Arkansas Operations Mill Metal/Non-Metal

Employee was exiting the confined space down an extension ladder. Upon trying to step on the 2nd from bottom ladder rung their left knee locked up on them causing them to miss the rung. This caused them to step all the way to the ground, twist their knee, and fall to the ground. The employee had pain in their left knee and couldn't apply pressure to it.

While using a drill, the bit hung up causing the drill to spin and come in contact with the mechanic's right hand.

Mechanic was putting new belts on shaker conveyor. The shaker shaft is offset and the belt rolled back causing EE's finger to get caught between belt and sheave.

Electric grinder kicked back contacting and cutting the back of the right hand.

Operator was descaling a piece of equipment and a large chunk fell on EE's left foot causing an unspecified fracture.

Employee was using a powered, hand-held cutting wheel when the tool kicked back and struck employee in the chin causing a deep laceration and broken tooth

Operator was attempting to move a wheelbarrow full of material and lost EE's balance and twisted EE's ankle.

Initial first aid event occurred 1/26/2021. Operator then went to the doctor 02/09/2021 and was put on restrictions. Operator was moving material in a wheeled cart and felt a pull in EE's groin when EE bent to dump the cart.

Employee felt faint and disoriented. Treated and released by the hospital for heat exhaustion. Hot and humid conditions.

Employee felt a strain in EE's lower abdomen while pushing a heavy wheelbarrow

Operator slipped on fired balls and twisted knee.

Operator stepped back and tripped over a sandbag at the unloading station. EE tried to catch self and sprained EE's wrist.

The operator sustained head, rib and shoulder injuries from a fall into a railcar. The operator radioed that the operator needed medical attention but could not state location. The operator was transported to a local hospital by ambulance.

The operator felt a strain in shoulder when a bag fell off the lift assist machine ee was using. The operator did not request a clinic visit until April

Fell while walking near the tracks; tripped over a hole and fell on right shoulder and cheek. Employee did not report until 11/7/18 and it was determined 11/29/18 that the injury happened at work.

The incident was not reportable until the employee asked to see a doctor on Sept. 7. The employee was removing dust covers from the top of a railcar and struck hardhat on the dust cover holder when the employee stood up. The diagnosis was a strained neck muscle. Employee was prescribed two medications & restricted from carrying or lifting more than 5 pounds until Sept. 12.

Contractor positioned a pane of glass in a window frame. The contractor was working below when the aforementioned pane of glass fell out of the window and fell on hardhat. A piece of broken glass landed on and cut the top of right hand.

Employee shoveling fines from the tabular converter hood. The material dropped down into the converter, displacing air and blowing hot dust out the door and toward the employee. A small quantity of dust went into the employees glove and burned a hand. Employee had doctor call in a prescription at a later date making it reportable

Employee lost footing on a concrete floor that was "sweating" due to rapid change in ambient temperature. Employee twisted ankle

Employee walked into the plant from an outside area in Building 415 on 3rd floor. Dust entered their right eye and they tried to wipe it out.

Contractor employee was sitting in a training session, began to feel hot/light-headed, stood up to walk out the back door. EE walked about 15' and passed out falling to the ground and striking EE's chin on a near-by table.

During a mid-shift inspection of the work area, a hazard was noticed. The employee thought it to be a good idea to look further into the hazard to see if EE could fix it. Watching the kiln and assessing danger EE moved closer to the Gaurd to see if it needed removal or adjustment. EE's hand rested on the guard momentarily and the guard failed pinching EE's finger.

Cleaning alumina inside clay bailey opening on screw conveyer housing - Screw caught two fingers creating open fractures

Member tripped over electrical cord from a fan at building 55 hurting employee's left shoulder

Mechanic was lowering an extension ladder 135 inches to the ground from a safety railed platform with toe boards. The ladder began to slide down the rail when the bracket for keeping the extension ladder in one piece slid against left index finger. Mechanic was not wearing gloves during the incident. The wound required 8 stitches to close. There are no restrictions on work duty.

