Miner operating a front end loader loading material into haul trucks. During the process a fire sparked inside the engine compartment. Miner noticed fire, stopped loader and shut down the engine. Miner then evacuated to the ground and left the immediate area. The fire was isolated to the loader only. No other property damage occurred.

An operator was found unresponsive in the cab of a bulldozer. A lead person drove to the area in which they were operating to pick up for shift change. Upon arrival the area the lead person found the bulldozer parked but wasn't able to get a radio response from the operator. Lead person ascended the bulldozer and found the operator seated in the cab unresponsive.

While servicing HVAC outdoor unit tripped on refrigerant hoses with the top of unit off and running. Fell into unit with left arm causing a laceration to the left wrist against the blower motor fan blade.

EE completed pre-op inspection on loader. Our Investigation indicates EE fell from standing position on ground prior to ascending the loader ladder suffering injury to right hand, forearm and forehead. Environmental factors (weather, ground) and loader conditions good and non-contributory to this incident. A pre-existing medical condition likely contributed to the fall.

The employee was performing a common task of knocking out rocks from a screen deck, using a sledge hammer. Employee safety glasses began to fog so EE slid glasses down just a little so they were not touching forehead then continued striking the lodged rocks with the hammer when flying debris struck EE in the right eye.

Employee was removing rock from catwalk on a rock crusher. During this process EE lifted a large rock overhead to roll it into the bin. In this process the rock rolled back toward EE as EE attempted to deflect the rock, EE received an injury to right pinky finger.

Employee operating water truck stopped at fuel island to fuel water truck. Upon exiting the cab, the employee was closing the door and had his left hand placed in the door way. While shutting door he slammed door on his left thumb, causing a one inch laceration requiring 10 sutures to close.

EE was dismounting CAT 980 G Loader in prep. to grease machine. During dismount EE's rt. foot slipped off 2nd to last step and fell backwards to the ground. EE extended lt. arm behind him to break impact of his fall. Resulting in a fractured wrist. This will require EE to have surgery on left wrist on 5/16/12.

Employee was using a 1/2" drive air impact wrench with a swivel socket extension to loosen bolts on a wash screen to change screen cloth. Employee failed to wear provided gloves and grabbed the swivel with his right hand and twisted it while applying air to the impact gun. The swivel caused a 3 cm laceration requiring 9 stitches to close

Employee was washing out under station #2 when a rock approx. 7" diameter bounced off scalp screen falling approx 20' striking employee on top left side of hard hat. The force drove hard hat down on EE's head causing bump and laceration to left side of head. Upon being examined at ER the Dr. administered 5 staples to laceration on employee's scalp.

Employee was manually extending the service truck boom out to install cylinder head on 769 water truck. The boom lock did not catch. The boom extension came all the way out and fell onto shoulder pushing him to the ground. The bolts holding boom lock plate were broken causing extension of boom to completely come out.

Employee had finished a repair on a piece of mobil equipment and was putting his tools away into his field service truck. He was putting a puller rod back into it's protective PVC tube, The rod was sliding down into the tube and he put the end of thumb on the smooth rod and it pulled it into a pinch point and pinched off the tip of his right thumb.

Heavy rains made work area wet & muddy, the floor had been swept though still wet, along with beams and soles of employees shoes. Employee's fingers slipped off beam, fell approx. 3.5-4' back onto diamond plate floor. Contusion/bruise left elbow. X-rays negative; ER requires visit to Orothopod.

High ambient temperature day. EE, a field service technician, was repairing heavy earthmoving equipment. When EE got home that night he was very tired and sat down and fell asleep. He awoke a couple of hours later with symptoms of heat exhaustion. He went to an emergency room for treatment.

Chain fell into flywheel shaker, tightened up, then slapped employee across both legs in shin area. Employee jumped down & got out of immediate area experienceing pain in legs. Gash in right shin.

Employee was repairing rubber wash down hose, Forced king nipple into open end of hose. Holding one end of hose with one hand and king nipple with other hand. He forced king nipple into open end of rubber hose, he felt pain in his left shoulder. He reported incident, felt he would be ok after a nights rest. Next morning pain persisted he was taken to doctor.

EE STEPPED UP FROM WALKWAY TO BACK OF THE ROCK BREAKER. WHEN HE STEPPED UP, PAIN WAS INSTANT IN THE MID TO LOWER PORTION OF HIS BACK. EE THEN NOTIFIED SUPERVISOR. EE SAID HE COULD WORK THE REST OF THE SHIFT. NEXT MORNING EE'S PAIN & STIFFNESS VERY BAD. PLANT MGR. TOOK HIM TO PHYSICIAN: MEDICATION AND PHYSICAL THERAPY FOR 1 WEEK AND WAS RELEASED TO RETURN TO WORK SAME DAY.

HOISTING MOTOR UP INTO POSITION USING A CHAIN-FALL COME ALONG THE RIGGING. SLIPPED AND THE 250 POUND MOTOR FELL APPROX 2 FT AND HIT THE EE ABOVE THE KNEE AND DROPPED INTO A WHEEL BALLRELL, WHI CH CAUSED THE WHEEL BARREL TO TIP AND HIT THE SAME KNEE AT THE KNEE CAP.
