Employee was working on replacing skirting on this particular conveyor. Employee was kneeling down when they stood up and turned, resulting in a knee strain. As of 8/15/25 employee has an MCL tear right knee.
Upper Pier on K4 Kiln welding retention blocks. Stepping down from ladder and stepped on things underneath the ladder causing employee to twist and strain back. As of 6/24/25 Modifications to desk duty by Medical professional.
Employee was using a pry bar to unplug a chute. left hand slipped and knuckles struck a piece of angle iron. laceration to knuckle area between index and middle finger. stitches required.

Employee was lowering a loader hood when it landed on employee's co-worker's foot.
Employee was climbing the ladder to the disentegrator deck when they lost footing, causing them to fall to the ground, and employee felt pain in their elbow.
Struck left elbow on extended reach forklift arm rest.
Employee was driving the service truck to the "boneyard". Employee drove over a small 6" dip in the road. As employee drove over the rough terrain employee felt soreness in the neck.
Employees noticed smoke coming out of the cooler tower. They called on the radio to alert coworkers of the fire. Multiple employees brought fire extinguishers to the cooling tower area. Employees tried to extinguish the fire with several fire extinguishers. While fighting the fire employees were exposed to smoke inhalation. They were given a prescription inhaler

Employee was trying to remove a metal door on the disentegrator. As the door came loose ee lost grip. The door fell and smashed left wrist.

