Employee was using a 2ft. pry bar to raise a tube into position to be welded. The bar slipped causing EE to pull the bar towards self, striking the safety glasses, which then rolled into EE's eye. The employee was prescribed prescription eye drops which resulted in the incident being a medical reportable injury.

Closing door on company field service truck and right hand was holding the handle on side of truck while stepping down and pinky finger was exposed (not closed in hand holding handle) and tech closed the door on finger. Finger tip laceration.

Employee was dismounting haul truck to help work on conveyor belt. While climbing down the ladder (with safety harness in one hand), he missed the last step and fell to the ground. He injured his wrist while trying to catch himself.

Employee diagnosed w/soft tissue neck strain after hitting pot hole on quarry road. Doctor said to rest & return Mon. regular duty. Did regular job on Mon. attended training class Tues. On Wed. 2/08/06 employee thought he might need to go back to doctor; did not have any pain or a headache, but some stiffness in his neck. Physician scheduled physical therapy.

Employee was climbing the telsmith crusher frame to adjust the water spray. His foot slipped off the frame, some 2-3 feet above the walkway, and fell backwards to the side. He caught himself on his left hand resulting in a fracture to the left wrist.

EE WAS DRIVING CHIPPER HAMMER UNDER BAKELITE SEAL. FELT PAIN IN HAND. FOUND 1/4" LACERATION IN HAND. X-RAY LATER DETERMINED A METAL FRAGMENT IN HAND. EE WAS SENT TO A HAND SPECIALIST FOR FURTH ER EVALUATION.

INJURED EE AND ANOTHER EE WERE TIGHTENING WEDGE BOLTS AT PRIMARY JAW CRUSHER WITH A 3/4 DRIVE RACHET AND SOCKET. THE SOCKET SLIPPED OFF THE NUT AND INJURED EE'S LT. INDEX FINGER; WAS MASHE D BETWEEN THE CHEATER PIPE THAT WAS ON THE RACHET AND THE BEARING HOUSING OF THE JAW.

EE WAS CUTTING LACINGS. OXYGEN LINE JUST BELOW CRIMP BLEW OPEN DURING A BURN BACK, CATCHING GLOVE ON FIRE AND BURNING HIS HAND. THE VANGUARD PROTECTION WAS NOT INSTALLED ON THE UNIT AFTER IT H AD BEEN REBUILT.

EMPLOYEE WAS HOLDING A PIECE OF METAL THAT WAS BEING WELDED TO A BUCKET. A LOOSE FIREBALL FROM THE WELD GOT DOWN INTO HIS SHOE. THE FIREBALL BURNED THROUGH EMPLOYEES SOCK AND INTO HIS SKIN. DI VISION OFFICE WAS NOT NOTIFIED UNTIL 9/19/01 OF EMPLOYEE'S INJURY. EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 9/19/01 WHEN PLANT MANAGER INSISTED HE GO TO DOCTOR.