Member bent over to pick up a blue flag (Used to mark the R x R tracks of cars that are not to be moved - Weight of it is ~ 10 lbs.) Member stood up and felt a sharp pain in lower back

Mechanics were replacing the seed pipe to the # 7 Ball former on the 2nd floor of 426. Pipe rotated out of position. Mechanic tried to arrest the motion of the pipe and felt a pain in the lower back. Member given restrictions and Rx medications.

Lab operator lifted a tote weighing 15.5 lbs. and twisted at the waist not pivoting feet and felt a pull in lower back.

Lab analyst was getting into Golf Buggy with hands full and tripped and fell on both knees.

Mechanic was working on a magnet when the valve actuated pinching arm between the magnet and back plate.

Operator was climbing a ladder on the 55 Building Charge Floor Hopper when employee felt a pop in the wrist. Employee stated it did not hurt so they did not report the incident to anyone. Upon going home and waking up the next day there was swelling and pain radiating down the wrist and thumb.

Catering employee was watching supervisor light fish cooker when cooker flashed and burned supervisor 2nd employee burned hands helping supervisor

Catering employee went to light propane fish cooker when a build up of gas caused cooker to flash burning the employee on the hands, chest, back and face.

Tabular Operator was closing the bottom of the 1-2 bin and 1-3 bin before opening 1-1 bin. As ee stepped off the platform in order to cross the 135 belt ee rolled right ankle and thereby causing a right ankle sprain.

The employee was installing a trunnion guard, while working with helper. The guard was being placed into position and lined up with connecting bolt holes. The guard shifted and pinched a finger.

Maintenance Mechanic was grinding on the #2 Shaker Pit and felt something graze left eyeball. EE did not report the incident to anyone. When ee returned to work on Sunday eye was red and irritated, ee did not report the incident to anyone. When mechanic woke up Monday 2/29 eye was swollen and red. The doctor pulled a piece of metal out of eye.

Operator was having personal issues and was teased by another worker which in turn made him angry and he struck a metal hopper with his fist. This self-inflicted injury caused him to break his hand.

An employee became overheated after going up and down the stairs a few times. The Operator was treated with ice packs and fluids. When the operator's blood pressure and pulse stayed accelerated they were transported to the hospital for treatment and diagnosed with heat exhaustion.

Operator was contacted with hot material from the waist up while checking dust flow from the cyclone to the leach tank. After opening the knife gate valve he opened the clay bailey to check the flow. Initially the dust flow was okay but then stopped. This was followed by a large slug of material that broke free and flushed out of the clay bailey, contacting the operator.

In the course of an outage a section of exhaust duct was removed from Kiln No. 1. During the refractory bake-out combustion gases were released into building 405. The next day a Calcines filter floor operator reported that he had a headache and became disoriented, and after going home and sleeping felt like he had a hangover.

Employee unrolled a roll of 6" ads hose. Upon returning to truck, slipped on an icy patch and fell on left shoulder/ribs. Employees left shoulder and ribs were very painful, therefore he was taken to the emergency room for x-ray. X-ray was negative for breaks.

Mechanic was working on a Ballformer. While one mechanic was lying on the ground making adjustments to the roller bearing bolts, the injured mechanic was using a track bar to leverage the support roller bearing into the proper position. The track bar slipped off the roller bearing striking the mechanic on the ground just above his right eye causing a laceration.

The manlift the employee was in, was struck by a falling kiln section which shook the manlift.

Employee entered into a pit and stepped into material that was approx 750F. Employee received 1st and 2nd degree burns to his left foot.

Employee showed a right ear Substantial Threshold Shift (STS) when testing was conducted by TK Group on 10/17/2012. Employee was retested by Saline Audiology Associates on 11/20/12 and the Substantial Threshold Shift was confirmed. After review by local and corporate EHS, this has been classified as a reportable injury on 12/10/2012.

Employee was cleaning under the 405 A HyTank when his foot slipped and he bumped his knee on a quick connect, at the time he said he didn't need medical attention. On 12/14/12 employee was seen by Concentra for a potential torn cartilage. On 3/4/2013 Concentra received back MRI report. Diagnosis was consistent with the incident employee reported on 8/9/2012.

Miner was filling bags and using repetitive motion to check weight and place on pallet.

