At the time of the incident, the injured employee had just cut a sheet loose and was trying to pry it up, they went to stand up and missed a step, stepping off the edge of the sheet and fell through the opening from the first removed panel. The crew had previously hung self-retracting lifelines for tie-off, but the injured employee was not tied-off at the time of the fall.

The injured employee was attempting to blow argon through the VOCL3 transfer line to clean out the system's transfer pump. When they charged the line with argon, it ruptured the filter housing, spraying hot VOCL3 over the lower half of their body, burning their left ankle.

The EE was working on a 60" shaker screen and as EE was installing a section ring, it slipped causing EE's left forearm to slide across the edge of the unit resulting in a laceration to EE's left forearm that required stitches.

The injured EE was helping free a frozen gate latch on the bed of a Mack dump truck. EE inadvertently placed EE's hand between the gate latch and the truck bed and as the air latch closed it pinched EE's right-hand pointer and middle fingers between the bed and gate latch.

The injured employee was installing a fill valve assembly on a product tank. As the employee was adjusting the valve with a 24" pipe wrench it slipped off the valve assembly causing employee to fall forward into the side of the tank. As employee fell forward felt a pop in right shoulder.

The injured employee was working on top of the #1 Crystallizer at MVO. As ee took a step ee lost balance and put right hand down to help steady self. EE hand flexed open a thin sheet metal joint and right hand ring finger went into/under the sheet metal cutting the tip and side of finger.

The injured employee was re-stacking a pallet of fiber drums so he could load them on a sea container for shipment. As he was lifting on one of the drums he felt something pop in his right elbow. Employee requested to see the Dr. on 12/16/15.

The injured employee was helping rebuild a Denver pump. The pump assembly slipped out of the workbench vise and landed on the employee's left hand smashing the tip of his middle finger between the work table and pump shaft.

The injured employee was using a utility knife to cut/split a 1/4" rubber hose. He was holding the hose in his left hand and cutting with his right hand. The knife came out of the rubber hose and cut through his leather glove and into his palm.

The injured employee was replacing the seals on the small pod. As he was positioning the west end housing back into place, a small amount of caustic cleaning solution got into his left boot causing a chemical burn to his lower leg and ankle area. (Deemed reportable on 8/19/2014)

The injured employee was working as part of the maintenance crew assembling and installing new sections of piping. A section of the HDPE pipe hung on the forks of the forklift and the injured employee reached in with his left hand and attempted to free the pipe. The flange released striking/pinching the employees left hand against the forks, fracturing his middle and ring fingers.

The injured employee was helping another mechanic remove bolts from the bottom of a shaker screen assembly that was propped up on its side. As the 2nd mechanic attempted to rotate the assembly to get to the next set of bolts the unit fell over and struck the injured employee across the right knee. This resulted in a right knee contusion injury.

The injured maintenance employee was helping remove old HDPE piping from a plant pipe rack. In the process of removing the pipe he broke another line containing 50% caustic soda which sprayed him on the right side of his upper body and face. As a result a small amount of the material got behind his safety glasses and into his right eye.

The injured employee was using a shop drill press to drill holes in a piece of flat metal. During the drilling process several metal shavings had built up around the bit. The employee then reached in to remove the shavings away from the bit and as he pulled on one of them it sliced through his leather glove, cutting his left index finger.

The employee was opening a crate of shipping boxes. He removed the plastic binding straps and threw them on the ground next to the container. As he turned to walk away his feet became entangled in the straps that he had thrown on the ground, causing him to fall and land on his arm and chest.

After EE completed grinding a weld on a handrail with a 4" grinder, he raised his face shield and a small debris particle came off the faceshield, went between his eyebrow and glasses and got is his left eye. He missed one day of work for a doctor's exam to check his eye. He was treated and he was prescribed eye drops. There is no permanent damage to the left eye.

Employee was moving/adjusting an extension ladder. He got his left wrist in between the lower and upper rung sections of the ladder, resulting in a bruise to wrist. Taken to emergency room-xray (no fracture), velcro brace, ice, pain medication given. Light duty 10/20/05 (our last work day of week), full duty 10/24/05. Physician follow up visit 11/2/05.

EE was working in the warehouse, helping reorganize & clean the inventory shelves. As he was cleaning, a pump shaft rolled across an upper shelf, fell to the floor, bounced, and then struck him across his right hand, smashing his middle finger.

EE had placed a 55 gallon barrel in a barrel inverter. As he was pulling up on the handle to tighten the chain around the barrel to lock it in place, he felt a pop in his left shoulder.

