGER AGAINST METAL CUT AND BROKE LEFT RING FINGER.