Employee was cleaning out extruder. The chute that drops down to run out the plugs, slipped and smashed his right ring finger.
Employee was vacuuming debris from jack hammer when a piece of debris flew up under his safety glasses. The debris caused minimum irritation. Employee's condition worsened as time went on. Doctors put him on restricted duty to stay away from dusty environments.
Employee was attempting to throw a rope over a portable water tank that was loaded on a flat bed trailer. When he threw the rope he felt a sharp pain in his left arm. Doctor put employee on restricted duty.
Employee was moving/connecting hose on a portable pump in muddy conditions. He slipped and strained his lower back. Doctor put him on restricted duty.
Employee was helping replace a shaft on a gear box. They were trying to beat the opposite side with a hammer. While holding the opposite side the gear box, it slipped and hit his hand. A small peace of metal was stuck in his left hand.
EE was shoveling underneath a conveyor belt. He struck an object embedded in the ground with the shovel. This caused a contusion to his right wrist.
Employee was installing new V-belts on K-3 shuttle drive using a pipe wrench to roll on belts when the pipe wrench slipped smashing his left index finger between the wrench and motor base. Employee suffered an open wound and hematoma.
Co-worker was using a hammer to drive the pin out of the excavator tracks employee was on the other side when the pin came out and struck his arm. This caused a large cut on employee's right forearm. He returned to work the following day.
Employee was changing out extruder grid when he inadvertenty moved a protective cover over a section of catwalk that was under construction. Employee had been aware and warned of the covering over the catwalk. Employee lost his balance causing him to fall.
Walking around K-3 Kiln gust of wind blew dust and metal particals in face and eyes. a foreign piece of metal went into left eye.
Employee felt hot, went to get more water and felt faint. Safety Coordinator at the facility called paramedics as a precatuionary. Employee was taken to Hospital and later that evening was released.
Employee was in manbasket attempting to weld wear ring on rear of kiln when a gust of wind took it out of his grip and pinched right thumb between new wear ring and rear of hood.
The employee cut his left wrist with a razor knife as he was cutting on a 3" water hose.
Employee was walking by a screen when a piece of material flew into his left eye.
EE tripped on steps, struck left knee on vertical part of hand rail. Fell, got back up, with bruised and scratched left knee.
Employee was filling a rental compressor with diesel. As he lifted the can to pour he lost his footing & fell, hurting his back.
Employee and other ee were attempting to restart a conveyor. as He was cleaning the inside of the convoyer with his arm, Other EE turned on the conveyor.
Truck driver fell off from the back step approximately 3 ft. while tarping trailer and broke his right leg.
While tightening the nose cone bolt with his foot and holding the wrench with his left hand and keeping the bolt from turning with his left foot, the bolt turned catching his left index finger between the end of the wrench and nose cone. This smashed his left index finger.
HE WAS PUTTING A NEW 14" THURST ROLLER ON K-Y COOLER WHEN IT SLIPPED AND PINCHED HIS RIGHT RING FINGER.
Hot dust putting out of rotary kiln #3 flew into eyes as i was walking by. Prescription issued 1-26-05.
EMPLOYEE WAS DUMPING HOT ROCK ONTO PILE WHEN WIND BLEW HOT AMBERS ONTO HIS ARMS, LOWER BACK AND NECK. EMPLOYEE LIFTED BUCKET OF BOBCAT ABOVE CAGE TO DUMP.
EE WAS DESCENDING STEPS WHEN HE FELT SHARP PAIN IN JAW. EE HAD AN INFECTION IN HIS JAW. EE PULLED HANDS OFF OF HANDRAILS TO GRAB AT HIS FACE. EE LOST HIS BALANCE & FELL & BROKE RIGHT LEG BETWEEN KNEE & ANKLE.
DURING DRIVEBELT REPLACEMENT, DRAWING V-BELTS THROUGH A SIX-GROOVE SHEAVE ATTEMPTING TO ROLL THE V-BELT INTO PLACE, HE PINCHED HIS LEFT INDEX & LITTLE FINGER BETWEEN SHEAVE & DRIVEBELT. MEDICATION: CEPHALEXIN FOR INFECTION, PROPOXYPH FOR PAIN, NAPROXEN FOR INFLAMMATION.
WAS CLIMBING OFF 966F LOADER GOT OFF THE LOADER TURNED TO WALK AWAY FROM THE LOADER AND FELT SOMETHING TWIST IN HIS BACK.
WAS WALKING BY WATER TANK FELL IN HOLE THAT WAS MADE BY OVERFLOWINNING WATER. STRAIN TO GROIN.
HIS EXTRUDER WET PLUGGED WENT TO DIG OUT LIFTED GAURD HELD IT UP ON HIS SHOULDER WHILE SHOVELING INTHE EXTRUDOR THE SHOVEL STUCK IN MUD GUARD FELL OFF HIS SHOULDER CAUSING A CONTUSION TO HIS L EFT MIDDLE FINGER.
DOING HIS NORMAL DAILY ROUTINE OF RUNNING THE RAW PLANT HE STRAINED HIS LOWER BACK. REPORTABLE BECAUSE OF THE PRESCRIPTION PILLS. MOTRIN, FLEXERIL, VICODIN.
WAS WALKING DOWN ACCESS RAMP SLIPPED ON LOOSE MATERIAL THAT HAD SPILLED ON RAMP AREA AND FELL, LANDED ON HIP, STRAIN TO BACK, KNEE, SHOULDER.
WAS CHECKING OUT HIS COOLER AREA SLIPPED ON SOME WET MATERIAL AND SPRAINED RIGHT KNEE. THIS WAS NOT RECORDABLE AND NOW THEY WANT TO SEND HIM TO THERAPY AND AN MRI
WAS PLACING SQUARE TUBING PIECE OF STEEL ON GROUND END SLID DOWN ACROSS FINGERS CUTTING FINGERS.
EMPLOYEE WAS USING A 2 LB SLEDGE HAMMER AND A BOLT TRYING TO REMOVE ANOTHER BOLT OUT OF THE HAMMER MILL ROTOR. HE SWUNG THE HAMMER AND HIT HIS LEFT THUMB CAUSING MEDIAL AND LATERAL LACERATIONS AND CRUSHING THE TIP OF HIS LEFT THUMB.
EE WAS FIXING A WATER LINE IN A TRENCH, EE WAS FINISHING UP THE JOB. WHEN HE STARTED WALKING BACKWARDS, HE TRIPED & FELL STRICKING HIS LEFT ELBOW ON THE GROUND.
PULLING OIL BARREL TO LEVEL IT OUT MARCH 2000. SHOVELING MATERIAL OUT OF SURGE TUNNEL PULLING GROIN AREA.
