Foreman Quarry & Plant Metal/Non-Metal

While conducting routine inspections, EO detected a smoldering inside of the coal building as they proceeded to investigate. EO made their way from the front of the coal building to the back, during this process EO sank into a hot ash pile up to their waist area, resulting in burns to their lower extremities & lower arm area.

employee was removing a skid steer bucket in order to switch to a different attachment. while bucket was propped up to release tension of the locking implement the bucket skipped off of the machine and bucket contacted employees' foot with the cutting edge, causing 4 broken bones in left foot.

Two employees were going to top of tower while elevator tech was performing work and got stuck in between floor 7-8. Employees failed to notify supervision, once made aware of the situation over an hour had passed and employees were removed within 15 minutes. Employees were offered a way out from the electrician but said no that they would wait.

EE utilizing a folding pocketknife in the upside-down position to cut a 6" hose. Knife folded back on itself, pinning the EE's finger between the handle & blade, resulting in a laceration to right index finger.

Worker was installing a liner. Worker was supporting liner with left hand. Liner was being tightened by another worker. Worker with injury had middle finger on left hand pinched during the tightening process. Injured finger suffered open laceration, and broken fingertip bone.

While wearing gloves, a laborer was moving an empty barrel. EE's finger was punctured by an unseen jagged edge on the lip of the barrel, requiring 4 stitches. No lost time, no restrictions issued, back to work full duty for EE's next shift.

Electrician dropped a GFCI receptacle and as they bent over to pick it up reached out to some nearby expanded metal for stability, cutting a finger in the process. EE received 7 stitches and anti-biotics. Released full duty no restrictions.

On 1/4 EE was lifting buckets of water and felt discomfort in EE's back. On 1/10 EE reported discomfort to EE's supervisor. EE went to Dr. on 1/12 and was diagnosed with a lumbar strain. As of 1/12 on restrictions, rx meds.

Employee walking down stairs took a step and felt their knee pop, aggravating a previous injury. On restrictions pending MRI.

Employee wearing gloves smashed fingertip with a bar before attempting to close a railcar door. Minor fracturing of fingertip.

Process engineer taking a water sample was stung in the right tricep by a wasp. Ice was used to soothe the sting, no further first aid administered.

Summer helper was taking a bag of trash to the dumpster outside of the office building and was stung by a wasp on the hand while opening the lid. First aid administered, no further injury.

Approximately 06:00am a smolder in the coal storage area was discovered. The smolder was extinguished at 10:53am. No injuries or entrapments.

There was a significant release of energy at the waste fuels facility resulting in substantial property and equipment damage. The incident occurred in the unloading tank area of the waste fuels building. A portion of the area was under maintenance activities at the time of the event. No injuries or entrapments.

While cleaning the tower employee wearing safety glasses sustained foreign material in eye. Off work.

A cable on an air tugger was being used to turn the mill, the eye bolt broke that was connected to the mill and the cable struck EE in the hard hat.

Strained or sprained shoulder. On 22 Dec 22 the miner reported miner injured self on 16 Dec 22. Due to late reporting, an investigation was conducted to determine if the injury happened at the mine site. It was determined on 6 Jan 23 that it did.

Miner rolled ankle dismounting haul truck.

Repeated contact with CKD over several days resulted in an allergic-like skin reaction with localized skin reddening with a rash and burn-like appearance with some swelling.

DOI the 4/5/22, not reported to company until the 8th. Went to clinic and seen by doctor on 4/12, x-rays showed arthritis, placed on restricted duty, injury was under investigation for being work related. MRI conducted on 5/2/22 and showed strain and meniscus tear making it MSHA Reportable.

Miner twisted R knee when stepped back and tripped over pipe. Became MSHA reportable on 2/17/22 when miner was placed on restricted duty.

Injured mechanic hit thumb with deadblow hammer while seating a piston in quarry haul truck engine.

Miner was getting a waste fuels sample and was bit by a spider on the back of head/neck.

Miner was closing pre heater tower elevator door and injured right hand when it was caught between upper and lower part of the door.

Injurd miner smashed left ring finger between ball sorter chute and barrel of mill balls when fork lift moved barrel.

Operating 40 ton haul truck and seat bottomed out on bump and hurt back.

