Employee was adjusting the bowl on Omnicone 1560 crusher when EE's right thumb was caught between two adjusting cylinders. EE's right thumb was fractured.

The employee was entering a rail car to clean out material. While stepping down from bottom rung of ladder the employee rolled and injured ankle. Treated as first aid initially, but employee indicated days later that healing was not progressing. Sought medical care on 10/31/16 when learned there is a fractured bone in left foot.

EE was using a pry bar to free up a blockage in the Rip Chute. The employee began to pry the rock upward in an effort to dislodge. At that time some rock began to move, forcing the pry bar down and catching EE hand between the bar and the edge of the opening at the door on the rip chute.(PPE, including impact resistant gloves were in use at time of incident)

Employee was working on an Atlas Copco drill at Martin Marietta Quarry in Hatton, Ar. The catwalk he was standing on has the gator type tread on it. Employee was attempting to remove the hood off engine compartment and when he went to twist his foot, it did not move because boot caught the tread and caused his left Knee to pop and swell.

The night of 2-20-13 a winter storm left snow and ice at the quarry. After the morning meeting on 2-21-13, employee proceeded to walk towards mobile equipment using a set of steps that were ice covered. Holding onto hand rail he fell twisting left knee. Employee worked but knee kept bothering. Work comp approved an MRI on 3-25-13 MRI results shows a tear at dr appt on 4-2-2013.

Commercial truck driver who had been trained & signed off on Hatton Site Specific Awareness training to stay in his truck at all times got out of his truck to release a rock from the tailgate. He lifted the tailgate with his left hand & removed the rock with his right hand. As he did this the tailgate went shut pinching his right hand.

Employee used a box knife to trim a patch to repair a hole in conveyor belting. The employee was pulling the knife toward him and it slipped, causing a 1" laceration on his right thigh.

Employee was using a grease pump to grease the Cat water truck. The pump air locked three times. He shook the pump two times with it in place to release the air lock. The third time he grabbed one side of the pump and the front to lift it off the barrel. When he set the pump down it smashed his thumb.

Employee was cleaning dirt out of an idler roller on the Cat 330 track-hoe using a spade shovel. When he went to make another swipe he caught his right middle finger on some broken fiberglass on the cab caushing a v shape laceration that required nine stitches

Employee was installing a driveshaft in a Off highway truck & was laying in the bellypan, holding the drive shaft in place while he started the bolts. He felt a pop in his back. He didn't report the incident to us for 3 days and we sent to the doctor to be checked out.

EE WAS REMOVING THE BELLY PAN ON A D-155 DOZER. HE HOOKED HIS CHAIN TO THE HINGE ON THE PAN AND PROCEEDED TO REMOVE THE BOLTS WHEN HE ACCIDENTALLY REMOVED THE BOLTS TO THE HINGE, CAUSING THE PAN TO FALL ON HIS ARMS.

BROKE BONE JUST UNDER LEFT KNEE EE WAS WORKING IN THE SOP THE AIR HOSE WAS HOOKED TO LUB TRUCK AND RUNNING FROM SHOP AIR. EE JUMPED INTO LUBE TRUCK TO MOVE IT WHEN HE BECAME ENTANGLED WITH THE AIR HOSE CAUSING HIM TO BE PULLED DOWN AND OUT OF SHOP BY MOVING LUBE TRUCK AND AIR HOSE.
