Maintenance worker was working on the 1313 portable crusher changing blow bars. EE was operating an air chisel to break up dirt and rock material and the chisel dropped down causing EE's right index finger to get pinched between the tool and the metal bar.

Employee was preforming maintenance on the 1313 portable crusher, EE was removing the guard to the to the v-belts. When they removed the last fastener to the guard, the guard came loose striking their left forearm causing a laceration to the top of the forearm near the elbow requiring sixteen stitches.

Mechanic was working under normal conditions, he was not under any unusual stress or strain. He was installing a drain plug in a wheel loader, wheel differential, which is on a vertical plane on the face of wheel hub. He was turning a ratchet, an felt a pop in his left bicep muscle and the muscle was at least partially torn.

EE's right ankle rolled to outside when he stepped down from control house @ Portable Crusher #1 and stepped on some sand that filled in an area around the ground rod--sand gave way under his weight. Result: sprained right ankle and injury to distal fibula.
