On 6/1/12 the employee requested medical treatment, was taken to the company clinic and was prescribed physical therapy for a sprained left shoulder. The employee alleged that he jammed his L-shoulder on an arm rest of a loader while stock piling. He was returned to full duty on 6/1/12.

On 9/14/10 The employee was taken to the hospital and received staples for a L-knee laceration. The employee was cutting a plastic piece of guarding with a utility knife when he lost control of his arm motion and hit the inner leg with the knife. The employee was returned to regular duty on 9/14/10.

The employee went to the company clinic on 11/30/09 and was placed on restricted duty. The employee was removing a wear plate from the side of a loader bucket, when the plate fell and hit his R-foot.

The employee was taken to the company clinic on 11/17, received 10 stitches and was placed on restricted duty. Employee tripped on a steel tube and fell on the dust sealing ring from a cone crusher resulting in a R-hand laceration.

The employee went to the company clinic on 10/3/08 and was placed on restricted duty. A burnt out light bulb on the mixer truck broke while being changed by employee, causing a lacerationto L-thumb, requiring sutures.

The employee went to an emergency room on 10/05/2007, received stitches, and placed on restricted duty. The employee was cutting zip ties with a box knife cutter and lacerated his left forearm.

The employee requested to go to the company clinic on 8/23/2007 and was placed on restricted duty. While moving a shaker screen, the employee slipped on a loose pile of rock resulting in a R-inguinal pain (ALLEGED).

The employee went to the company clinic on 6/25/2007 and was placed on restricted duty. While disconnecting a cable from the scraper, the cable jerked and caused a sprain to L-shoulder.

The employee was taken to the hospital on 5/10/07 and was taken off work. The employee was struck on the head and chest by a tree limb while it was being cut down by another employee.

Employee went to see company doctor on 6/16/2005 and was placed on restricted work; while removing a guard on the tunnel feeder, the cutting knife slipped and caused a laceration to L-palm that required stitches.

Employee went to see company doctor on 9/30/04 and was placed on restricted duty. Employee, while sliding himself on concrete floor under loader to install front differential, punctured his left hand/arm.

EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 6-10-03 AND WAS PLACED ON RESTRICTED DUTY. EE WAS WASHING OUT GRIZZLY AND WHILE STANDING ON THE MATERIAL HE WAS WASHING AWAY, HE LOST HIS FOOTING AND FEL L INTO GRIZZLY UP TO HIS ARMS WHICH CAUSED A STRAIN TO HIS LEFT SHOULDER.

EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 3/21/03, STITCHES WERE APPLIED AND EMPLOYEE WAS PLACED ON RESTRICTED DUTY. ATTEMPTING TO REMOVE A STRAP FROM A COUNTER WEIGHT ON #10 CONVEYOR, PRYBAR SLI PPED ALLOWING HAND TO HIT A GUARD CAUSING A LACERATION TO R-WRIST.

EMPLOYEE WENT TO SEE COMPANY DOCTOR ON 9/19/02, STITCHES WERE APPLIED AND HE WAS RETURNED TO FULL DUTY. DESCENDING STAIRS WITH A 5-GALLON BUCKET FILLED WITH TOOLS, EMPLOYEE LOST FOOTING, HELD ONTO BUCKET WHICH THREW HIM ONTO RAILING AND HIS SAFETY GLASSES CAUSED 2 LACERATIONS TO HIS FACE.

EE WENT TO COMPANY DOCTOR ON 7-8-02 AND WAS TAKEN OFF WORK. EE WAS ATTEMPTING TO REMOVE A SCREEN FROM THE SIMPLICITY SHAKER THAT HAD BEEN SILICONED IN. HE PULLED TOO HARD AND STRAINED A MUSCLE IN HIS LOWER ABDOMEN. (ALLEGED)

WHILE DISCONNECTING PLANT RECIRCULATING PUMP FOR REPAIR, EE STRAINED HIS CHEST WALL. EE WENT TO SEE COMPANY DR ON 3/14/02 & WAS PLACED ON RESTRICTED DUTY.

EMPLOYEE FELL OFF THE STEP OF THE TRUCK RESULTING IN A BROKEN ANKLE.

EMPLOYEE WENT TO A DOCTOR ON 6/20/00 & WAS PLACED ON RESTRICTED DUTY. WHILE PULLING A WATER HOSE TO WASH OFF A SLAB, HOSE SLIPPED CAUSING EMPLOYEE TO LOSE HIS BALANCE AND STRAINING HIS LEFT K NEE.