Employee, a foreman on the night shift, was working on a new platform at the level of what will be the fourth floor. The area was surrounded by barricade tape. EE attempted to enter the area by stepping over the tape and in the process - his trailing foot caught on the tape causing him to fall. His face and throat came down on the grating causing the laceration to his neck.

An employee became overheated after having to use a 60lbs pry bar to open 2 railcars. The Operator was treated with ice packs and fluids. When the Operator failed to respond they were transported to a hospital for treatment and diagnosed with heat exhaustion.

A Mechanic was trying to remove a sprocket from a reducer when the hydraulic gear puller came loose and struck him on the abdomen, causing him to fall on his right arm. The employee suffered a laceration to his right elbow. He was taken to the local emergency room for treatment where he received eight stitches and was restricted to light duty and restricted use of his right arm

Calcines employee was unloading chemicals on the 3rd floor Filter end. As he was pushing a dolly loaded with fluoride, he struck his right hand against the edge of a piece of sheet metal guarding that was wrapped around a pipe. The employee suffered a cut to his hand that was fairly deep that required stitches.

Employee was tested on 12/28/2011 and found to have a Permanent Threshold Shift in his left ear.

Employee underwent yearly hearing test and was confirmed to have a Standard Threshold Shift in his right ear.

Employee reported she was opening the bottom of a rail car door when she slipped and fell while attempting to pick up a piece of metal that fell from the rail car onto the knife gate.

Employee was lifting 50lbs bags from a high lift to a rework station. As he was twisting around to move the bag, he felt a sharp pain in his left lower back.

A employee was cleaning the magnet located on the Tabular 112 Fired Ball belt. The employee had to place his hand and arm between the belt and magnet to clean the magnet. The cable which holds up the magnet failed. This caused the frame of the magnet frame to fall on the employee left hand.

Injury was helping install ring in #2 cooler and pulled his back while holding ring up in cooler.

Employee was lifting up a steel door on a packing water tank when he experienced a sharp pain in his back. Pain was found to be a pulled ligament.

While eating employee got something in her right eye. Other EE's washed it out and EE still complained about pain in her eye. She was taken to Concentra where her eye was determined to have been chemically burned.

Employee was getting off a forklift from the wrong side and his tyvek suit became caught on the battery connection causing the employee to fall onto a cement floor.

Employee was walking off a designated footpath and stepped into a hole and twisted his knee.

Miner was shoveling tabular balls into a chute. When the miner finshed, he turned and hit his head on a support rail. Miner was wearing a hardhat at the time of the incident.

Miner was shoveling material out of the #6 Tabualr elevator. The repetitive motion caused him to suffer a strain to his groin area.

Employee was repairing 106 Elevator Chute leaks, when she stepped off step ladder missing bottom step. Employee sprained her neck and lower back.

Employee was walking in Bldg 426, 7th floor when he lost his footing after stepping on a tabular ball injuring his lower back. He was taken for evaluation to a medical clinic and received work restrictions.

After completing a series of tasks including supersack bagging, driving forklift, collecting samples, and housekeeping, employee was climbing a flight of stairs when he reported feeling pain in his lower back.

Bldg 405A, 4th Floor trying to get #1 Kiln Dust Tank emptied, using air lance with low pressure, Hot material blew back on employee giving first degree burn on right forearm. Employee was sent for medical evaluation and given prescription for Silvedine.

After making repairs to the Jet Mill Bagging Station, employee was watching the bagging process when the bag burst, forcing alumina into his eyes.

Right middle fingers were caught between moving bars on super sack station. Employee's fingers were mashed and swollen. Injury was treated initially as first aid, then sent for medical evaluation on 6/20/08.

Metal weight (35-45 lbs) used to dislodge scale from tank fell 10-12' and struck employee on right knee and ankle.

Employee was moving Bandsaw in B420 storage area when he felt pain in back.

Employee was working under Trays turning valve, when rubber hose blew/split, allowing caustic liquor to spray over employee's upper body, head, and eyes.

Employee was walking up metal stairs while wearing a tyvek suit. Tyvek suit limited mobility at top stair, causing her to fall, cutting her left knee. She received first aid and sent to the local ER for evaluation, where she received sutures.

