Employee was exiting the confined space down an extension ladder. Upon trying to step on the 2nd from bottom ladder rung their left knee locked up on them causing them to miss the rung. This caused them to step all the way to the ground, twist their knee, and fall to the ground. The employee had pain in their left knee and couldn't apply pressure to it.

The incident was not reportable until the employee asked to see a doctor on Sept. 7. The employee was removing dust covers from the top of a railcar and struck hardhat on the dust cover holder when the employee stood up. The diagnosis was a strained neck muscle. Employee was prescribed two medications & restricted from carrying or lifting more than 5 pounds until Sept. 12.

During a mid-shift inspection of the work area, a hazard was noticed. The employee thought it to be a good idea to look further into the hazard to see if EE could fix it. Watching the kiln and assessing danger EE moved closer to the Gaurd to see if it needed removal or adjustment. EE's hand rested on the guard momentarily and the guard failed pinching EE's finger.

Mechanic was lowering an extension ladder 135 inches to the ground from a safety railed platform with toe boards. The ladder began to slide down the rail when the bracket for keeping the extension ladder in one piece slid against left index finger. Mechanic was not wearing gloves during the incident. The wound required 8 stitches to close. There are no restrictions on work duty.

Maintenance Mechanic was grinding on the #2 Shaker Pit and felt something graze left eyeball. EE did not report the incident to anyone. When ee returned to work on Sunday eye was red and irritated, ee did not report the incident to anyone. When mechanic woke up Monday 2/29 eye was swollen and red. The doctor pulled a piece of metal out of eye.

An employee became overheated after going up and down the stairs a few times. The Operator was treated with ice packs and fluids. When the operator's blood pressure and pulse stayed accelerated they were transported to the hospital for treatment and diagnosed with heat exhaustion.

Operator was contacted with hot material from the waist up while checking dust flow from the cyclone to the leach tank. After opening the knife gate valve he opened the clay bailey to check the flow. Initially the dust flow was okay but then stopped. This was followed by a large slug of material that broke free and flushed out of the clay bailey, contacting the operator.

In the course of an outage a section of exhaust duct was removed from Kiln No. 1. During the refractory bake-out combustion gases were released into building 405. The next day a Calcines filter floor operator reported that he had a headache and became disoriented, and after going home and sleeping felt like he had a hangover.

Employee unrolled a roll of 6" ads hose. Upon returning to truck, slipped on an icy patch and fell on left shoulder/ribs. Employees left shoulder and ribs were very painful, therefore he was taken to the emergency room for x-ray. X-ray was negative for breaks.

Mechanic was working on a Ballformer. While one mechanic was lying on the ground making adjustments to the roller bearing bolts, the injured mechanic was using a track bar to leverage the support roller bearing into the proper position. The track bar slipped off the roller bearing striking the mechanic on the ground just above his right eye causing a laceration.

Employee showed a right ear Substantial Threshold Shift (STS) when testing was conducted by TK Group on 10/17/2012. Employee was retested by Saline Audiology Associates on 11/20/12 and the Substantial Threshold Shift was confirmed. After review by local and corporate EHS, this has been classified as a reportable injury on 12/10/2012.

Employee was cleaning under the 405 A HyTank when his foot slipped and he bumped his knee on a quick connect, at the time he said he didn't need medical attention. On 12/14/12 employee was seen by Concentra for a potential torn cartilage. On 3/4/2013 Concentra received back MRI report. Diagnosis was consistent with the incident employee reported on 8/9/2012.

Employee, a foreman on the night shift, was working on a new platform at the level of what will be the fourth floor. The area was surrounded by barricade tape. EE attempted to enter the area by stepping over the tape and in the process - his trailing foot caught on the tape causing him to fall. His face and throat came down on the grating causing the laceration to his neck.

A Mechanic was trying to remove a sprocket from a reducer when the hydraulic gear puller came loose and struck him on the abdomen, causing him to fall on his right arm. The employee suffered a laceration to his right elbow. He was taken to the local emergency room for treatment where he received eight stitches and was restricted to light duty and restricted use of his right arm

As he was pushing a dolly loaded with fluoride, he struck his right hand against the edge of a piece of sheet metal guarding that was wrapped around a pipe. The employee suffered a cut to his hand that was fairly deep that required stitches.

A employee was cleaning the magnet located on the Tabular 112 Fired Ball belt. The employee had to place his hand and arm between the belt and magnet to clean the magnet. The cable which holds up the magnet failed. This caused the frame of the magnet frame to fall on the employee left hand.