As EE was shifting the welder to fit through an opening he smashed his left hand pinky finger between the frame of the welder and the handrail around the hoist way opening.

Employee was removing old pump part from inventory that had been discontinued. As he was removing a casing for a galigher pump from the bottom shelf it fell over. EE stated he then reached down and tried to pick up the casing that had fallen. As he was trying to lift it up he felt a sharp pain in his neck and shoulder blades.

A PLANT MECHANIC WAS REMOVING A 3/4" CAP OFF OF AN AMMONIA LINE TO PURGE THE SYSTEM. HE SHUT OFF THE MAIN VALVE TO THE TANK BUT NEGLECTED TO BLEED THE PRESSURE OFF THE LINE BEFORE REMOVING THE CAP. THE CAP BLEW OFF ENGULFING HIM IN A VAPOR CLOUD OF AMMONIA. THIS RESULTED IN MINOR BURNS TO HIS RIGHT HAND AND CHEEK AND INHALATION INJURIES.

SEVERAL MECHANICS WERE USING AN ELECTRIC HOIST TO LIFT A PUMP UP TO THE THIRD FLOOR OF THE MVO BUILDING. AFTER HOOKING UP THE PUMP, THE INJURED STARTED UP THE STAIRS. AS HE REACHED THE SECOND FLOOR HE SAW THE PUMP HIT AN I-BEAM AND SNAP THE CHOKER, CAUSING THE PUMP TO FALL. HE RAN BACK DOWN THE STAIRS TO CHECK ON THE MECHANICS ON THE BOTTOM FLOOR AND FELT A SLIGHT PAIN IN HIS KNEE.

HE ASKED HIS PARTNER TO HAND HIM ANOTHER BOLT FOR THE FLANGE, AND AS HIS PARTNER REACHED OUT TO H AND IT TO HIM HE LEANED AGAINST AN ACID LINE CAUSING IT TO START LEAKING AT THE NIPPLE TO THE FLOW METER. ANOTHER MECHANIC THEN TRIED TO SHUT THE ACID OFF AT THE VALVE, WHICH CAUSED THE NIPPL

THERE IS APPROXM A 10 INCH DROP FROM THE CONTROL ROOM FLOOR TO THE WORK AREA FLOOR, AND AS HE MADE THE STEP HE LANDED ON HIS LEFT ANKLE, RESULTING IN A MILD SPRAIN.

THE INJURED EE HAD RELOCATED HIS KEYBOARD TO THE TOP OF HIS DESK IN HIS OFFICE. THE USE OF HIS KEYBOARD IN THIS POSITION RESULTED IN HIS WRIST BEING PLACED ON THE EDGE OF HIS DESK DURING DATA ENTRY. THE AWKWARD POSITION CAUSED IRRITATION TO THE TENDONS IN HIS RIGHT WRIST AND FOREARM.

EE WAS FUELING A FRONT-END LOADER AT THE FUELINGSTATION. HE TURNED TO WALK TO THE PUMP AND AS HESTEPPED OVER THE FUEL LINES HIS LEFT FOOT CAUGHTTHE LINE CAUSING HIM TO LOSE HIS BALANCE. AS HE FELL ALL OF HIS WEIGHT SHIFTED TO HIS RIGHT KNEEWHICH TWISTED UNDERNEATH HIM, CAUSING A STRAIN.

THE INJURED EE WAS WASHING MATERIAL OUT OF A TROUGH AT THE END OF THE ROD MILL WITH A WATER HOSE. AS HE PULLED THE HOSE FORWARD IT BECAME INTANGLED IN A WODEN 2X4, HE THEN PULLED FORWARD AGAIN S WHICH CAUSED THE 2X4 TO KICK OUT AND GO INTO THE ROD MILL. THE MILL THEN KICKED THE 2X4 BACK OUT AT HIM, STIKING HIM IN THE SHOULDER AND GRAZING HIM ACROSS THE CHIN.

THE INJURED WAS INSPECTING THE LID OF A CLACINER, THAT WAS LOCATED IN THE SHOP. THE LID HAD BEEN TURNED UPSIDE DOWN SO THAT IT COULD BE INSPECTED AND NEW REFRACTORY WORK PERFORMED. AS THE INJU RED STEPPED OFF THE LID ONTO THE FLOOR HE STEPPED ON A PIECE OF BROKEN REFRACTORY CAUSING HIM TO FALL BACKWARDS. AS HE FELL HE CAUGHT HIMSELF WITH HIS HANDS, RESULTING IN INJURY TO R PINKY.
