Employee was operating CAT 980k front end loader. While driving through the shipping yard at some point, employee hit a pothole causing employee to get jarred on the seat, this was a combination of operating while out of position ergonomically,

The EE was in the passenger seat of a haul truck, ridding back to the excavator and the main yard, when the haul truck struck a bump. The EE sustained a contusion on the left side of the buttocks from the seat belt support plastic.

EE was called on the company radio and told that the Jaw Rock Chute to the ground was plugged with a big rock. He walked up the small pile of rock under the chute in order to dislodge the large rock. When the rock was dislodged the other rocks in the chute began to flow and he turned to see what the noise was. At that point he felt a pain in his left leg.

A screen cover (wire cloth) fell into the "Jaw Rock" chute. The victim was removeing the screen cover and a rock approx. 4" x 8" came traveling down the chute and pinched his fingers btwn. the rock and chute wall. He suffered lacerations on his L/H middle and ring fingers. The middle finger was broken behind the fingernail.

Laborer was shoveling material into loader bucket. Material slid off and employee was pinched between bucket and pile. Employee suffered bruising to both thighs. Employee was treated and released at Dr.'s Office. Given pain medication and modified duty.