Miner was using shovel handle to open latch on #14 Converter. Converter is hot and latch requires some effort to operate. He then bent down to pick up sample bucket <10lbs and felt pain in his lower back.

Failed to lower head enough when going under airslide at Bldg 415, R/R platform. Employee was wearing hard hat and bumped into airslide, causing Cervical and Thorasic strain.

Employee was returning to Control Room, when he stopped to talked to another employee. A yellow jacket landed on his arm just above the elbow and stung him. He is allergic to insect stings and his arm began to swell. He was taken to medical center for evaluation.

Employee had removed cutting goggles and was attempting to change to safety glasses when dust particle blew into his eye.

Injured lower back while shaking fiber drums at Bldg 55, Jet Mill. Felt pain for a week until 9/12/2007 when he felt intense pain while working at the A-2 Mill Bagging station. Sent in for medical evaluation.

Employee sprained right ankle while walking across Bldg 55 Warehouse floor. Employee stated that he "rolled" his ankle, no contributing causes were identified.

While recharging A-2 Mill, employee was in West access hole when access hole covering fell striking his hardhat. He refused medical evaulation until 7/3/07 where he was diagnosed with a contussion and cervical strain and released to work with restrictions.

Employee stepped off platform ladder and slipped on wet surface injuring right wrist. Injury occurred on ground level.

Employee was attempting to turn valve on Back Pressure Control, when he felt pain in his Left shoulder. Employee was using wrench and cheater bar. Employee quit upon feeling pain. Did not seek medical attention until 5/14/2007.

Employee attempted to measure the hole pattern on a Vibrator by rolling it onto its side. The Vibrator was heavy and the Employee let it back down. His right hand - small finger became caught between base and pallet resulting in a laceration and inability to straighten finger. He was diagnosed with a contusion and closed fracture of the right small finger.

While walking from Bldg 450 Jet Mill Bagging Area to Stencil table, employee slipped on wet floor with left foot. He was able to prevent himself from falling, however he did report pain in lower back and left shoulder.

Employee tripped on air hose lying across walkway in Bldg 415, 3rd floor. The area around the air hose was wet with water from an overflowed roof drain that had dripped down and mixed with alumina. Area was well lit and alternate routes were available. Employee stated that he did not see air hose before tripping.

While adjusting #3 Pannivis Filter Head Pulley, employee was tightening the head bolts when he felt burning in lower back. Employee had been working on equipment for approximately 30 minutes, loosening and tighting bolts in an effort to align the Head Pulley.

Held open pocket knife in right hand while opening bearing box, cardboard tore allowing hands to slip. Left thumb made contact with open kife blade, resulting in 4 stitches to repair wound.

Employee retrieved Pannivis Filter Cloth from storeroom. He removed the Filter Cloth (128lbs 90"X10") from storage rack, and carried it outside to truck. He stated that he felt pain in his lower back when he maneuvered Filter Cloth through door and upon putting it into the truck. Employee did complete task of replacing Filter Cloth with assistance from 2nd Operator.

Lying down in an awkward position while working #1 Filter at Bldg 400. Employee felt pain in lower right side of back, once he resumed normal upright body position.

Mechanic was carrying a piece of pipe to a job site when he tripped on a hose and fell, scrapping his face and spraining his wrist. He was treated at the local clinic and placed on restrictions.

Employee suffered a lumbar area muscle strain while using wrench to tighten and loosen 3/4 nuts and bolts on #13 Precepitator Pump. He stated that he felt pain after completing task and self treated with ibruprophen and delayed reporting injury until 8/7/2006. He was sent to doctor and taken off work for one week.

While walking in the area of the #1 Cooling Tower, Employee slipped and fell on algae growing on concrete pad, striking concrete footing and I-beam, injuring his left shoulder.

While assisting another employee manuever the charge bucket, employee was using the guide ropes to help control the location of the charge bucket. Employee was looking down when the charge bucket was lowered striking him on hardhat.

Miner was using impact wrench to loosen bolts on South Door of 55 Mill. Bolt became stuck and resulting repeated jerking motion of impact wrench, body posturing being bent low, and fatigue from removing 4 other bolts, caused a strain to the lumbar region of his back.

