employee stepped up on oil containment wall. EE's boot caught the top of wall causing employee to trip and fall. Employee extended EE's right arm to catch self pulling something in EE's right bicep.

Employee was using a pry bar to spread two flanges apart. Pry bar slipped causing EE to smash the tip of EE's middle finger on right hand on motor mount. This caused a cut requiring 9 stitches.

EMPLOYEE WAS LIFTING A 55 GALLON DRUM WITH A CHAIN AND THE BARREL SHIFTED CAUSING SLACK IN THE CHAIN AND CAUGHT HIS MIDDLE FINGER ON HIS RIGHT HAND.

OPERATOR WAS IN A HURRY TO GET OFF OF THE LOADER AND LOST HIS BALANCE. HE THEN TRIPPED OVER A ROCK LANDING ON HIS HIP AND RT. LEG. HIS RT. WRIST HIT THE GROUND AND RT. ARM HIT LOADER TIRE. HE SUSTAINED A SPRAINED WRIST & SHOULDER.
