CEMEX Construction Materials South LLC · Struck against a moving object

Employee states with the repetative movement of operating a dozer, he sustained an unknown injury to the neck. Employee completed a WC case with Industrial Commission on 12/4/08, we did not receive confirmation on this injury until 1/8/09. This case is currently under investigation and an amendment may be submitted at a later date.

Cemex Construction Materials LP · Caught in, under or between a moving and a stationary object

EE's were working together to install a bolt on the dewatering screen, when injured EE put his finger in the bolt hole to check the alignment. Another EE inserted bolt from the other side and struck it twice with a hammer, pinching injured EE's finger tip, causing him to loose the finger nail and fracturing the finger tip.

EE WAS ASSISTING IN REPAIRING 12" PVC PIPE THAT WAS DISLODGED AT JOINT. THE PIPE WAS ABOVE HEAD LEVEL APPROX 7 FT ABOVE GROUND LEVEL. AS HE PLACED HAND ON PIPE SUPPORTING STRUCTURE IT GAVE WAY CAUSING THE PIPE FULL OF MUD TO ROLL ONTO HIS FINGER RESULT WAS SEVERE LACERATIONS TO BOTH HANDS
