Miner was walking and rolled EE's ankle. EE did not trip or fall over anything or step up to nor down to any level. EE was walking on flat ground. Conditions were clear and dry at the time of event. Employee admitted to HR that EE has a hyper mobility condition that causes this to happen often.

Premier Sand and Stone LLC · Contact with electrical current

Customer truck driver pulled to the washout station, lifted the bed of EE's truck into a overhead power line and stepped out of EE's vehicle. The driver touched the side of the vehicle and was electrocuted. EE was knocked to the ground but got up and attempted to start washing out the bed of EE's truck. The only visible injury was a burn on EE's hand.

EE was a temp employee. Friday morning EE appeared to be paranoid and irritated claiming "someone was after EE." At 1:28pm EE was walking in the parking lot and fell to the ground between 2 cars and seemed to be having a seizure of some sort. 911 was called immediately.

The Hayden Group Inc. · Caught in, under or between a moving and a stationary object

Employee put foot on moving conveyor to give self leverage while moving bags and caught foot between the return chain and the frame of the conveyor. Employee was able to remove foot from conveyor and take off boot where ee noted foot was red and slightly swollen near toes.
