Employee was pulling knife gate out from chute when a finger was pinched causing a laceration.

Employee struck their left forearm with a knife while tasked with patching screens on screen deck.

Employee was walking up stairs at the water plant when they slipped and sprained an ankle.

employee was inserting a new upper pin into the bucket ram. As they hit the pin head, a piece of steal shattered off and implanted into left shoulder.

Employee was relocating a storage locker from an employee breakroom to the maintenance shop. Upon doing so, the employees left hand was caught between the locker and a heavy duty shop table causing a contusion.

Employee reported pain in right shoulder from shoveling.

Employee was pulling a pin out of the ground and felt a pain in their lower back while doing so. On August 3rd, the employee made a determination that the continued pain warranted being evaluated by a doctor. The visit to the doctor occurred on 8/3/2018.

Employee reported lower back pain a week after using a 5 gallon bucket to transfer liquid from a spill to a poly tote.

Pulling debris from import loads employee stepped backward and lost footing. Employee used left hand/arm to stop fall resulting in a wrist fracture.

Employee was guiding an equipment operator relocating the weight and guy wire for a telephone pole. The pole cracked and bees flew out stinging the employee multiple times.

During the pre shift inspection on the Dredge EE hit the (top) back side of their hand and EE does not recall what they hit their hand on but does recall while inspecting a dewatering screen they hit their hand on something. At the time they did not notice anything but about a hour later they noticed their hand bleeding.

Employee was preparing a rear bumper on service truck #7 to weld on by grinding with an air grinder and the grinding wheel came apart and struck employee's abdomen cutting open employee's stomach.

EE was climbing into screen #701 to replace skirt rubber that deflects rock at the end of screen. While entering the screen EE had to push existing skirt. While pushing EE heard a "pop" in their back but did not feel pain so continued & completed the task. In the next morning while getting up EE's back was sore & EE notified supervisor (mill foreman).

EE was shoveling in P-9 tunnel and felt a little pain in right shoulder on Saturday and thought nothing of it. That night EE woke to considerably a lot more pain and the next morning EE notified supervisor. On Monday the 10th EE requested to go to the clinic and EE was provided that.

Employee was climbing the ladder of the 972H front end loader and lost grip with right hand and while do so left pant leg caught on ladder causing the employee to fall.

EE was assisting with removal of the front hubs on the 988F front end loader. While they were attempting to place the hub on shop floor with use of overhead crane, EE had placed a lifting strap around hub & to the crane hook, EE was trying to force the hub to set down level & right hand middle finger became pinched between strap & a 90 oil fitting on the hub.

While assembling a 12-inch pipe, using an excavator as a crane, the pipe moved bumping EE in the right thigh. EE was taken to the clinic and diagnosed with a bruised right hip and strain of right thigh. Physical therapy was recommended. This was treated as a first aid until 6/15/2016 when the clinic requested that the EE see a doctor for continued pain.

The part being worked on was not adequately blocked to prevent movement. The people who performed the hazard analysis did not believe the part would or could move. EE leg was broken above the ankle and will undergo surgery to repair.

Employee suffered a strained back when EE attempted to lift and move a return roller.

A miner suffered from neck strain when the bed of his haul truck came down suddenly. The bed's hydraulic ram hose ruptured as it was descending causing it to fall. He was given OTC meds and restricted work.

After work a miner noticed he had a bump below his abdominal area. He noted that he had been shoveling material that day for a long period of time. He reported it 2 days later after noticing the bump became larger. He did not have pain but wanted to report it anyway. He was sent to the Industrial clinic after he reported it to management.

Loader operator was injured while driving loader on uneven grad the loader rocked from side to side and his right leg below the hip hit the metal bar support to the arm rest inside the loader. "This incident was just viewed as a first aid until a drain was applied to the bruise to aid in healing of the bruise."

Employee was using a pneumatic chipping hammer with a chisel head to aid in the removal of an axle on a water truck. The chisel dislodged from the hammer and struck the employee on the upper lip. Sutures closed the laceration.

Employee suffered a left knee strain as he descended the top of an earthen berm. As he turned to walk down the berm, his right foot slipped on loose material, twisting his left knee. He was provided prescription meds and a knee brace.

Employee received a hypertextension injury to his left thumb while clearing spilled material adjacent to a shaker screen. The employee was using a shovel to clear rocks that had spilled from the 711 screen onto the work deck. His foot slipped on a rock causing him to slip and fall which resulted in the shovel handle jam into his left thumb.

Injured employee suffered a smashing injury to tip of his right forefinger. Employee was engaged in removing the cab from an old Terex haul truck by cutting it away with a torch. As he finished cutting the truck's frame, the cab settled, nipping the end of his right forefinger. The injury resulted in a laceration requiring sutures to close.

Lab tech was maneuvering a wheelbarrow filled with wet aggregate by backing through a doorway when his right forearm struck the door frame, resulting in a laceration requiring sutures to close.

Employee was removing ram cylinder from 773D. Chain hoist was attached to cylinder with nylon strap at center of ram. When employee pulled on the lower clevis with his hands, the ram came free and swung towards fuel tank smashing his hand in between resulting in pressure cut to middle finger requiring stitches.