Miner was working on conveyor belt in a tight area. When standing up, hit hard hat covered head two or three times on a beam they were working under. Was not reported to company until 28 Jan 2021.

Diagnosed with Carpal Tunnel Syndrome. Individual was placed on restricted duty on 12/16/20. There was no specific incident or accident to record.

Approaching an air cannon for repair assessment when the cannon went off, blowing material into face

Fell exiting Finish Mill Separator after internal inspection and lacerated left arm.

Miner slipped descending stairs on preheater tower and strained/sprained big toe on left foot. Became MSHA reportable on 8/10/20 when placed on restrictions.

Slipped on clinker ball and fell, caught self with left hand on 7/28/20. Was not MSHA reportable until 9/11/20 when miner placed on restrictions.

Waste Fuel in eyes.

EE reports trying to loosen a bolt on the chem fuel line up at the Down Draft Calciner (DDC) on the tower. EE states they reached up over head and pull hard and jerk on the bolt to get it loose. EE said they didn't have pain that day but it was hurting the following morning. EE did not report the injury until 1/29/20 because they thought it would get better.

Injured miner reached to remove a metal shaving from the Maintenance Shop lathe and it cut through EE's leather gloved right hand lacerating EE's thumb.

Injured miner was cleaning on Kiln Feed Shelf. EE was using a T-handle bar to clean built up material. EE broke through built material easier than expected and smashed EE's right pinky finger between handle of metal bar and metal wall of feed shelf.

The injured worker was opening the door to the Wirtgen Continuous Miner with left hand. The miner was parked on a slope and the heavy door opened quicker than expected. Employee reached for and grabbed the door with the right hand and it twisted their right thumb and wrist.

Lying on back holding wrench as a backer. Stood up and got a tiny speck of what we think was metal in left eye. Doctor removed it Monday (3/11/19) afternoon, no restrictions, no medications, no time off. Eye still bothered EE on Wednesday (3/13/19) morning. Went to hospital, doctor did not see anything but prescribed antibiotic. No restrictions, no time off, no follow up.

Injured employee was mounting Wirtgen Continuous Miner and struck head on air conditioner. Employee jammed their neck.

Pulling on drag chain and strained lower back

EE was passing brick and stacking them on the board for installation. A brick fell from above and hit EE on EE's right hand smashing EE's fingers and making a cut on right ring finger.

One of our dump truck drivers injured leg as EE was getting out of truck. EE caught foot on part of the truck causing EE to lose balance. EE landed awkwardly on other leg resulting in a fracture.

Employee was holding pin with chain pliers when another person swung a sledge hammer and it glanced off the pin and hit hand.

Wiping grease off golf cart seat and jammed/rolled left middle finger on 13 Nov 15. Did not become MSHA reportable until 10 Dec 15.

Got dust in his eyes

He twisted his knee stepping out of a connex box on 17 Sep. Re-aggravated the knee on 21 Oct and on 23 Oct he requested treatment. We set up an appointment for 26 Oct. He went to the doctor on the 26th and they gave him a knee brace and Ibuprofen 800 and returned him to work at full capacity.

Faulty equipment design w/ no identified hand placement area & a floating cap on mixer is guided by 3 flat pieces of metal. Cap side loaded & entered into the tanker it jarred falling on employee's rt hand.

Miner slipped in mud and "tweaked left knee". Injury happened on 4 May 2014, but did not become MSHA reportable until 15 May 2014.

Ladder slipped and fell on buttocks.

Employee aggravated a previous injury to ankle through general day to day work activities such as walking, stair climbing etc.. Employee was placed on light duty as a result of the aggravated injury.

Miner was leaning over a piece of angle iron and injured ribs.

Pain in neck.

Miner was carrying CKD valve motor and slipped on wet concrete. Same level fall and broke rib.

Did not become MSHA reportable claim until 2/6/2014. This was originally just a first aid case. Employee strained shoulder while performing general duties as a maintenance employee.

Was using vacuum hose to remove debris from dome tunnel. Strained upper back pulling vacuum hose.

Miner was removing bags in dust collector. Strained upper back and left shoulder. Did not become MSHA reportable until 10/7/13.

Employee was lifting the cover off of a measuring port on top of a material silo in order to measure the level of the silo. The employee bent over to lift the cover and strained their lower left back.

