Employee was cleaning conveyor close to the head pulley and snubber idler. EE failed to follow Lock Out / Tag Out procedures and inadvertently got hand caught between the head pulley and snubber.

Miner was standing under forklift mast that was being supported by another forklift and a sling. The sling felt off the forks letting the mast fall striking the miner and landing on his left foot.

A MANHOLE COVER ON TRUCK SCALE HAD A CORRODED SUPPORT BLOCK WHICH GAVE AS EE WALKED ON IT. EMPLOYEE FELL, HIT HIS KNEE AND SCRAPED HIS SHINS AND CAUGHT HIMSELF WITH HIS HANDS. THIS RESULTED IN MINOR CONTUSTIONS/SCRAPES. DR. PRESCRIBED CELEBREX & VICODIN AND EMPLOYEE WAS PUT ON MODIFIED DUTY. FOLLOW UP ON 7/19/2004, DR ORDERED EE OFF FROM WORK.
