While replacing a 38" return roller on conveyor C6 the roller slipped out of the bracket on one end striking the employee in the mouth knocking one tooth out and breaking two more off at the gum.

Employee was assisting in putting conveyor together. Chain slipped and pinned the employees left hand under conveyor. Injury to middle ring and little finger. Out patient surgery to install pins. Light duty