Employee was using a pry bar in an attempt to prod limestone down to the crusher rollers. The pry bar hit a cavity in the material which allowed the pry bar to travel down and make contact with the rotating crusher roller. This caused the pry bar to kick back at the employee. The end of the pry bar hit the employee on his right cheek below his right eye.

Employee was coupling railcars. Concrete slab near where employee was working had become wet due to rain. The employee stepped onto the concrete slab with his right foot. Employee's right foot slipped forward causing the employee's left knee to drive into the ground and contact the ballast rock around the railroad which caused a laceration to the left knee.

Employee slipped while walking down an incline on 1/22/12 and strained his left knee. Employee logged this as a first aid at that time and continued work. Within the past couple of weeks the employee's left knee began causing discomfort.

Employee was carrying a 2'x 6' panform for outside silo #2 to the inside area to hoist up to the ring beam inside hopper. Employee stepped down on a rock and left foot twisted off the rock.

Employee was rigging a bundle of rebar and he was guiding the cable with (R) hand to keep the load getting caught with hopper wall, Employee was grabbing the radio to signal employee on top to slow down, then he dropped the radio mic and left his hand on cable, hand was smashed between cable and sheave block.

Employee was transferring cement from one silo to another. During the process the cone of the silo became clogged. The employee removed an access port on the cone in order to free material. When the access port was opened pressure in the silo caused cement dust to be blown into employees eyes and face.

EE shoveling clinker and transporting the clinker up an incline using a wheelbarrow. While working the EE strained his lower back. This incident was initially a first aid case. After a number of days the EE's back continued to give him problems. The EE saw a dr. on 1/4/12 and was placed on restrictions at that time. There were no restrictions prior to 1/4/12.

Employee was walking across maint. shop and slipped/tripped on cables strung across walkway. Employee's overextended his left ankle. This incident was initially a first aid. After a number of weeks the employee was still having issues with ankle and was seen by a dr. on 1/10/12 and was given work restrictions at that time. No restrictions were given prior to 1/10/12.

Lab employee with a history of hip problems including hip displacements was retrieving cement samples at the beginning of his shift. Employee stepped out of his vehicle, took a few steps and dropped his empty sample can. When the employee bent over to pick up the empty sample can his right hip displaced.

Dust accumulated between the bill of employee's hardhat and the lip of employee's faceshield. Employee looked up slightly and dust trickled down into employee's left eye.

Employee was operating long reach fork-lift. When exiting the fork-lift the employee stepped down to the ground with his left foot. When the employee stepped down he indicated that he stepped on a piece of limestone and experienced pain in his left knee. Medical evaluation of the employee indicates that the employee suffers from Degenerative Joint Disease in the left knee.

The handle of an air lance pinched the webbing in between the employees middle and index finger on his right hand. The employee received 4 stitches in this area and was returned to work at full duty.

The employee was outside of a Terex Haul Truck when he reached back in the cab and activated the Engine Compartment Suppresion system for the truck. The employee then inhaled the dry chemical extinguishing agent that was released. Employee complained of shortness of breath and was taken for medical attention. Employee was released to return to work on 10/29/10.

The employee was walking, when he looked up and had small amount (4 ounces) of kiln dust drop from the ceiling inside the kiln and go under his safety glasses, contacting his eyes.

While walking to a work area of the new construction, the injured stepped into a washed out hole and he heard a pop and pain in his foot. He fractured the 4th metatarsal in his foot. Layed off 7/12/2010 with several other men hired for a specific job.

The employee noticed a rash on his neck a week ago he contributed to cement dust. The employee asked for medical attention on 6-15-10 and was taken to seek medical attention where he was diagnosed with an alergic reaction to dust. Employee was given a scrip for allergies.

While walking to his personal vehicle to go home the injured tripped as he stepped over safety fencing caught his foot and fell towards the ground hitting his shoulder on a large rock. The injured dislocated his shoulder. The hospital set his shoulder, gave him a prescription for pain medication and released him to return to work the next scheduled work day.

The employee was walking between #2 and #3 Bucket Conveyors on a catwalk, when his right foot slipped on the wet catwalk, causing him to strike his right knee on the catwalk. Employee was taken for medical treatment and given a prescription.

