Employee was exiting a 633D scraper using 3pts of contact when they felt a pop in right shoulder. Employee did not think nothing of it until the following day when the shoulder felt much worse.

Employee notified manager that they were having discomfort in their lower back which was radiating to their neck and left shoulder. Employee was operating a CAT 775 haul truck and stated that it was due to bumps in the pit and haul road.

Employee was removing a zip tie from a coolant line on a water truck using a utility cutter in their right hand; employee was pulling the zip tie with left hand. Employee did not expect the resistance of the zip tie until it had let loose which caused the left forearm to strike the right hand that was holding the utility cutter. Resulted in a laceration requiring stitches.

Employee working plant maintenance on feeder and tunnel when employee strained right shoulder. Employee notified manager and denied medical treatment at that time. Employee notified manager a second time (Aug 17, 2024) that the shoulder was still hurting, but again denied medical. On Aug 20, 2024 employee requested medical treatment and was placed on modified duty.

Employee noticed swelling in the left hand and was not sure if it was due to a bee sting or spider bite. Employee was seen by doctor whom stated it had looked like a possible open wound that had become infected. No determination of when employee had a open wound.

CALPORTLAND CONSTRUCTION · Over-exertion in pulling or pushing objects

Employee is a mechanic and was working on the 992 loader, repairing a hydraulic hose. EE was pulling on a wrench and felt a pop in EE's back. EE reported the incident at 5:00 PM to the Equipment Manager. Employee went to clinic the next morning where doctor confirmed it was reportable and would result in modified duty. Employee will receive an MRI and is on modified duty.

Employee was working on a plugged crusher. Employee was removing rocks from the crusher to clean it out. The right hand was inside the crusher. Using the left hand, the employee lifted a rock and then dropped the rock on their ring finger of the right hand, smashing it. The employee received 4 stitches.

Employee was working on hands and knees while positioning a grader blade on a motor grader. The employee suddenly felt a pain in EE's lower back.

Employee complained of pain to his left Buttocks. He stated that the back of his leg goes weak when the pain occurs. He requested to get checked out by a doctor. When asked if it was a work injury that caused it, the employee stated that it was from years of working as a mechanic for the company.

On 7/15/13 the employee informed his supervisor that he had pain in the left knee and he requested treatment from a medical professional at that time. The employee was released to full duty with NO restrictions but he did receive a prescription for an anti-inflammatory and is scheduled for a follow up MRI. EE was given knee brace.

The plant was shut down for a maintenance day & employee was shoveling alongside the conveyor in the reclaim tunnel. He was doing a continuous motion of bending down, then raising back up, to put the material on the belt. EE states he felt a "head rush" and fell down, cutting chin, requiring 7 stitches. He also required a Tetanus shot.

Employee was washing down a concrete slab with water hose and slipped on hose falling on the left side breaking his fumor bone. At first he just thought it was bruised. However, per company policy, he was taken to the medical facility for follow up, where he was exmanined and had x-rays determining the extent of the injury would need surgery.
