A contract truck driver was delivering Ione clay to the plant. After dumping almost all of the material from the end dump trailer. The driver climbed inside the trailer to remove some material that was stuck to the bottom of the trailer. The driver was found unconscious and unresponsive inside the trailer.

Non injury unplanned ignition occurred at the H7-6-CM1 Day Silo.

PCS received a displaced rupture disk alarm in Forney indicating an explosion in the Coal Mill 1 system. The investigative process is continuing at this time to identify the root cause. All CM1 Recovery Phase work will be performed safely and methodically.

EE twisted their knee while walking down the stairs in the D3 Mill building. EE states they planted their right foot to turn and felt a pop and pain in right knee. EE thought the pain would dissipate but it did not.

EE states that during normal tower cleaning operations EE heard a pop in right shoulder and began to experience pain.

EE states that while changing a hose on a piece of equipment, EE began to experience pain and discomfort in the neck and back.

While cutting porta-vac hose with EE's pocket-knife, EE cut through the hose and cut left palm.

EMPLOYEE WAS CLIMBING UP THE STAIRS, WHEN LEFT FOOT WAS CAUGHT UNDER THE STAIR STEP. EMPLOYEE SLIPPED DOWN TO THE NEXT STAIR STEP AND LANDED ON LEFT ANKLE. EMPLOYEE BROKE LEFT ANKLE ON IMPACT AND MANAGED TO WALK DOWN THE STAIR ONTO LEVEL GROUND FLOOR AND CONTINUE TO WALK 20' TO A PARKED PICKUP TRUCK, WHEN EMPLOYEE COLLAPSED ONTO BACK.

EE received a laceration and tearing injury to the right index fingertip during a hoisting operation.

Employee states that while grinding steel plate EE received metal shards in EE's right knee.

PCS team recognized a temperature rise in the CM1 system. Rupture disks were then displaced.

EE informed supervisor that they were experiencing pain and discomfort in upper back and arms. EE states that they attributes the pain to repetitive impact from operating all heavy equipment.

Carbon monoxide and dust infiltrated the D-27 mill while miners were inside the mill performing regular preventative maintenance. A 4gas meter near the opening of the mill detected the CO and alarmed. All miners evacuated safely.

Lack of communication between assigned plant employee and UWCI employee about equipment lockout and rushing to complete the task of aligning conveyor belt with chain come along. UWCI employee right hand, caught between the chain and conveyor belt roller. laceration to right thumb/palm and commuted fracture of thumb.

EE states that EE has had pain in EE's back and arms since august. EE has refused medical care on multiple occasions. EE contacted EE's supervisor on 2/3/2023, and requested to be seen for the pain. EE was seen at the urgent care facility on 2/6/2023. Work status report on file - regular work status approved by physician. EE returned to urgent care for follow up 2/16/2023.

A chunk of clinker became stuck in the bottom chute of a rail car. EE reached into the opening to free the clinker. When the clinker chunk came free and fell, it lacerated the middle finger on EE's left hand.

Due to arc flash incident (loss of power) in T2-3-1, Central Control was unable to complete the H46-B1 emptying procedures. This allowed pulverized coal to sit in the bottom of the coal surge bin. The coal then caught fire.

EE was racking in a 4160v disconnect to power a system fan. As EE closed the circuit, the Limit amp shorted causing an arc flash. EE sustained burns to hands and face.

EE was hoisting roll 3 at the D4 Mill for a hydro spring change out. Chain come along broke causing EE to lose balance and bump left elbow on the D4 structure. EE initially reported to supervisor for documentation purposes. On 6/1/2022 EE requested to be seen by a physician.

EE was loading front loader cutting edges into loader bucket. EE turned to place edge in bucket and felt pain in back and leg.

Picked up an LN25 wire feed welder and heard a pop in EE's abdomen. Employee is off work until further notice. Referred to higher level care for further evaluation. Follow up in clinic 5/3/22

EE stated that EE was feeling some pain and discomfort in left shoulder from cleaning material from the finish silo chutes.

While repositioning hole cover EE stepped into the cross over duct hole and twisted leg. EE is off work until re-evaluation on 5/4/22

Stepped on angle flange 2-3 feet up and hand/footing slipped. Fell backwards, leg locked up on landing injuring right knee

EE tripped and fell while walking through the submarine in the reclaimer (dome).

EE was walking into the hydraulic carrying a 1" screw pipe. As the walking into the hydraulic room EE's foot slipped on an oily area of the concrete floor. The EE twisted EE's knee in the process of falling to the ground.

A member of the maintenance team was installing a metal pin in the pan feeder chain. After striking the pin with a sledgehammer a small sliver of the metal pin broke off. The metal entered the miners upper right chest area.