EE was taking measurements so a roof piece could be installed. The ee was tied off and as he maneuvered his cheater/lanyard became hung a few feet behind him. The ee pulled on his lanyard to dislodge it. The lanyard dislodged and swung towards the ee. The lanyard hit the ee in the mouth chipping two of his top front teeth.

The employee was exiting a dozer in rainy conditions when his foot slipped causing the employee to strike his left knee on a dozer track. The employee did not request to seek medical attention until 11-2-2009. The employee was returned to regular duty but was given a prescription.

Employee was laying brick and was driving brick in place with the rawhide missed brick and hit his thumb on left hand.

The employee was attempting to empty out a dust bin by having an electrician reverse the screw direction. The employee was standing 10' from the screw when the screw became compacted with dust causing some to spray in the direction of the employee striking him in his eyes. The dust spray knocked the employee's safety glasses off. Employee was given a prescription.

While standing on a frame inside a kiln, the employee ground off welds holding the frame to the side, causing the frame to slide. The employee fell with the frame and was struck by the frame.

Employee was welding a piece of handrail. The employee finishd a weld and raised his welding hood to inspect the weld. The employee then felt a foriegn body in right eye.

Employee was offloading steel plates from a forklift onto a float. The employee grabbed the first of the 2 plates and slid the plate off of the pallet and onto the float. This caused the 2nd plate to shift and slide off of the pallet as well. The 2nd plate landed on top of the plate the employee was holding. The employee's left thumb was caught between the two plates.

Employee was inside of a connex box being used as a temporary office. As the employee was exiting the connex the employee stepped on the side of a piece of grating in the doorway being used to remove mud from boots. The grate flipped up causing the employee's left ankle to roll.

Employee was tapping holes in order to finish a splice on an apron feeder belt. The employee was using a wrench to tap the holes and the wrench slipped and struck the employee in the mouth.

The injured employee had climbed 3.5' up a portable ladder, when the ladder slide over 1', causing him to fall the 3.5' to the ground below. The employee caught himself with his hand but his mouth struck his tool bucket on the ground, causing a laceration.

The employee stepped backwards on to his right leg and felt a pop in his right knee on 4-13-09. Employee did not seek medical treatment until 6-30-09. Employee ended up having ortoscopic surgery on 8-18-09 and was returned to work and placed on restrictive duty.

Employee was attempting to loosen a four inch drain line when the coupling he was unlatching fell and struck him in the right forearm. Employee was taken to the Hospital were he received stitches and a prescription.

EE was walking to his work area on the ground level. An ironworker overhead dropped a bolt that hit the injured EE on the left shoulder blade. EE was taken to the Dr & released to full duty. In March, the EE's shoulder was still giving him problems. The EE was reevaluated by the Dr & was again released to work with no restrictions, but was placed on physical therapy.

Employee tying rebar on vertical wall which was 9 ft tall. Tied to top of wall with fall lanyard and below the top with positioning lanyard. Failed to reconnect positioning lanyard after repositioning, leaned back and started falling. Lanyard caught twisting him to where left side of body was parallel to ground. The employee bumped the left side of his head on the concrete footing.

Employee was attempting to tighten a bolt when the wrench he was using slipped off of the bolt. Employees fingers were slammed between wrench and floor.

Employee involved in incident was walking under a 4' high catwalk that had a conduit running 6" underneath it. As he was walking under catwalk he bumped the conduit with his head causing a neck strain. He did not seek medical treatment until 2/5/09 and was diagnosed with a neck starin and placed on a scrip. Employee has had three shoulder/neck injuries previously.

A quarry operator had a confirmed STS shift as part of our Hearing Conservation plan. Quarry Operator operates an enclosed haul truck 8 hours per day. The employee indicated hunting as one of his off work activities but the hearing test did list a recordable shift, that was confirmed. *Initial test proven faulty due to EE's sinus infection. Re-test showed no STS shift.*

Installing floor plates and one fell on his foot. Lowering floor plates into place.

Two employees were carrying crosstie used as cribbing. One employee dropped his end which caused the injured employee to lose control of his end and drop it on his foot. The crosstie hit the injured worker's foot just behind where his steel toe ended.

Employee was cutting a padeye off of ductwork using a metabo. The blade bound up in the material that was being cut and kicked back and went over the back of the employee's left hand causing a laceration to the back of the hand.

