Employee was holding a wedge used to remove a stuck sheave pulley on a motor shaft. Another employee was striking the wedge with a 4lb. hammer and the hammer missed the wedge and struck holding employee in top of left hand. Employees were wearing full PPE, but hammer strike resulted in fracture of left hand.
Employee was exiting break room on a 2-foot ladder and missed bottom step injuring back, handrail was available.
Employee was assisting with supplies in the area where two welders were welding. Employee stated they did not look directly at the welding light but due to being in area while welding was taking place, the reflection of the welding light caused their eyes to be subjected to arc flash.
Employee picked up a piece of flat metal off back of welding truck. When employee began to carry metal to the work area, they felt an intense pain in their abdominal region. After medical review, employee was diagnosed with an abdominal strain.
Employee was in haul truck being loaded with material in pit. Loader was dumping material in bed of haul truck when a large boulder dropped out of loader bucket into haul truck bed and caused haul truck to shift abruptly. Thus, resulting in a strain to haul truck operator's neck and shoulders.
Employee was replacing a roller on conveyor. They lost grip of roller and it fell and struck their right hand.
Temp worker was un-bridging rock from C4X front feeder. After they un-bridged the feeder and was standing to the side and observing the feeder, a rock rolled off the conveyor belt and hit temp worker in left leg. The temp did not seek medical attention and finished shift. The next morning 11/8/2022, temp went to dr. and returned to work on light duty.
EE was changing a bearing. The bearing housing slipped and smashed EE's left ring finger. EE was a temp and EE's assignment was ended on 6/8/2022 because EE didn't follow instructions on going to the medical facility to have the injury looked at.
EE was cutting on a conveyor belt with a pocket knife slipped and caused the knife to fold cutting EE's left index finger. EE had 4 stitches. EE didn't use the proper tool to cut the belt, so EE was let go on 3/25/2021. EE was working through a temporary service.
Employee was putting in feeder bottoms and twisted then back started hurting. Employee didn't want to go to the doctor, so they waited till today 9/3/2020 for us to take them to the doctor.
EE was moving the beam for the HP500 to set on. When EE reached for the beam EE's arm popped.
Employee was walking down the stairs of C6 conveyor when one (3rd step) of the stairs gave away and employee fell through the stair well.
EE was removing a guard on the counter weight of C4X when the guard pinched right hand middle finger.
EE was walking on a catwalk and carry come-a-long and right foot slipped and ee slid on catwalk landing on left knee.
Was climbing down the ladder on the cyclone when he stepped off the ladder his right foot stepped on a piece of tube metal that was laying on the platform and turned his ankle.
Employee was stepping backwards off ladder on loader and his knee popped
Cleaning the scales and he jump under the scales landing on small pile of rock twisting his ankle
Was setting a bearing housing onto a stand when it slipped and smashed his left hand middle finger.
Greasing the bobcat bucket when the bucket came off striking his right foot.
Was using a crescent wrench when the wrench slipped and hit his mouth and tooth.
Was placing a screen on the shaker when he lost his grip and the screen slipped landing on his right foot
Beating in a wedge when the hammer slipped off and smashed his finger between the shaker and hammer. Smashing his right index finger.
He was taking out kingpins when his hammer slipped and smashed his finger.
He was under the gyro crusher cutting bars out, when a bar fell and hit his foot, right behind the steel toe of the boot.
Was changing screen on 6x20, slipped and hurt his back. Didn't go to the doctor till 1/13/09.
Was backing the take up off with 3/4" impact wrench when the bolt broke causing the impact to smash his fingers against a piece of metal on the frame.
He was parking the loader in the equipment lot to get on to another piece of equipment. When he came down the ladder and placed his foot on the ground twisted his right ankle.
Tighten bolt on bottom of gear box at a head pulley with wrench when the wrench slipped off a cut his left elbow on a guard.
Was removing a big rock from the grizzley with a bar and pulled upward on the bar hurting his back
Hammering norback when the material fell and a piece fell on his ankle and foot.