Employee was handling stair treads, the treads slipped from their gloved hand, pinching the right ring finger between stair tread and wood 4x4

Employee was attempting to pry down on a chunk of built up material from the inside wall of a vessel on the preheat tower through an open port via a tower pry bar. When the slab freed up and fell down, the momentum of the bar shifted slamming up and back down onto the EE's hard hat, knocking EE unconscious.

While EE was exiting an enclosed cab sweeper, the wind caught the door and smashed EE's right hand index finger in between the door jam and the door of the sweeper.

Our tank truck loaded bulk cement. Upon departing plant EE's truck & trailer were impacted by 2 loaded railcars at crossing. No lights or crossing arms were working. Truck and trailer were over-turned upon impact.

The operator was dumping bauxite material at the C4 area. After unloading the material at the C4 area, the employee admitted that employee forgot to lower the bed of the haul truck and struck a conveyor belt.

The employee was repairing a broken screw flight shaft. LOTOTO was not implemented for this task by the employee.

Employees were lowering a bolt template to the ground with a reach forklift and a hoisting bar. The hoisting bar slid off the forks because the locking pins were not in position. The template or the hoisting attachment struck an employee nearby. The pins were not in place because an employee earlier had installed it and the operator did not include that in the pre-task inspection.

EE reported that around 9:45am they were driving a Caterpillar 777G and hit a bump and felt pain in their right shoulder. EE was diagnosed with a sprained right shoulder on 4-21-21.

On 2-17-21 EE walked up to C3-DC2 to check on dust collector work progress. EE walked near a platform where bags were being lowered and a bag hit their hard hat. On 2-22-21 EE requested to be evaluated due to their neck being sore.

EE was traveling in packhouse elevator when it stopped at 3:24am, EE notified Central Control at that time. EE exited the packhouse elevator at 5:25am. The cause of this incident was a door lock that was jammed in the closed position. This elevator last had maintenance performed on it on 10-27-20 by Thyssen Krupp.

EE was checking flow into KS1 when they got dust in their left eye. EE requested on 8/10/2020 to have their left eye evaluated. EE was diagnosed with a corneal abrasion of the left eye and given a prescription.

EE was a passenger in an F-250 when the driver hit a rock, causing EE to bounce and hit the top of hardhat against the cab of the F-250. EE was diagnosed with a sprain of the back.

EE reported on 5/19/20 that they have been having back pains since operating loader #70 (Caterpillar 992G loader) on 4/28/20. EE was diagnosed on 5/19/20 to have a lower back disorder.

EE reported chest pains on 3/20/20 and was evaluated with no restrictions. EE was reevaluated on 4/29/2020 and diagnosed with a back sprain.

EE was troubleshooting a limit switch when the pneumatic ram actuated pinching EE's left hand. EE was evaluated on 10-31-19 and placed on modified duty.

EE reported on 10/6/2019 they had a sore back assumed to be from loading mill balls into ball mills on 10/5/2019. EE was evaluated on 10/6/2019 and put on modified duty for one day.

EE was unlocking kiln pony motor and turned around and tripped. EE hit their left knee on the concrete. EE was evaluated and put on modified duty.

EE was using an elevator at 8:10pm when an earthquake occurred in Ridgecrest. Elevator had a tripped relay that was replaced resulting in employee exiting elevator at 9:00pm. Thyssen Krupp was called out to inspect elevator.

EE was lowering a screw when it became stuck on a guardrail. EE tried moving it with foot and left hand when it moved causing the screw to shift and strike EE's left middle finger between screw and guardrail.

EE opened an electrical cabinet on an overhead crane to inspect the wiring when it arc flashed. This resulted in a burn to EE's right index and middle finger.

On 2/25/19 EE reported a sore right arm from cleaning the tower on floor 1C. EE was evaluated on 3/1/19 and given a prescription.

EE was moving a hopper of kiln dust with a forklift. While moving the hopper a port on the chute opened causing dust to come out and come in contact with employees eyes.

EE reported on 2/22/2019 they received a burn on 2/12/2019. EE stated they did not report incident. EE was evaluated on 2/23/2019 and received prescription burn cream.

EE was cleaning Tower on floor 1A when dust blew toward them. Causing dust to get under face shield and safety glasses. EE had dust removed from under eyelid.

EE was measuring a section of rail. EE's tape measure got stuck so EE reached down to pull on tape measure. While doing so the rail shifted and hit the steel toe of EE's left boot.

EE claims they cut their right arm on an unknown object. EE had an abrasion on right arm and received a prescription.

Contractor was tightening a bolt with a wrench when the wrench slipped and hit the contractor in the head.