Employee was connecting a brace to the bottom of a column. As the employee was connecting, the brace slipped up on the column and pinched the employees left index finger.

Employee was using a 12lb mallett to strike a 2' long 2" wrench. Employee swung the mallett and struck his left index finger with the mallett causing the employee to smash his finger. Employee was taken to Little River Hospital were he received 6 stitches and was returned to work at full duty.

Employe swung a lanyard around a beam. The lanyard swung around the beam and hit the employee in the mouth and chipped his front two teeth.

Employee observing a concrete pour. Pump truck belonging to company handling sanitation of work sites portable toilets began backing up. The employee was behind the truck. Employee was bumped in lower back by equipment on back of pump truck and knocked forward; the employee caught himself with his left arm. Incident occurred on 10/2, the employee did not see a dr. until 10/6.

EE was lifting one end of a 7' steel beam, putting on forklift, when he felt minor discomfort in lower back.

Employee was jackhammering buildup at the base of a bucket elevator. The buildup was 16-24" tall and was 30" wide. As he was jackhammering from the top of the buildup to the bottom the material sloughed off towards his feet causing the his hand to contact a 3" wide flange that caused him to suffer cuts on his finger that required 7 stitiches.

Employee was cleaning up/covering work materials in a laydown yard work area, when he fell backwards over a pallet of material.

The injured employee was attempting to untangle a small cable when the cable moved 2 inches due to the employees bracing moving causing the employee to suffer a cut and cracked bone on the tip of his right index finger. The system was deenergized at the time of the accident.

Employee was welding in a spider (basket) when slack in cable stopped and dropped spider 2 feet abruptly, jerking employee against the hand rail striking left lower rib.

Employee was loading brick on conveyor and brick got hung up. The controls to the conveyor were on the opposite side from where employee was standing. Employee jumped over conveyor to get controls. When he did so he got off balance & fell approximately 2' to one level & rolled 2' to ground.

Employee was placing safety lanyards around I-beams. To accomplish this, the employee was swinging the lanyards around the I-beams. As one of the lanyards was swung around the I-beam, the lanyards metal hook came around the I-beam, hit the employee in the face, and chipped his top left canine tooth and the top left incisor directly in front of the top left canine.

Employee was preparing rigging for a crane in a fabrication area. Co-workers nearby were grinding metal. The injures employee got a burr in his left eye from sparks/debris thrown by the grinder. The injured employee was wearing all proper PPE at the time including safety glasses.

Employee was handling and stacking material. Employee got a headache and began feeling ill. Employee took a break and still felt ill. Employee then informed his supervisor that he still was not feeling well.

Employee was newly hired and not conditioned to manual labor in warm weather. Employee was compacting dirt with a sand tamper and began feeling ill. Employee informed his supervisor that he wasn't feeling well.

While flying a piece of grating into place with a 2250 crane, the grating hit a piece of existing steel. This contact caused a toe board which was welded onto the grating to be knocked off. The toe board fell approximately 20' to the next level below and struck the employee above his right eye.

Employee was weedeating and pitching trash around jobsite. At some point the employee developed a rash that he claimed was caused by poison ivy. The employee's supervisor could not find any poison ivy in the areas the employee was working in.

Employee had a seizure and fell over on to his side.

Employee stepped off a crane and twisted his knee.

Employee was impacting bolts on iron structure. The bolts were above the employee's head and the employee's arms were outstretched overhead. While in the process of impacting, the employee strained his left shoulder.

Employee was walking on rebar mat with 1'x2' gapsin the rebar. Rebar is 4" above corrogate decking. He took an awkward step in a groove which caused his foot to slip on the rebar. He felt a pop in left knee.

The employee was lifting a 50lb SWDF Bucket when he felt a burning feeling in his lower back. Employee has had a prior surgery prior to begining employment here and also a medical case on his lower back.

EE was sitting at desk, she felt insect bite on her neck. She brushed it off, it fell into her right sleeve and bit her on the right bicep. Small lumps developed at the bite mark. She did not report the incident until 5/15/08, when she felt continued pain in her arm.

Employee was setting a channel iron and caught right hand pinky finger between the channel iron and an angle iron.

Based on the information that was provided the employee was either climbing onto the machine or off the machine and slipped and fell approx 6-7 feet. It is also possible he reached the work platform and then fell backward thru the entrance way.

