Granite Mountain Quarry #1 Metal/Non-Metal

Employee was trying to free up a seized release pin on the skid steer mast so the bucket could be removed. The release pin broke free resulting in the skid steer bucket falling and striking top of employee's foot directly behind safety toe of work boot.

Employee was dragging roller bearing out of travel way when inner bearing and race turned and pinched left hand middle and ring finger. Employee received two stitches.

Positioning Jaw Crusher in, it sprang back and penned arm against a beam and fractured wrist.

Employee was shoveling material off the C-4 pad and caught a snag on the surface of the pad with the shovel while scooping. Thus, causing EE to twist a little and strain lower back.

Night crew was rebuilding 5.5' Standard Crusher, which required the use of a 60- Ton crane, which was an outside contractor. After completion of the rebuild, the employee was assisting the crane operator with stowing away the outrigger foot pads. The employee placed their right hand on top of outrigger and crane operator extracted outrigger, thus crushing the employee's right hand.

Employee was trying to thaw out the tail pulley at the wash tower of C207. Employee poured brake-line antifreeze on the belt at the tail pulley of C207 and lit it on fire. Employee then poured more brake-line antifreeze on the fire which caused it to blow up catching EE on fire and burning EE's lower legs.

Prior to tool use, employee was changing defective fitting on a railcar jack stand after pre-shift inspection. Metal sliver cut employees thumb requiring stitches by medical prof. Jack stands owned by contractor. Injury did not involve mine equipment or system. Tool training & inspection covered in Part 46, why fitting was being changed prior to use. All PPE worn.

Employee was walking past parked skid steer with pole attachment. As EE went to step over pole, EE rolled their right ankle.

Employee was hammering a wedge in on a screen, with a hammer and strained left shoulder.

IE and a co-worker were carry a piece of metal. When the co-worker tripped causing the co-worker to slam the piece of metal into the wall. This caused IE's finger to get smashed and have 3 stitches.

EE was setting down a bucket with metal in and turned the wrong way and sprained back

EE was carrying a motor and when EE went to set it down it slipped. It caused the shaft to hit right behind the steel toe.

EE was pulling screens on S5 shaker. EE slipped hitting their right shin on the shaker strap. EE had to have stitches.

Was helping to pick up ne plates from the gradall straight across to the metal belt. When employee was putting them on the belt it slipped out of hand and fell on their other hand cutting the palm of their hand right under the fingers.

Was changing a return roller when EE reached for the roller EE's leg turned while EE's foot stayed put causing EE's knee to pop

EE was pulling on a rubber screen.

EE was helping the welder with a job and stepped off of C-2 conveyer frame and stepped on a rock and sprain right ankle.

Employee was straightening up a clamp bar on a conveyor belt, using a hammer to line it up and hit thumb.

EE was working on a screw that was stuck. EE was turning it backward to get it unstuck and that's when the v-belt smashed left index finger

EE was holding metal piece while tightening bolt with other hand. When the bolt was tight ee moved left out of the way and finger was cut on a burr from the metal ee was holding. Left index finger

Load of angle iron was being dropped in a new location when the load slipped and fell. One piece of angle iron bounced and hit EE in the lower leg. EE had a laceration with stitches.

EE was shoveling out a feeder when EE smashed EE's finger between the shovel handle and the frame of the conveyor.

Threw a piece of rip over car and it landed on my left index finger instead of falling off the side of rail car. Cleaning out rail cars

EE was working on a track hoe bucket and was hitting the bucket with a 4lb hammer when a piece of the hammer broke off going into left upper thigh.

EE was using a pry bar when it slipped and hit ee above right ear. EE was wearing hard hat.

Operator got dozer stuck and walked down a slope to see how to get the machine unstuck. Stepped on a rock and turned ankle. Resulted in an ankle sprain.

Employee was getting out of the service truck when he rolled his left ankle. This caused him to sprain his ankle.

Was working on the SB2 with a loader when some of the SB2 fell and landed on the loader bucket causing the loader to bounce. Employees head hit the dome light and cut the top of his head.