EE reported their right thumb was sore on 5/29 from operating the Caterpillar D8. EE was evaluated on 5/29 with a strained right thumb and put on modified duty. On 5/31 EE was evaluated and released to full duty.

EE was climbing down a ladder on Crane #2 and slipped. EE caught self with EE's left hand. EE was evaluated on 5/15/18 with a strained left shoulder. EE was put on modified duty on 5/16 and was released to full duty on 5/23.

EE was using a screw driver to unscrew the light cover on the 6-5 drill and felt pain in their right thumb. EE was evaluated on 1-3-18 with a strained right thumb and put on modified duty.

EE was opening a vessel door on Tower Floor 1E and in doing so EE's finger got caught between the door and its latch. This resulted in a fracture on the tip of EE's Left middle finger.

EE was moving a box from a cart to a pallet. While doing so the box slipped causing some discomfort in EE's left wrist. EE had left wrist evaluated on 10/23/17 and was diagnosed with a sprained left wrist.

EE was sweeping in the crusher basement and tripped over studs that were protruding from the ground. The EE lost their balance and in their attempts to regain their balance they strained both knees. The EE was taken for medical evaluation and placed on modified duty.

EE was moving a hydraulic ram in a storage connex and felt pain in lower back. EE was evaluated and put on modified duty.

EE was driving a mule and felt material get in their left eye. EE flushed out eye and requested a medical evaluation. EE was given a prescription.

Employee was using a vise grip to secure a piece of plate while they were drilling holes. The drill became bound up in a hole spinning finger against the vise grip, pinching pinky finger against the vise. Employee was given pain meds, antibiotics, and removal of fingernail. Returned to work the next day.

3 contractor employees were using the Packhouse elevator when it became stuck. The contractor employees were out of the elevator at 11:22am.

EE stepped down 2 feet to lower level at reclaimer and felt a sharp pain in back. EE was taken to clinic and received medications for pain.

EE was using a hammer to remove a bolt. EE had left hand holding onto bolt. EE hit left thumb with hammer.

EE was attempting to put a tooth on a loader bucket when they felt a pain in their right shoulder.

EE was traveling in Mill Building elevator when it stopped at 1:40am and would not move. EE was extracted from Mill elevator at 3:20am. Thyssen Krupp elevator technician determined fault to be caused by blown fuse.

EE reported to the supervisor they twisted their knee while setting cribbing down on 12/20. EE was evaluated on 5/9/17 for a continued sore knee and given a prescription.

EE was using a hammer and wedge to remove a rotary feeder end plate. EE struck left thumb with hammer.

EE was cleaning the burner pipe and reported a sore back. EE was evaluated to have a sprained lumbar.

EE hit hand while opening a kiln door. EE had hand evaluated on 11-10-16 and was put on modified duty.

EE was skiving a belt in preparation to splice the belt. EE was using a box cutter in right hand, box cutter cut through belt and gave EE a laceration above left knee.

EE was cleaning tower on Floor 1B. While cleaning employee backed into an I-beam and over time noticed their left knee and left shoulder and right wrist were sore. EE was diagnosed with a sprained left shoulder, left knee and right wrist.

Miner fell through ladder opening in scaffold onto deck below. Miner sustained bruises to upper and lower back. Was transported to hospital for evaluation and released three hours later with pain prescription. Miner returned to work the next day.

Miner reported on 8/21/16 that they were moving a heavy object on their own on 8/20/16 when they felt a pain in their abdomen. There were no witnesses to this happening and miner told no one of it until the following day. Miner went for evaluation on 8/22/16 at a nearby occupational clinic. The doctor then diagnosed miner with an umbilical hernia.

EE was replacing a roller bearing when the roller frame came down and struck their left thumb. EE was diagnosed with a left thumb fracture.

EE reported had a sore back from operating 65 drill on 7/13/16. EE was medically evaluated on 7/13/16 and put on regular duty. EE went back to doctor on 7/25/16 and was prescribed physical therapy and muscle relaxers.

EE was air lancing material off burner pipe and reported a sore right shoulder. EE declined medical evaluation on 6/25/16. EE was seen by a doctor on 7/7/2016 and put on modified duty.

Employee was assisting in removing a plug in the clinker cooler from the outside of the cooler, when EE was struck in the chest by an object. Employee was diagnosed with a bruised chest.

Employee was walking from East to West on Tower Floor 2B listening for air leaks and tripped over an uneven section of grating. Employee was evaluated to have a Left Knee Sprain.

6 employees were taking the Mill Elevator up and the elevator became stuck at 7:19am. The Mill Elevator became unstuck at 7:57am. An electrician found the elevator had an overload. Thyssen Krupp was called out and did a PM on the Mill Elevator.