Employee was power washing rebar and hit his foot with the spray.

Employee was doing maint. on a crane and went to exit when he closed the door on his finger.

The employee involved in the incident was attempting to make a left turn using a walk behind loader when the controls became stuck in the left turn position which caused the machine to make a full circle and strike the employee on the left knee. Employee taken to seek medical attention and was returned to work on restricted duty after being diagnosed with a left knee bruise.

The employee was driving a golf cart when he hit a bump and a spider fell from the golf cart canopy and bit the employee on his left cheek. Employee was taken to seek medical attention on 08/17/2007 and given a scrip and returned to work at full duty.

Employee was jackhammering build-up when his jackhammer became wedged into some build-up. When employee jerked jackhammer attempting to free it he fell backwards onto his right shoulder. Employee requested to see DR. on 6/27/07 and was diagnosed with a shoulder strain.

Employee was pushing on a piece of 2" angle iron when he felt a pop in his shoulder. Employee requested to see DR on July 16th and was returned to work at full duty. Employee was given a prescription for a muscle relaxer at the visit.

Employee was walking around the front of the cooling water tower and slipped on the concrete which was wet at the time injurying his left shoulder. Employee was first taken for observation and later taken back to DR on June 5th and received a scrip. Employee is at full duty.

The injured employee was attempting to free a stationary roller that a belt conveyor moved tires onto. As the employee was moving the roller the belt came on and caught his hand between the head pulley and the roller. He received bruising on his hand and was taken to Little River Memorial Hospital where he was treated and released back to restricted duty.

The employee was attempting to reposition a 2" by 6" piece of rectangualr tubing that was 20' long onto a jackstand. The tubing fell off the jackstand and pinned the employees left wrist between the stand and tubing causing a small fracture of the Ulna Styloid. Employee was returned to work on restricted duty.

Employee was pulling on a trunion jack when he felt a pain in his elbow. Elbow was returned to work at full duty but his condition regressed causing the employee to have surgery on 1-3-07. The employee had a minor procedure and did not miss any work due to this.

Employee was lifting a 70 lb SWDF Pail when he felt a pain in his lower back. Employee reported accident on 6/8/06 and asked to go to the Dr. on 6/19/06 and was taken that day. Employee has had two his vertabrates fused togther in his back 4 years ago before begining employment with us and has had problems with his back since then.

Employee was removing a vent-hose (4" VAC hose) from camlock fitting. The hose hit him in the chest causing cement dust to enter under his safety glasses and land in his right eye.

Employee was cutting a piece of insulation off of a new electrical wire when the knife he was using slipped off of the wire and cut his hand. Employee was taken to a clinic and received stitches. He was then released to return to work

Employee was walking down stairs when he felt a pop in his left knee. This resulted in a tendon tear of the left quadracep where it attaches to the knee. The Physicians said that the incident was caused by 20 years of use and the tear was going to occur at work or at home. He said it was going to happen how ever.

Employee was pulling a water pump out of a 1' deep hole when he felt a pain in his left shoulder. Employee had reconstructive surgery on this shoulder 10 years before.

Employee was using a 59lb Jackhammer when he felt a pain in his left elbow. Employee had seen his private physician two months ago concerning pain in his elbow and was diagnosed with brusitis. The Work Comp Dr. said the jackhammer vibration had aggrevated a pre-existing condition.

Employee took 20 ml from centrifuge 5 when he tried to open it the vial ruptured acid splashed on arms, neck and a small spot below the lip-went to er and dr gave him an ointmeal to put on burns.

Employee was pulling a car tire off of a roller bed when he felt a pain in his shoulder. Employee did not seek medical attention until 4-18-04.

Employee entered the engine compartment of the Wirtgen Surface Miner with the engine running checking for an airleak. Employee placed his hand into a plastic fan blade that is used to cool the Miner Oil Pump. Employee had a cut on his right hand that required stitches.

Employee had removed a 12" by 14" cooler grate when he stood up to retrieve a tool. He then stepped back into opening, bruising his shin. Employee was returned to work but went back to the doctor on 2/28/05, complaining of knee pain. Employee was placed on restricted duty that day.

Employee was walking and slipped on ice which had accumulated on the ground from a ice storm we had in the area. Employee was taken to the Dr. on 12-22-04 and returned to work. Employee was sent for Professional treatment later in the month of January for continued soreness in his left shoulder.