Was putting pin in the hyd. cylinder and got his hand between the cylinder and the cylinder head and smashed his right index finger.
Stepping off loader twisted knee. He waited to go to the doctor thinking that it would get better.
Lowering a ladder into the gyro to check worn concaves and didn't notice that the safety cable had tangled around his ankle and he released the ladder into the gyro causing him to lose his balance and hurt his left knee.
Unbridging SB2 bin when mud clot came down and bent left hand back hurting his wrist.
He stepped off of the fender of the volvo, lost his balance and fell.
Lifting the cap of the HP 500 pinched his right little finger between the cap and the crusher.
Pulling motor back with come-along when the hook broke as he was pulling on the handle, causing him to slip and cut his hand on a piece of metal.
Cleaning with the walk behind bobcat under a conveyor when the slab was uneven tipping the machine and smashing his finger on his right hand against a beam.
Contusion to the right shoulder when a port a power jack fell while squaring off the sprider on the gyro
Cleaning tail pulley with shovel, he hit frozen dirt which kicked back the shovel putting pressure on his back.
Changing flashing on C4Y when his wrench slipped and he cut his arm on a piece of metal.
Backing haul truck under loader bucket. The hyd. failed and the bucket came down on bed of truck. Ee twisted his neck.
Taking out the hyd. adjustment on HP700 and it fell on his foot.
Washing mud out of gyro, when belt was started up a rock fell out of gyro & hit the belt & bounced off hitting ee's little finger on handrail.
Driving a screen wedge with a 4 lb hammer on the left hand bottom deck 6x16 and missed the wedge and hit his finger on the bar grate cat walk.
EE WAS STACKING BATTERIES. WHEN LIFTING A BATTERY, EE HURT HIS LOWER BACK.
STEPPED OFF CONVEYOR FEEDER BOX, FOOT WENT SIDE WAYS AND LANDED ON RIGHT ANKLE.
WAS SITTING IN LOADER WENT TO GET OUT AND TWISTED HIS BACK.
HAND SLIPPED WHEN HAMMERING A WEDGE ON THE 6 BY 20 AND HIT THE GUARD.
WAS PULLING SCREENS ON THE 8 BY 20 WHEN HE PULLED HIS BACK.
DROPPED A PIECE OF 1" X 2" X 3" METAL ON RIGHT FOOT ABOUT 3 WEEKS AGO. WASN'T REPORTABLE UNTIL 10/24/02 WHEN HE WENT ON LIGHT DUTY AFTER AGGREVATING THE INJURY BY STEPPING ON BRAKE PEDAL OF WE LDING TRUCK, RIGHT FOOT POPPED.
WIND BLEW DUST IN EYE CAUSING A SCRATCH.
DROPPED A PIECE OF SQUARE CHANNEL ON HIS RIGHT FOOT WHILE REPLACING A BEARING ON A SHAFT.
WHILE BEING LOADED, THE END DUMP DRIVER HAD HIS WINDOW DOWN WHICH ALLOWED TRASH TO BE BLOWN INTOEYES.
CLIMBING DOWN STEP ON A CAT 769 WATER TRUCK AND MISSED THE LAST STEP.
PIN CAME OUT OF LIFT ARM ON TERMINATOR AND BOOM FELL FORWARD AND JARRED EMPLOYEE IN LOADER.
CAUGHT LARGE PIECE OF METAL WHILE METAL SWINGING AND CAUGHT ARM BETWEEN METAL AND WALL.
CHANGING A SCREW ON A 6 X 20 SHAKER. LIFTING SCREEN IN PLACE AND STRAIN LUMBAR AREA OF BACK. (REPORTED TO SUPERVISOR ON 3/7/00).
INSTALLING NEW ENGINE IN CAT HAUL TRUCK. HE WAS HOOKING UP HOSES WHEN MATERIAL FELL INTO HIS EYE.
USING DISC GRINDER ON PIECE OF METAL. PARTICLES FLEW BEHIND SAFETY GLASSES.