During sampling some Lime Slurry got on the employee. At 6:00pm employee noticed their Right Foot was tingling and went to central control to look at their foot. Employee noticed Lime Slurry had gotten through holes/cracks in their work boots. Employee cleaned foot with water. Employee had foot evaluated on 4-9 where they were diagnosed with chemical burn on Right Foot.

On 2/16/16 Employee was descending a warehouse ladder and slipped one step down. Employee grabbed handrails and had Left Leg catch them. Employee said they were okay and declined medical evaluation. On 4/18/16 employee requested to get Left Leg evaluated as it was still sore. On 4/19/16 employee was evaluated to have a Left Knee sprain.

A Cement Truck Driver went to the scale house from scale #5 after ticket did not print and tripped, misjudging last step on stairs. Cement Truck Driver did not see last step as it was covered in a cement spill from rail car loading. Driver tripped down last step landing on the walking working surface, spraining left ankle, right knee contusion, and left wrist contusion.

EE was installing new head pin for the mill, while they were positioning the head pin it suddenly shifted although it was connected to their come-a-long. When it shifted it pinched the EE's hand causing a laceration that required stitching to close. EE was placed on modified duty.

EE was attempting to unplug a feed chute using a pry bar. In his attempt to loosen the material the pry bar slipped and pinched his right hand against the feeder. The EE reported the injury but declined medical evaluation. The following day the EE was taken to the Dr. for an xray. No broken bone but modified duty was given due to the swelling. EE has returned to full duty

EE was attempting to adjust oil valves at the roller mill. In his attempts he lost his balance and his body started to twist while leaving his hand on the valve. The EE felt a sharp pain in his shoulder. Medical evaluation was diagnosed with strained shoulder and placed on modified duty.

MINER WAS UNLOADING COAL CARS VIA BOTTOM DOORS. WHEN CLOSING AND SECURING SAID DOOR, HIS WAS CAUGHT IN THE LATCH. RESULTING IN A LACERATION TO HIS MIDDLE FINGER THAT REQUIRED STITCHES.

EE entered the cyclone area of the riser through the man way door on floor 1E. As EE was walking to his work area he said that the fan that was inside the work area blew dust into his eyes. The fan was inside to provide air movement along with an air horn to disperse airborne particulates to the outside atmosphere.

EE was walking in the cooler, while stepping down from one grate to another the EE rolled his left foot causing a sharp pain in the outer portion of his foot. Dr evaluation showed a torn ligament and placed on modified duty.

After cleaning the pre heater tower the EE went to lift the face shield that he was wearing and hot dust fell into his eyes. The EE was taken for medical evaluation and referred to eye specialist. Eye specialist removed small dust particles from the eye and reported small burn to the eye.

EE was attempting to open the quarry office door when the handle broke in his hand. When this happened the sharp edges that were created in the break cut the right hand of the EE causing a laceration that required 6 stitches. The EE was placed on modified duty.

EE was exiting the cab of the 50 ton water truck utilizing the manufacturer's ladder and lost his footing. The EE was more than half way down the ladder and fell approximately 4'. When the EE landed he landed on his feet but dropped to his knees. The EE felt pain in his left knee. He was examined on the 21st and taken of work due to the swelling in his knee.

While EE was assisting work of another contractor, a 20-lb. cylinder fell from approx. 18" striking EE's right hand and caused a laceration to middle finger.

EE was measuring cement storage silos. When completed the EE got into the passenger elevator to go to the ground floor. The elevator became stuck between the 4th and 3rd floor. No injuries to the employee. EE was trapped for 2 hours until elevator service tech arrived. Overloads for elevator had tripped.

Stripping floor when employee lost control of floor machine he reel & hit his arm on the wall. Went to doctors was place on light duty worked the next day. He never went back to original doctor. Never came back to work.

EE had right shoulder injury on 10/21/14. EE was taken for medical evaluation and returned same day with no restrictions or further treatment. On 3/11/15 EE reported pain in same shoulder, taken back for evaluation, Dr treated as first aid. EE went back for evaluation on 3/24/15 for evaluation and physical therapy to begin and pain meds prescribed.

EE was going to the second floor of the packhouse via the passenger elevator when the elevator became stuck just short of the second floor parking zone for the elevator. The employee was able to get himself out of the elevator due to the elevator being stuck just shy of the elevator door. the EE was stuck for one hour. No injuries occurred to the EE

EE was attempting to ride passenger elevator to top of silos. The elevator got stuck between floors. The EE was trapped inside from 3:24 am - 5:33 am when he was released with the help of elevator tech from Thyssen Krupp. The elevator needed brake contacts replaced to correct the issue.

EE was lowering an access ladder to board the fuel/oil truck and failed to move her fingers off the ladder before it was positioned against the truck. The EE smashed and fractured the right middle finger. She was taken for medical evaluation and placed on modified duty.