Employee was pulling a shaft through a bearing housing when the shaft end fell on the opposite end and pinched the employee's middle and left index fingers causing a small break on the end of the middle finger.

Employee was twisting with a 65lb bucket lifted when he felt a pain in his back

Employee was lifting a 70lb troughing roller over his head to place on a 4' high belt when he suffered a pain in his stomach

Employee was walking down a flight of stairs when he steped on a 1" piece of Gypsium and twisted his knee. Employee was taken to Dr. on 10/04/04 and returned to Full Duty. Employee was placed on restricted duty on 12/03/04

Employees thumb was pinched between two weights when the employee attempted to move one of the weights

Employee was lifting a 70 lb grate when he felt a pain in his back.

Employee was swinging a 12lb sledgehammer when he felt a burning sensation in his arm. Diagnosed initially with muscle strain

Employee was pulling on a grinder pump when he felt a sharp pain in his neck.

A guide rail on #2 tire line broke when a tire moved up against it. The guide rail then swung into the employees forhead. Employee was taken to hospital and had six stiches placed in his forhead and returned to work. Employee was looking at the tire system on the way to clock in to come to work when this happened

Employee noticed irritation on eyeball after making solution which consists of nitric acid & hydrochloric acid.

EE WAS MOVING A FLOP GATE HANDLE WHEN HE PINCHED HIS LEFT MIDDLE FINGER BETWEEN THE HANDLE AND A CHUTE.

Employee was pulling up on a pry bar when he felt discommfort in his back. Employee had been to the Dr. 3 other times with his back before this incident. Employee received medical treatment off and on in 2004 & 2005. On May 22nd 2006 the employee said his back was starting to bother him again and was taken to the Dr. and placed on restrictive duty.

EE WAS LIFTING A GOULD PUMP CASING WHEN HIS HANDS SLIPPED OFF THE CASING. THE EE ATTEMPTED TO HOLD ON TO THE CASING BEFORE IT FELL CAUSING HIS LEFT, 2,3,4 FINGERS TO BE CUT.

EMPLOYEE WAS JABBING AT DIRT CHUNKS WITH A POLE WHEN HE FELT A SHARP PAIN IN HIS LEFT ELBOW. HE WAS INITIALLY RETURNED TO WORK AT FULL DUTY UNTIL 2/2/04 WHEN HE WAS PLACED ON RESTRICTIONS.

EE WAS WALKING ACROSS BOTTOM OF SLURRY TANK WHEN HE STEPPED ON AN INCLINE WITH HIS LEFT LEG AND BEGAN TO SLIDE. EE CAUGHT HIMSELF WITH HIS RIGHT LEG AND STRAINED RIGHT KNEE.

EE WAS STEPPING OFF OF WALKWAY ON TO TOP OF RAILCAR WHEN HE TWISTED HIS LEFT KNEE.

EE WAS PUSHING ROLLOFF BOX DOOR WITH LEFT SHOULDER WHEN HE FELT A SHARP PAIN IN THE SHOULDER.

EMPLOYEE STEPPED OFF OF A CEMENT COOLER AND JAMMED HIS KNEE CAUSING A SMALL HAIRLINE FRACTURE. EE REPORTED INJURY ON 5-7-03 AND ASKED FOR MEDICAL TREATMENT ON 5-22-03.

EE WAS PULLING DOWN ON A WRENCH WHEN IT SLIPPED CAUSING EE TO PIN HIS LEFT MIDDLE FINGER BETWEEN THE WRENCH AND A PIECE OF ANGLE IRON.

EE SLIPPED WHILE WALKING DOWN STAIRS INJURING HIS RIGHT ANKLE.

EMPLOYEE WAS WALKING DOWN STAIRS WHEN HE TWISTED HIS ANKLE. HE HAS HAD NUMEROUS ANKLE STRAINS ON HIS RIGHT ANKLE OVER THE LAST FOUR YEARS. HE FIRST WENT TO THE DOCTOR WITH THIS ANKLE IN 2000. HE WAS RETURNED TO WORK WITH NO RESTRICTIONS UNTIL 2/13/04 WHEN HE WAS PLACED ON RESTRICTED DUTY HERE AT THE PLANT. HE HAS BEEN RELEASED TO FULL DUTY 3/1/04.

