The employee began to set up a 6' ladder to begin working on the belt scales. When the employee reached the 4th rung, the employee realized the ladder will not be tall enough to perform the job tasks. While climbing down the ladder, the ladder then toppled over causing the employee to fall, injuring right ankle. Employee is unsure as to how the ladder toppled over.

As EE pulled on the wrench, it slipped and the wrench struck the tip of EE's finger smashing it between the wrench and the bumper of the truck. EE was taken to the clinic where it was determined that EE suffered from a laceration to the tip of the finger which required 3 stitches and a fracture to the bone as well

Employee was was on the cat walk of the cone crusher checking for bowl float when the cone over flowed and an 8" rock fell down cutting ee left little finger that required three stitches