EE was descending from 977 front end loader when he heard his left knee pop. EE did not report the injury until the following morning and taken for medical eval. EE was placed on modified duty for left thigh strain on 6/23/14.

EE was cleaning up after installing new conduit, when he attempted to lift the threader he heard a loud pop in his left arm and a sharp pain in his left bicep. THe EE reported to the safety office where he was taken for a medical evaluation and diagnosed with a full bicep tear.

Employee was climbing off a drill. He slipped and grabbed the handle but still fell to the ground. The drill, including handle, had grease and oil on surfaces contributing to the slippery conditions. Employee has a strained right ankle resulting in restrictions to walking, climbing and crawling which requires the use of crutches.

EE was taking the passenger elevator in the packhouse to the top of the cement silos when the passenger elevator stopped in between the third and fourth floor. Operators were sent to check the elevator doors and all were closed. An elevator tech from Thyssen Krup was called out to get the trapped miner out. He was out of the elevator at 4:20 am.

While ARSI contractor employees were welding on the housing of B4 BC 1 the sparks from the welding landed on the conveyor and it began to burn. The belt is enclosed in a metal housing and made extinguishing the fire a challenge. The fire was extinguished 45 mins after it had started.

EE was rebuilding brakes and moving parts around when he felt a strain in his lower back. EE reported the injury to his supervisor and taken for medical evaluation and placed on modified duty.

EE was attempting to remove the build up of material from the burner pipe using an air lance when he felt a sharp pain to his left arm. The forearm became swollen and tingling in his hand.

EE was attempting to unchoke material pipe using a 2lb sledge, while swinging the sledge the EE missed the pipe striking his left leg, just above the knee cap.

EE was under a piece of mobile equipment and dirt dropped into his eye. The EE flushed out his eye but due to the intense rubbing done by the EE the eye was scratched.

Employee was stepping up onto grizzly when he tripped on the edge, coming down on his right knee.

EE finger was pinch while aligning pipe flanges, resulting in laceration and fracture.

At 3:35 pm IE reported to (PCS)that he was stuck in the passenger elevator of the mill building PCS called out an electrician at 3:40 MSHA was notified at 3:45 and elevator company notified at 3:50. Electrician arrived at the plant an 4:20 and employee was out of the elevator at 4:24pm Thyssen Krupp reported to PM elevator on 9/23/13

EE was cleaning the built up material on the burner pipe for the kiln when he said he felt a sharp pain in his left shoulder. EE did not report the injury until 8/30/13 and refused a medical evaluation. EE requested to be seen by Dr on 9/19/13 and EE was placed on modified duty that day.

EE reported pain in his lower left back. EE felt it was a sciatic nerve problem he sustained while operating quarry equipment. The EE said the pain started 3 years ago. He never reported the injury and just dealt with the pain. He finally reported the injury since he felt the pain was getting worse. EE was placed on modified duty after doctor examination.

EE sustained a sprained ankle as a result of a fall.

Injured Employee lacerated pointer and middle finger resulting in sutures from Hobart Mixer.

EE was in the process of moving a ladder, and in doing so he stepped back off the concrete slab and lost his balance and fell. He was taken in for a medical evaluation and it was determined that he strained his Achilles tendon.He was taken off work for 2 days and returned on modified duty for 2 days. He has since been released to full duty.

Employee was attempting to lift the boiler gun off the tool room table and in the process of doing so began coughing very hard and felt a strain in his abdominal area. He noticed a bulge and attempted to push it back in. EE was taken for examination and diagnosed with a hernia. EE was given modified duty until surgery can be scheduled. EE called off the next day due to pain

EE was assisting with installation of Rock Knockers on water truck. While exiting form under the truck the EE struck his head on the spray bar of the truck. EE reported accident but refused medical evaluation until 9/28/12. At that point the EE was placed on modified duty for 1 week.

In the process of tying off, employee stepped back, into hole, fell approximately 35 feet onto a pile of clinker fines.

EE slipped and fell while attempting to pivot tower hoist- resulting in fractured ankle.

EE slipped on inside of kiln shell resulting in a broken ankle

EE was attempting to extract a 30 ft pipe from the ground using a backhoe. The EE rigged the pipe with a 30' sling and attempted to pull the irrigation pipe out of the ground, while attempting this the rigged end of the pipe broke free striking the EE in the head when it entered the cab. The laceration required 7 stitches to close.

Broken segments were found in the mill and believed to be the source of ignition which was conveyed via draft to he dust collectors. Since the coal mill rejects were high, it was likely the grinding of coal within the mill ceased and dust levels inside the mill were not significant enough to ignite.