Injury Date of 5/24/2006, not reported until 6/6/2006. Incident occurred at #3 Bagging Station, employee had to manually remove sticking product bags weighing 50lbs from tube fill nozzle. Material was hot and would stick to metal nozzle, automatic equipment was not effective in removing bags, operator would reach down and out and pull bag off nozzle.

During a rainstorm, the area around the Bathroom was flooded. Employee had to jump to get up on the metal walkway going to the bathroom stairs. When he jumped he reported feeling a sharp pain in his Right hip. This injury was originally reported on 4/21/06 as a near miss, employee reported feeling pain on and off and requested medical attention 7/17/2006.

Employee was opening and closing an air valve in order to operate a slide gate on a tank. He pinched his finger between the handle and valve.

Employee was walking to the oil storage cabinet and slipped and fell. The R Tank had overflowed and flooded the area with slick material.

Operator was swapping tanks at the Bldg. 45 Tank Farm. He had just closed #67 and had opened #71 and had stepped down from platform. He took a step on the ground and felt a sharp pain in his right lower lumbar area. Operators use 8 lb. sledge hammer to strike locken bar to open and close valves.

Employee initially reported pain and numbness in right hand, wrist and forearm. An ergonomic computer trackball was purchased. When complaints persisted, he was sent to doctor on 3/31/06 who requested an NCV test. Work station has been ergonomically evaluated.

Employee was attempting to remove a drinking water container weighing approximately 41.5lbs from the storage bin, at approximately waist to chest high. He pulled the container from bin, and in a fluid motion moved container up to and rested it on his shoulder for transporting. After he did this motion he felt a sharp pain in his lower abdomen.

Employee was descending stairs from #14 Product Bin to #17 Product Bin when he slipped and fell on Alumina and Ice accumulation. He injured his right leg (muscle strain) and left arm (scrape on skin) and wrist (soreness).

General Mechanic welded cracks around the mounting bracket of the No. 3 bin vibrator in Bldg 400. To access the vibrator he pulled himself up between 2 pipes. As he did he felt a pull in his left shoulder. He was seen by the company doctor 12/7/2005 & given a restriction from overhead work, climbing and will be seen by a physical therapist. Not reported until 12/06/2005.

Lab technician was mixing a flask when it broke and cut his right hand between his thumb and index finger.

EE turned his ankle while lifting the feed chute on the ceramic mill at building 425.

Mechanic was changing a tension roller on #1 pannevis filter. The roller slipped out of the cradle after the bolts were removed, striking him on the left hand.

Operator was tossing small pices of scale into a wheelbarrow where an air lance was lying, energized with compressed air. One of the pieces of scale hit the valve causing it to open and the air lance jumped up toward the operator. The operator lifted his arm to protect his face. The lance struck his left pinky finger and helmet causing a fracture to his pinky.

While lifting a expansion coupling into place, the coupling came apart striking employee on the right instep from around 18" above the ground.

The employee used a pry bar, placing one end under the shaft for lifting and positioned the other end on his shoulder, allowing him to lift the bar with his hands and arms while simultaneously pushing it upward with his knees and legs. When he finished the job he realized that his back was sore

THE EE REPORTED WAS REPAIRING A BROKEN ALUMINUM CONDUIT LOCATED ON THE 1.5 FLOOR IN B.426. AS HE ATTEMPTED TO PULL BOTH RUBBER-TYPE INSULATED CABLES (APPROXIMATELY .25 INCH DIAMETER EACH), BOT H CABLES HUNG IN THE .75 INCH DIAMETER ALUMINUM CONDUIT AT AN LB CONDULET. AT THIS MOMENT THE EE EXPERIENCED PAIN IN HIS LOWER BACK. THE EE WAS TREATED AT THE LOCAL MEDICAL CLINIC & RETURNED

EE WAS RETURNING A ROURA HOPPER TO ITS UPRIGHT POSITION AFTER DUMPING ITS CONTENTS WHEN HIS RIGHT RING FINGER WAS CAUGHT BETWEEN THE ROURA HOPPER TOP REAR EDGE AND THE FORKLIFT TRUCK'S MAST.