Trespasser on mine site. Police suspect blunt force trauma.

He is a welder helper after he had gotten off work at 2:30 a.m. he went home and then around 5:00 a.m. his eyes started hurting. He had gotten a flash burn. He received a prescription for his eyes

Was changing v-belts when pipe wrench slipped causing his hand to go around the shive cutting his right hand little finger

Climbing down ladder when his hand slid across metal bur cutting his finger.

He was on the welding truck to turn the welder off when he stepped off his knee gave away. EE didn't want any medical at that time he felt it would be ok. He continue to work but didn't feel it was getting any better when he came in on 8/13/12. So he didn't see a doctor till 8/13/12

EE was holding the pin while another ee was using the hammer to drive the pin into the chain when a ee missed hitting ee finger.

He was getting a piece of metal out of a chute and the metal flipped cutting his right ring finger.

Was using a 16lb hammer to strike the loader bucket to remove the shank, when a piece of metal came off, hitting him in the lower left leg. He was treated with just first aid, they said to just leave the metal in. Then the doctor decided to remove the metal on 4/29/11 and he was released to full duty.

When he first went to the clinic they did first aid and said to leave the metal in that it would be ok. Now he has an infection and the metal has moved into a muscle, so now they will be doing surgery on 9/22/10. What had happen was he was removing a belt clam with a hammer and the chisel made for belt clams when a piece of metal went into his leg.

EE was drilling with the beetle drill. He moved the pipe wrench from the holding position around to break the steel from the power head smashing his finger between the wrench and the tracks on the drill.

EE was exiting the locomotive when his foot got hung on the bottom step and he fell on his hands and knees.

EE was being loaded with material when the bucket of the loader came down and hit the bed of the haul truck. EE was jarred when the bucket hit.

Unbridging the feeder when he sprained the muscle in his abdomen. He didn't report until 11/09/2009.

Employee was using wrench to remove hose and it slipped off of nut and left hand hit bottom of engine block. Laceration to 2nd knuckle of hand.

Metal sheave counterwight came loose from a come-a-long and fell across his lower leg and foot.

Trying to put a rail bolt into a shaker when he slipped on some ice and his foot went between the floor and shaker.

Installing cylinder pin in 769 water truck, while pinning cylinder in truck he over-strained his muscles in confined space.

Was cutting metal when a fireball fell on his blue jeans over his left knee causing a burn on his knee. Didn't go to the doctor till 7/30/08.

Had patch holes in a rail car and was climbing out when he slipped and fell hurting his back. It was his three day.

He was replacing cutting edges on a loader and strained his groin while picking up a cutting edge.

Working on loader bent down to connect lines on the engine when he put his knee on a bolt that was sticking out from the frame.

Was inside shaker changing screens and replacing bed rubber, when he was cutting a piece to install and cut his thumb.

Employee was beating on a piece of metal to close off chute when he hit a rock that came up and glanced his left eye.

Employee was using an air operated impact wrench and the socket flew off and struck employee on a tooth and broke the crown.

Knelt down to tie wires on the blasting caps for a shot.

Shoveling spills of rock from conveyor belts and shakers, he strained his shoulder. He didn't go to the doctor until 3/20/07.

While watching hub hammer remove concaves from gyro crusher, was struck with flying debris. Had laceration to the chin.

Shoveling out the gyro when something hit him from behind and knocked him down. It was a large rock spraining his right ankle.

Turned the wrong way while moving a lifting cable out of the way. Didn't go to the dr. till 10/04/06.

was pulling on fuel hose of a 992 loader when felt pain in his arm and elbow. Thought it would get better so waited to go to the doctor. It continue to hurt so when to the doctor on 3-7-06

Welding in the gyro and the flash from another welder flash burned his eyes.

Stepping off conveyor twisted right ankle.

Finished on bridging feeder; when he was coming down the hand rail broke causing him to fall, landing on both hands.

Was unbridging the bottom of the Jaw and a rock fell and hit the bar mashing his finger against the wall.

Changing wear plates on loader. When taking out the bolts with an impact wrench the bolt spun, cutting his middle finger on right hand.