Employee was using a scraper to remove heavy build up of oil and grease off the gearbox. The employee extended the scraper beyond the guard and making contact with the clutch. The scraper kicked back at the employee causing a laceration to the hand that required stitching to close. EE was taken off work.

Smoldering coal was believed to be in the VF 4 of the coal silo. Plans to drawn down the silo at its normal rate while monitoring the temperature of coal drawing the silo was implemented. Once coal silo was near empty water was applied to the coal to eliminate the hot spot of coal. The coal had not fed from VF 4 for quite a while. The hazard was eliminated without incident.

EE & contractor were removing a bearing contractor hit bearing with hammer causing piece of metal to chip off and enter the right leg of EE. EE didn't report injury until 10-21-11 EE taken to clinic Dr cleaned wound. EE was prescribed antibiotics on 10-26-11.

Employee drove front end loader into a dip in roadway causing the loader to bounce which caused the employee to strike his abdomen on the steering wheel and along with tension from the seat belt resulted in an aggravation of an off duty injury.

On 7/26/2011 the employee indicated to his supervisor that he had injured his back on 7/22/2011 while changing out a bearing and coupling on a reverse air fan.

While replacing a guide vane liner, the employee's left index finger was cut when the top half of the liner fell on the employee's hand which required medical treatment.

Walking to work area.

EE caught finger in the hoist block - resulting in sutures and fracture of the right fourth distal phalanx.

Employee was sorting through metal plates, in a storage rack, looking for a sheet of expanded metal to use. As the employee separated the metal plates in the rack, his fingers were pinched between two plates.

Received report from Employee's doctor on 5/17/2011 that, as a result of a slip and fall on December 29, 2010 while clocking in on the company time clock, the employee has been placed on disability.

On the date of the injury the employee was working under an electric cart replacing the elec motor. When he stood up he felt pain in his lower back. On 10/6/2010 the employee went to Dr. for assessment and was given a prescription for a muscle relaxant and placed on light duty.

Employee had a finger cut when it was pinched in the door frame of a front end loader when the door was blown shut by wind while he was in the process of cleaning the rear window of the loader. The cut required three sutures.

Employee indicated to her supervisor on 9/22/2010 that she injured her elbow while closing the bottom door of a rail car on 9/08/2010.

EE was lifting a piece of channel iron to be installed on a shop wall. He was about half way up a six ft. step ladder. He lost his balance. He stepped backward down to the ground to regain his footing, but, the piece of channel he was holding remained on the top rung of the ladder. His chin struck the iron on his way down, resulting in a laceration and two cracked teeth.

Employee reported soreness in his back on 6/17/2010 after finishing his shift, having three days off on regular rotation and taking some vacation time.

High winds caused a high voltage line of Southern Cal Edison to arc causing the electrical power to the plant to drop. As the power dropped an employee was leaving his work area via a personnel elevator. The employee was not abe to exit elevator until power was restored. The power line arc also caused a grass fire on property that burned uncontrolled greater than 30 minutes.

Employee was in the cab of a shuttle car completing his pre-op check when the shuttle car he was in was struck by a rail car that was under the control of the Union pacific Railroad. The employee suffered a whiplash and was prescribed a muscle relaxant by a medical doctor.

Miner was using pry bar to align bolt hole, the bar broke as pressure was applied by miner causing bar to strike him on the right side of his face. Cosmetic sutures were applied to the miner's right cheek and the Miner was released for full duty without limitations on the day of the incident by cccupational medicine center.

The injured party was preparing to measure a silo and upon removal of a vent cover, material was exhausted by a gust of air through the vent into his face. PPE was in use at the time of the incident.

Employee received disciplinary action and as a result claimed mental stress. As a result of this being a potential for workplace violence the employee was taken off work and missed first day of scheduled work 9-28-2009.

Employee was struck in face by blowing Cement Dust.

Walking down the stair and stepped wrong causing victim to fall and hit his right arm on step.

Employee jumped off knuckle of Rail Car and injured his right knee.

Incident report was reported on Monday 07/20/2009. Employee was treated for dehydration by medical facility.

Employee stated that he was working in an area that caused discomfort (ringing) in his ears. Employee supplied company with a letter from his doctor listing a requirement of restricted duty on 6-1-2009 indicating a restriction of work duties not to include high noise

The plant received information on 2/26/2010 that medical charges were received for treatment conducted in January of 2010 for a condition that was identified by an employee that was a result of a 4/28/2009 injury where the employee struck himself on the backside of his left knee with a bucket of material samples as he was carrying it for processing.

Employee states that while he was helping another employee to to move some hydraulic jacks around on a pallet for shipment, he was squatting down and felt pain in lower back.

