Employee stepped off onto the platform on the 826 miner to descend using the access steps & stairway. The bottom steps were in the raised position and moved downward when weight was applied, stopping abruptly. The sudden movement caused the employee to experience pain in the lower back and umbilical area.

While conducting routine inspections, EO detected a smoldering inside of the coal building as they proceeded to investigate. EO made their way from the front of the coal building to the back, during this process EO sank into a hot ash pile up to their waist area, resulting in burns to their lower extremities & lower arm area.

employee was removing a skid steer bucket in order to switch to a different attachment. while bucket was propped up to release tension of the locking implement the bucket skipped off of the machine and bucket contacted employees' foot with the cutting edge, causing 4 broken bones in left foot.

Two employees were going to top of tower while elevator tech was performing work and got stuck in between floor 7-8. Employees failed to notify supervision, once made aware of the situation over an hour had passed and employees were removed within 15 minutes. Employees were offered a way out from the electrician but said no that they would wait.

While wearing gloves, a laborer was moving an empty barrel. EE's finger was punctured by an unseen jagged edge on the lip of the barrel, requiring 4 stitches. No lost time, no restrictions issued, back to work full duty for EE's next shift.

Electrician dropped a GFCI receptacle and as they bent over to pick it up reached out to some nearby expanded metal for stability, cutting a finger in the process. EE received 7 stitches and anti-biotics. Released full duty no restrictions.

The incident occurred in the unloading tank area of the waste fuels building. A portion of the area was under maintenance activities at the time of the event. No injuries or entrapments.

Strained or sprained shoulder. On 22 Dec 22 the miner reported miner injured self on 16 Dec 22. Due to late reporting, an investigation was conducted to determine if the injury happened at the mine site. It was determined on 6 Jan 23 that it did.

EE reports trying to loosen a bolt on the chem fuel line up at the Down Draft Calciner (DDC) on the tower. EE states they reached up over head and pull hard and jerk on the bolt to get it loose. EE said they didn't have pain that day but it was hurting the following morning. EE did not report the injury until 1/29/20 because they thought it would get better.

The injured worker was opening the door to the Wirtgen Continuous Miner with left hand. The miner was parked on a slope and the heavy door opened quicker than expected. Employee reached for and grabbed the door with the right hand and it twisted their right thumb and wrist.

Lying on back holding wrench as a backer. Stood up and got a tiny speck of what we think was metal in left eye. Doctor removed it Monday (3/11/19) afternoon, no restrictions, no medications, no time off. Eye still bothered EE on Wednesday (3/13/19) morning. Went to hospital, doctor did not see anything but prescribed antibiotic. No restrictions, no time off, no follow up.

He twisted his knee stepping out of a connex box on 17 Sep. Re-aggravated the knee on 21 Oct and on 23 Oct he requested treatment. We set up an appointment for 26 Oct. He went to the doctor on the 26th and they gave him a knee brace and Ibuprofen 800 and returned him to work at full capacity.

Employee was using a pry bar in an attempt to prod limestone down to the crusher rollers. The pry bar hit a cavity in the material which allowed the pry bar to travel down and make contact with the rotating crusher roller. This caused the pry bar to kick back at the employee. The end of the pry bar hit the employee on his right cheek below his right eye.

Employee was coupling railcars. Concrete slab near where employee was working had become wet due to rain. The employee stepped onto the concrete slab with his right foot. Employee's right foot slipped forward causing the employee's left knee to drive into the ground and contact the ballast rock around the railroad which caused a laceration to the left knee.

Medical evaluation of the employee indicates that the employee suffers from Degenerative Joint Disease in the left knee.

The employee then inhaled the dry chemical extinguishing agent that was released. Employee complained of shortness of breath and was taken for medical attention. Employee was released to return to work on 10/29/10.

While walking to his personal vehicle to go home the injured tripped as he stepped over safety fencing caught his foot and fell towards the ground hitting his shoulder on a large rock. The injured dislocated his shoulder. The hospital set his shoulder, gave him a prescription for pain medication and released him to return to work the next scheduled work day.

The employee's left thumb was caught between the two plates.

A quarry operator had a confirmed STS shift as part of our Hearing Conservation plan. Quarry Operator operates an enclosed haul truck 8 hours per day. The employee indicated hunting as one of his off work activities but the hearing test did list a recordable shift, that was confirmed. *Initial test proven faulty due to EE's sinus infection. Re-test showed no STS shift.*

As the employee was moving the roller the belt came on and caught his hand between the head pulley and the roller.