Employee was drilling a welding lug connected to a 6" piece of cable and a quick connect. The drill press's bit caught the lug and pulled the cable into the drill as well as his hand that was holding the cable. He was correctly not wearing any gloves while operating the press. He was not using the equipment supplied clamping device.

Employee was on p-8 stacker to perform a belt splice. While securing the tail of the belt to the splicing board the employee struck his right thumb with a hammer.

Employee moved from a squatting position to a standing position while installing a knife blade into a crusher.

Employee was cleaning a walkway next to Feeder 306. Suddenly a pocket of wet material pushed through the feeder with no warning, stricking the employee on the left knee.

The employee felt/heard a popping sound in his right knee after rising from a sitting position from working on the brakes of a water truck.

While putting a ladder away, the employee half-opened the ladder with his fingers inside the folding hinge intended to lock down the ladders open position. The employee picked up the ladder with his finger caught in the pinch point.

While welding grizzly bars on the dredge the employee lost his footing falling into a sitting position and striking his left elbow on one of the bars.

While shoveling at the 520 conveyor belt the employee felt low back pain.

Employee was cutting steel from loading box when he was struck by the 15" by 50" plate.

The employee was clearing out a head-box with a long bar when his left hand became caught between the bar and a piece of angle stock inside the box.

While loading rake teeth into the back of a pick-up truck, the employee injured his lower back.

Employee was walking on a wet dredge deck when he slipped and fell.

While walking the water conveyor line, the employee's knee felt like it slipped out of joint.

While the employee was assisting another employee with putting in a return roller, he felt a twitching pain in his low back.

While changing a hydraulic pump, the employee and a coworker were lifting a small junction cable box to tie it into position. Muscle strain to lower back. Injury was initially minor in nature; on 2/24/06 physical therapy was requested, making the injury reportable to MSHA.

While replacing a lip on a shaker with the assistance of a co-worker, the employee strained his right shoulder muscle.

While cutting flag cable for a water conveyor extension with a short handled four pound sledge hammer, the hammer missed the correct contact. It instead struck the employee on the webbing of the left hand between the forefinger and thumb.

The employee was using a utility knife to open a package of padlocks for his work locker. He bended his knee and used the upper thigh as a wooden block for cutting purposes. The utility knife slipped and sliced open the left leg, upper thigh.

The employee was moving from one smaller container to a larger container while carrying a box. When leaving through the door he misjudged the height of the ground to the container. He slipped and fell to the ground.

The employee was cleaning screens. While lifting one of the screens he felt a pop in the arch of his left foot.

The employee was driving his private vehicle when he began to experience extreme pain in his abdomen area. Hernia was determined to be work related on 8/13/04.

The employee was cutting a piece of conveyor belting. While doing so, his right hand slipped causing the razor knife he was holding to strike his left thumb.

The employee was repairing the underflow pump at the clarifer. During the job, the employee bent down to pick up a small piece of the pump and felt a sharp pain in his low back.

The employee was removing a broken troughing can from the 543 Conveyor. As he handed one of the cans up to his partner for removal from the area, the can broke in half and fell on the employee's left hand.

The employee was tightening the conveyor belt on F1 conveyor. Employee hit is left hand fingers between a ratchet handle and conveyor frame.

EE WAS REPAIRING A BROKEN HYDRAULIC HOSE ON THE LOWER PORTION OF DREDGE CLAMSHELL BUCKET RAM. HE WRAPPED A CHAIN COME-ALONG AROUND THE SHAFT OF THE RAM TO MOVE THE RAM BACK FOR ACCESS TO THE HOSE FITTINGS. AFTER TENSION WAS APPLIED TO THE COME-ALONG, THE EE REACHED BEHIND THE RAM TO REMOVE THE BROKEN HOSE & THE HOSE FITTING. THE CHAIN ATTACHED TO THE RAM SLIPPED & ARM WAS CAUGHT.

REPAIR TRUCK HAD AN ELECTRICAL FIRE UNDER THE HOOD. THE EE, WITH THE ASSISTANCE OF ANOTHER EE, ATTEMPTED TO DOUSE THE FIRE. EE HAD TO GET CLOSE TO THE FLAME, SMOKE AND FIRE EXTINGUISHER CHEMIC AL IN ORDER TO PUT OUT THE FIRE.

PORT SIDE RAKE MOVED TO STARBOARD SIDE, BUMPED STARBOARD SIDE RAKE & CUT EE'S ANKLE BREAKING HIS LEG

INJURED EE WAS ATTEMPTING TO CONNECT AN AMP METER COUNTED ON AN ELECTRICAL CABINET. ALTHOUGH STILL UNDER INVESTIGATION, IT APPEARS THAT ELECTRICITY ARCED TOWARDS HIS RIGHT ARM. THE ELECTRICITY , (4160 VOLTS) SOUGHT GROUND TO THE CABINET, MISSING HIS ARM. HE RECEOVE AN ELECTRICAL SHOCK THROUGH CONTACT WITH THE CABINET.