Employee stated that while he was walking down a flight of stairs his right knee started to hurt.

Employee was coming to work. While driving on the entrance road which was icy, he went into a curve and lost control causing him to hit an oncoming cement truck. He was transported to the hospital with contusions, a dislocated little toe, one broken rib and fluid on a lung. He was released on 12/30/08.

Employee was removing plow bolts on a loader bucket wear plate and he placed his hand on the head of the bolt to keep it from spinning. The bolt did spin cutting through his glove into his palm on his right hand. No sutures, restricted duty only.

Employee states that while he was going to get into a pick-up truck, he pulled on the door handle and the door stuck and he strained a muscle in his chest and shoulder. He was released to work without restrictions. He returned to the doctor on 8/22/08 and the doctor ordered some physical therapy.

Employee states that while he was climbing into the grader, he felt pain in his lower back.

Employee was operating a 9" drill when he the compressor went bad causing a lot of smoke to exit the engine compartment. He deployed the fire suppresion system which caused more smoke and he then exited the drill by jumping over the handrail rather than using the ladder hurting his lower back and ankle.

Employee was removing bucket teeth from a 992 cat loader. He had removed the nut and was hitting the tooth with a sledge hammer when a piece of tooth broke off striking him in the leg. The piece of steel had to be removed by an out patient surgeon on 5/23/08. Lost no time, modified duty only.

While employee was talking to two other employees, he said he felt dizzy and then he fainted and fell to the floor face first. He received a broken nose and some loose teeth from the fall. He was taken to the hospital for testing and observation to find out what happened. After all the test it is still unknown what caused the fainting, possible seizure. Testing ongoing.

Employee was unplugging a chute and while he was hitting the chute he had his hand resting on the air ram for the diverter gate and when the plug broke free the diverter gate opened causing the air ram to to pinch his right hand. He factured his middle finger and has a small laceration that required sutures and abrasion.

While changing out the burner pipe on the discharge of the kiln, an ignition of coal in the preheat tower causing an explosion in the preheat tower. Dust blew out the burner pipe opening.

Employee was using a hydraulic jack to spread open a drive link for a pan feeder. A spacer slipped out from the jack hitting employee on the upper lip causing a laceration that required 4 sutures.

Employee was working under a chinker material drop box when the air cannons went off causing hot material to fall from 10 - 12 feet landing on him and causing 3rd degree burns to his arms.

Employee states that while he was holding onto a piece of conduit, while cutting with his other hand he felt pain in his left shoulder. He did not report this until 05/01/07 and went to the doctor on 06/07/07. Employee started losing time due to surgery on 10/16/07.

Employee states that a contract employee hit him with a bolt in the testicles that bruised them and he became very upset and his blood pressure went high and he was told to see his doctor. His doctor took him off work for 2 days. I did not receive a report until 1/31/07. No witnesses to the bolt event.

Employee was removing a pittman arm from a clinker cooler using a 1 1/2 ton com-a-long. When the arm came off the shafts, one end dropped down onto the hand of the employee causing a laceration to his left middle finger that required sutures.

Employee states that while she was walking down a stairway carrying a computor monitor, she got to the midway platform and had a misstep and fell down onto the grating lacerating both of her legs just below the knee caps. This required sutures.

Employee states that he has been having trouble with his left hand for several months. This employee lost his right hand in a accident about 20 years ago. He has had a number of other accidents to his left hand over the years. He has been to the doctor who has placed him on restricted duty until test are done to his hand.

While the employee was removing a wear sleeve from the crankshaft of a 773 cat truck, he was using a torch to heat the sleeve, the flame caused the vapors from the cleaning solution to flash causing a burn to his nose and cheeks.

Employee was cleaning around an elevator with a port-a-vac truck. He used his hand to push a small pile of cement away from the elevator when his finger went into a hole that had developed in the elevator housing beneath the pile causing a bucket to cut off the tip of his left middle finger.

While trying to get a hopper to feed, employee was hitting the hopper with a 12# hammer. He missed the hopper and his hand hit the chute causing a fracture and laceration to his left pinky finger.

Employee was using the overhead crane to move a 6"x7' long steel shaft. The shaft had a groove cut in each end for a keeper ring and also had two steel bushing on the shaft. When he lifted the shaft, a bushing on one end slid off of the shaft striking his finger cutting off 1/2'' of the tip of left middle finger.

I/E was using a chain hoist to lower a nose casting into the bucket of a skid steer loader. The casting caught the pocket of his jacket while it was on it's way into the hopper, pulling him down onto a piece of sharp metal, cutting his left ear.

While employee was opening a hopper door, hot dust came out and fell into the top of his glove causing 1st and 2nd degree burns to his right hand and wrist.