EE WAS HOLDING SEAL WHILE SETTING TRUNION ROLLER. TRUNION ROLLER SLID PINCHING EE'S LEFT MIDDLE FINGER.

EE WAS WALKING WITH ONE GRATE IN FRONT OF #2 COOLER WHEN HE STEPPED ON A LOOSE CATWALK GRATE AND HAD ONE LEG GO THROUGH THE GRATING 5 INCHES CAUSING THE EE TO HURT HIS LOWER BACK.

EE WAS BENT OVER MOVING FRAMEWORK WHEN HE TWISTED AND FELT A SHARP PAIN IN HIS BACK.

EE WAS KNEELING PUTTING CHOCKER THROUGH BOLT HOLE IN #3 MILL WHEN EE FELT A SHARP PAIN IN HIS BACK.

EE WAS PULLING ON A CHAIN HOIST WHEN HE FELT A PAIN IN HIS LOWER ABDOMEN.

EE WAS CHANGING OUT AN INSTRUMENT ON CLINKER COOLER. EE WAS STANDING ON A STEP LADDER WHEN LADDER TIPPED OVER.

EE WAS CHANGING OUT LIGHTS WHEN HE CUT HIS HAND ON A SHARP PIECE OF METAL. HAD A CUT ON HIS RT. RING FINGER.

EE WAS PLACING 35#-50# BUCKET ON SWDF LINE WHEN HE FELT A SHARP PAIN IN HIS LEFT ELBOW.

FIRE FOAM SYSTEM BEING INSTALLED. MECHANICAL ISSUES CORRECTED.

EE WAS LIFTING A SWDF BUCKET WHEN HE FELT A SHARP PAIN.

EMPLOYEE WAS IN A HURRY PUTTING 50-70 LB BUCKETS ON A CONVEYOR LINE.

EMPLOYEE WAS OPERATING AN AIR IMPACT WRENCH WHENDUST WAS BLOWN INTO HIS LEFT EYE.

EMPLOYEE HAD BEEN WORKING IN A HIGH HEAT AREA IN THE MORNING. EMPLOYEE SUCCOMBED TO HEAT STRESS AROUND 10:00 AM.

EE WAS TIGHTENING A BELT WHEN HE BEGAN TO HAVE BACK SPASMS.

EMPLOYEE WAS USING HIS HANDS TO CLEAN OUT A DROP BOX WHEN A PIECE OF METAL IN THE DROP BOX PENETRATED HIS RIGHT RING FINGER AND BECAME IMBEDED.

EMPLOYEE WORKING IN DAMP CONDITIONS WHICH LED TO EXTENSIVE FOOT IRRITATION. SUCH IRRITATION IS A SUBSTANTIAL AMOUNT SUCH AS YOU MIGHT SEE WITH SEVERE DIABETICS.

EMPLOYEE WAS CARRYING A PIECE OF PIPE UP AN INCLINE WHEN HE MADE A WRONG STEP AND FELT A PAIN IN HIS KNEE.

EE WAS WORKING UNDERNEATH A CEMENT SILO WHEN DUST BLEW INTO THE EE'S LEFT EYE.

EE WAS REPLACING A GUARD ON #3 CLINKER COOLER FAN WHEN HE DROPPED GUARD ON FINGER.

EE WAS TRYING TO REMOVE AN AIRHOSE FROM INSIDE A CLINDER COOLER. HE STEPPED ON AN UNEVEN SURFACE & FELL. X-RAYS REVEALED A FRACTURE OF THE FIFTHMETATARSAL IN HIS LEFT FOOT.

EMPLOYEE WAS KNEELING ON CATWALK TRYING TO BREAK BOLTS LOOSE ON A BELT ROLLER. HE FELT SOMETHING GIVE AND THEN LOCK UP IN RIGHT KNEE. HE WENT TO DOCTOR & SPECIALIST BUT RECEIVED NO MEDICAL TRE ATMENT UNTIL 6/6/02.

THE INITIAL VISIT WAS 3-11-02 AND DIAGNOSIS WAS A POSSIBLE PULLED MUSCLE. A SUBSEQUENT VISIT & TEST REVEALED A COMPRESSION OF THE C-SPINE.