Cutting shrink wrap off motor with pocket knife and cut hand.

Changing tire on water truck using a breaker bar to tighten lug nuts was pulling up when had a pain in lower back. He didn't report to worker comp. till 8/8/05. They felt that is wasn't compesable and went to court. Just settled that it was worker comp. Missed 33 day and still on restricted.

Cutting metal on bucket metal popped off cutting left hand little finger.

While getting off plate feeder his foot got caught between plate feeder and side board.

EE WAS HELPING TO THROW SCREENS OFF THE TOWER WHEN HE PULLED HIS BACK.

EE WAS WALKING ACROSS THE FLOOR UNDER THE GYRO WHEN HE TURNED HIS ANKLE.

Slip in bucket while trying to hold a piece of metal. didn't go to the dr til 4-19-04.

HE WAS LOADING A SHOT WHEN PUTTING TUBE IN HOLE THE ROCK HOLDING THE CAP WIRES FLIP UP HITTING HIS LEFT FOOT.

EMPLOYEE FELL WHILE CHECKING SCREENS ON A VIBRATORY SHAKER. EE FELL OFF ANOTHER VIBRATORY SHAKER EARLIER INT HE EVENING, BUT IT WAS NOT REORTED UNTIL AFTER THE SECOND FALL.

HELPING TO WELD A PATCH PLATE WHEN HE RECEIVED A FLASH BURN TO EYES.

WHILE HELPING TO LEAD A SUMP PUMP IN THE BACK OF A PICKUP, EE HURT LOWER BACK.

DOING CLEANUP WITH WATERHOSE AND TWISTED.

ABOUT 3 WEEKS PRIOR TO RECEIVING MEDICAL ATTENTION EE COMPLAINED OF A WRIST PROBLEM AFTER LIFTING A SCREEN. WHILE SHOVELING ON 9/12/02 HE SAID HIS FINGERS WENT NUMB AND WAS GIVEN MEDICAL ATTEN TION.

USING A 16 POUND SLEDGE HAMMER TIGHTENING BOLTS TO SLIPPED ON GYRO CRUSHER.

CHANGING A SCREEN ON THE 6 X 16 SHAKER AND CAUGHT FINGER BETWEEN THE RAIL AND THE SCREEN.

CLOSING A DOOR TO A FEEDER, THE HANDLE SLIPPED & STRUCK HIS RIGHT UPPER ARM.

WORKING AROUND BASE PLANT WHEN ROCK BOUNCED AND HIT HIM IN THE EYE.

WALKING ALONG CONVEYOR WALKWAY AND BUMPED HEAD ON OVERHEAD BEAM.

USING A SHOVEL TO CLEAN OUT RAIL CARS.

HAUL TRK WAS BEING LOADED WHEN A LARGE BOULDER JARRED THE TRK & CAUSED BACK PAIN TO EE

EE WAS OPERATING A WATER TRUCK AND HIT POTHOLE IN ROAD AND JARRED HIS BACK.

WELDING ON SPRAY BAR PIPE WHEN A PIECE OF RUST POPPED AND FLEW INTO EYE.

CLOSING ELEVATOR DOOR ON SECONDARY TOWER AND CAUGHT FINGER BETWEEN DOOR.

USING HIGH PRESSURE WATER HOSE WHILE WASHING OUT BALLAST SCREWS WHEN POWER SURGE CAUSED HIM TO FALL OVER 8" PIPE.

CLEANING WINDSHIELD ON 970F LOADER, SLIPPED AND FELL TO THE GROUND.

LIFTING DIRTY WATER PUMP BY CABLE AND CUT HAND.

LIFTING A 2 FOOT BLOCK ON 8"X8" WOOD AND BUMPED IT AGAINST UNDERSIDE OF 1 TON TRUCK BED.

PULLING SCREEN OUT OF VIBRATOR WHEN HOOK SLIPPED AND EE FELL BACK AGAINST SPRAY NOZZLE.

INSTALLING A NEW BLADE ON HOSE CUTTER HIS HAND SLIPPED ON CUTTER BLADE.