Employee was at base of the bucket elevator floor , when a co-worker dropped a 7/8" nut on the inside of unit where employee was working. The nut struck employee on the right index finger. Employee didn't seek medical treatment until 10-15-2005

While employee was climbing down a fixed ladder his foot slipped off the rung and he caught himself with one hand, causing pain in his left shoulder. He did not want to go to the doctor until 10/10/05 at which time the doctor put him on modified duty. EMPLOYEE STARTED LOSING TIME ON 2/21/06. MRI SHOWED TORN ROTATOR CUFF - SURGICALLY REPAIRED ON 2/21/06.

Employee states that while he was lifting a 5 gallon bucket of chlorine tablets, his back started to hurt. He did not want to go to the dr at this time. On 7/19/05 he said that his back was still hurting and now wanted to go to the dr. He was sent to the dr. and was put on modified duty starting 7/20/05.

Employee was standing by the side of his cart when the wind blew the door shut on his left thumb. Employee did not request medical treatment until 07/21/05. The doctor took x-rays that were negative and refered to orthopedics. Orthopedic doctor did in office surgery with sutures on 09/14/05 and returned employee to work 09/15/05 with no restrictions.

Employee was moving steel plates with a hoist when one began to fall toward her, falling on her leg.

Employee states that while he was using a hammer to remove a bearing his right elbow started to hurt. He did not want to go to the doctor. On 5/18/05 he said his elbow was still hurting and now wanted to go to the doctor. He saw the doctor on 5/19/05 and the report was strain or sprain/tendonitis from repetitive movment. No lost time or restricted duty.

EMPLOYEE WAS STEPPING OFF AN AIRSLIDE, MIS-STEPPED AND WENT TO THE GROUND, LANDING ON THE LEFT KNEE.

EE HIRED IN 1994 WITH PRE-EXISTING BACK INJURY IS CLAIMING A CONTINUOUS TRAUMA TO HIS BACK FROM 9/94 TO 7/24/04. THIS EE HAS NOT REPORTED ANY INJURIES TO HIS BACK NOR HAS HE FILLED OUT ANY ACCIDENT REPORTS. HE HAS NOT LOST ANY TIME DUE TO HIS BACK. 9/24/04 COMPANY RECEIVED NOTE FROM HIS DR. WHO WAS TAKING HIM OFF WORK DUE TO HIS BACK. STILL NO REPORT

Employee called into work and complained of back pain on 6/25/04 but not work related. He went to his doctor on 7/2/04 and was refered to a spine specalist who on 7/9/04 said that his injury was work related. Employee did not report anything until 6/29/04 and then went to his personal doctor who did not report it as work related.

EE STATES THAT WHILE HE WAS WALKING UP A STAIRWAY HIS FOOT HIT THE TOP STEP TRIPPING HIM UP CAUSING HIM TO STRIKE THE GRADING WITH HIS KNEE CAUSING A LACERATION.

EMPLOYEE STATES THAT WHILE HE WAS LIFTING 5 GALLON BUCKETS OF CEMENT, HE FELT A PAIN IN HIS RIGHT SHOULDER. HE SAID HE DID NOT WANT TO GO TO THE DOCTOR AT THAT TIME. ON JAN. 5, 2004 HE SAID HE WANTED TO SEE A DOCTOR AND WAS SCHEDULED FOR 1-9-04. DOCTOR RETURNED HIM TO WORK AND REFERED TO ORTHOPEDIC AND HE WAS SEEN ON 1-19-04 WITH MODIFIED DUTY WORK.

EE STATES THAT WHILE WALKING DOWN CATWALK HE STEPPED ON A ROCK AND TWISTED HIS LEFT ANKLE.

EE STATES THAT WHILE HE WAS KNEELING DOWN TO PICK UP A WELDING LEAD, HIS LEFT KNEE HIT A PIECE OF UNISTRUT CAUSING A CONTUSION. EE DID NOT WANT TO GO TO THE DOCTOR AND WAITED UNTIL 9/22/03 TO SEE A DOCTOR. HE RETURNED TO WORK WITHOUT RESTRICTIONS UNTIL HE SAW AN ORTHOPEDIC DOCTOR ON 10/22/03 AND WAS PLACED ON MODIFIED DUTY.

EE WAS PICKING OVERSIZED ROCKS FROM B3-BC1 BELT CONVEYOR & THROWING TO THE GROUND WHEN HE FELT A PAIN IN STOMACH. HE DID NOT REPORT THIS UNTIL 08/25/03 AND SAID HE WANTED A DOCTOR TO LOOK AT H IS STOMACH. THE DOCTOR SAID THAT HE HAD A HERNIA IN THE UMBILICUS AREA. HE WAS REFERRED TO A SURGEON.