EE WAS ATTEMPTING TO SPREAD A PIPE FLANGE TO INSTALL A BLIND WHEN HE STRUCK HIS FINGER WITH A HAMMER. THE EE WAS TREATED AT THE LOCAL HOSPITALAND RECEIVED SUTURES FOR THE LACERATION. THE EE RETURNED TO WORK.

EE WAS RETURNING FROM HER MAINTENANCE CART WHEN SHE SLIPPED ON A WET SPOT. THE EE WAS TREATED AT LOCAL MEDICAL CLINIC FOR A PULLED GROIN & RETURNED TO WORK WITH WORK RESTRICTIONS.

EE WAS USING A 16LB HAMMER TO DRIVE A DRIFT PIN FOR ALIGNMENT ON A GEAR WHEN HE FELT A SHARP PAIN IN HIS BACK. THE EE WAS PLACED ON RESTRICTIONSBY THE LOCAL MEDICAL CLINIC AND RETURNED TO WORK

EE WAS TIGHTENING A BOLT WITH HIS WRENCH WHEN THE WRENCH FELL. THE EE BENT OVER TO PICK UP THE WRENCH AND EXPERIENCED BACK PAINS AS HE WAS BENDING OVER. THE EE WAS TREATED AT THE LOCAL MEDICAL CLINIC AND RETURNED TO WORK WITH WORK RESTRICTIONS.

EMPLOYEE WAS TROUBLESHOOTING A MALFUNCTIONING GATE WHEN THE PNEUMATIC CYLINDER ACTIVATED, TRAPPING HIS RIGHT RING FINGER. EE WAS TREATED AT THE LOCAL HOSPITAL AND RETURNED TO WORK. THE EE RECE IVED OUT PATIENT SURGERY ON HIS FINGER ON MAY 16, 2003.

EE WAS ATTEMPTING TO CLOSE THE BOTTOM CAR GATE ON A BULK RAIL CAR WHEN HE LOST HIS FOOTING AND FELL AGAINST A CONCRETE SUPPORT. THE EE WAS TREATED AT THE LOCAL HOSPITAL'S E.R. AND RETURNED TO WORK AT HIS NEXT SCHEDULED WORKDAY.

THE EE WAS SITTING IN THE TABULAR CONTROL ROOM WHEN HE BECAME ILL AND LOST CONSCIOUSNESS. THE EE HAD BEEN DOING CLEAN-UP WORK IN 2 AREAS AT 3 DIFFERENT TIMES WITH BREAKS BETWEEN EACH CLEAN- UP. EE WAS TRANSPORTED BY AMBULANCE TO A LOCAL HOSPITAL FOR MEDICAL TREATMENT FOR HEAT EXHAUSTION AND RELEASED THAT SAME DAY. HE RETURNED TO WORK ON HIS NEXT SCHEDULED WORKDAY.

EMPLOYEE WAS WORKING TO CONNECT TWO HOSE ENDS WHEN FELT PAIN IN HIS LEFT WRIST. THE EE WAS TREATED AT THE LOCAL DOCTOR'S CLINIC AND RETURNED TO WORK WITH RESTRICTIONS.

EMPLOYEE WAS ADJUSTING A GATE PLATE TO POSITION FOR PROPER WELDING WHEN THE GATE PLATE CLOSED, CATCHING HIS GLOVE & RIGHT MIDDLE FINGER. HE WAS TREATED AT THE LOCAL HOSPITAL, RECEIVED SUTURES & RETURNED TO WORK.

EE WALKED THROUGH A FLOODED AREA AND REALIZED LATER HE HAD LIQUID ON HIS FOOT. HE RECEIVE TREATMENT AND RETURNED TO WORK. WHEN THE BURN DID NOT HEAL, HE WAS PLACED ON WHIRLPOOL TREATMENT.

EMPLOYEE WAS ATTEMPTING TO BREAK LOOSE A FLANGE BOLT ON A PUMP AT BUILDING 45 WHEN HIS LOWER BACK BEGAN TO HURT. THE EMPLOYEE WAS TREATED AT THE LOCAL MEDICAL CLINIC, PLACED ON WORK RESTRICTIO NS AND RETURNED TO WORK.

EMPLOYEE KNELT DOWN AND CRAWLED UNDER A BRACE WHEN HE FELT PAIN IN HIS LEFT HIP JOINT. THE EMPLOYEE WAS TREATED AT THE LOCAL MEDICAL CLINICAND PLACED ON RESTRICTIONS TO RETURN TO WORK.

EE WAS MOVING VIBRATORS AT THE BALL FORMERS IN BUILDING 426 AND USING A HAND GRINDER TO REMOVE OLD WELDS. THE EES LEFT EYE CAME IN CONTACT WITH A METALLIC OBJECT. THE EMBEDDED OBJECT WAS REMOV ED AND THE EE RETURNED TO WORK.

THE EE PLACED A 4" X 4" PIECE OF WOOD CRIBBING UNDE THE SHEET OF METAL SO SHEET METAL WOULD REST ON THE 4"X4" AND ALLOW THE FORK TRUCK TO BACK AWAY FROM THE LOAD. AS THE EE WITHDREW HIS LEFT H AND FROM THE WOOD CRIBBINGK IT CAME IN CONTACT WITH THE EDGE OF A SHEET OF EXPANDED METAL, CAUSING THE LACERATION.

EMPLOYEE WORKING WITH A SECOND EMPLOYEE ON A KILN TRUNNION WHEN USED A METAL BAR TO ROLL THE TRUNNION. WHEN THE TRUNNION MOVED OR BAR SLIPPED CAUSING THE EMPLOYEE TO LOSE BALANCE. EE WAS TIGHTEN JAWS OF PIPE THREADING MACHINE WITH A CHUCK WRENCH WHEN THE WRENCH HANDLE CAME IN CONTACT WITH THE ON/OFF SWITCH & STARTED THE MACHINE. THE EE'S RIGHT MIDDLE WAS HIT BY A WRENCH , CUTTING THE FINGER. THE EE RECEIVED STITCHES FROM THE LOCAL CLINIC AND RETURNED TO WORK.

EMPLOYEE STEPPED UP ON TO A FOUR INCH HIGH RAMP AND EXPERIENCED SORENESS IN HIS LEFT KNEE CAP. EMPLOYEE WAS PLACED ON RESTRICTIONS BY THE LOCAL MEDICAL CLINIC AND RETURNED TO WORK.

EE WAS PART OF AN 8-PERSON CREW MOVING A STACK OF SHELVES. WHEN THE STACK WAS PULLED, IT SHIFTEDAND ONE SECTION OF SHELVES SLID INTO AN EMPLOYEE'S ANKLE. HE WAS TREATED AT THE LOCAL CLINIC AND RETURNED TO WORK WITH RESTRICTIONS.

EMPLOYEE REPORTED ON FEB 2ND HE WAS CLOSING A VALVE WHEN THE VALVE UNEXPECTEDLY SEATED OR STOPPED CAUSING PAIN TO HIS LEFT SHOULDER. HE RECEIVED PHYSICAL TREATMENT AND INJECTIONS ON SEPTEMBER 24TH AND CONTINUES TO WORK.

GRINDING A BURR FROM A METAL FILER BAR WHEN HAND GRINDER KICKED BACK AND CUT OPERATOR'S KNEE, RESULTING IN 20 STITCHES.

EE WAS LOWERING A SUBMERSIBLE PUMP INTO A PREXIPATOR TANK WHEN HE STOPPED AND ALLOWED IT TO FREE FALL. THE PUMP STARTED TO FREE FALL TOO FAST WHEN HE ATTEMPTED TO FLIP THE STOP BACK. UNSUCCESS FUL, HE THEN TRIED TO GRAB THE CRANK HANDLE WHENTHE JAMMED HIS THUMB. ON 1-5-01, EE HAD SURGERY ON HIS RIGHT THUMB.

EE WAS WORKING WITH A SECOND EE INSTALLING A 4" O.D. PIPE (17" LONG WITH AN ELBOW). THE PIPE SLIPPED AND CUT THE EE'S RIGHT INDEX FINGER. THE EE WAS TREATED AT THE LOCAL CLINIC AND RETURNED TO WORK.
