THE RUBBER SNUBBERS ON THE CRUSHER SCREEN NEEDED TO BE REPLACED. AS HE WAS REPLACING THEM HE DISCOVERED THE RUBBER NEEDED TO BE TRIMMED A 1/4". AS HE WAS TRIMMING, THE LAST CUT DIDN'T HAVE ANY RESISTANCE & THE BLADE PROCEEDED TO CUT HIS UPPER LEFT LEG THAT REQUIRED 7 STITCHES.

OPERATOR WAS USING A TORCH AND A PIECE OF SLAG FELL DOWN-BURNED A HOLE IN PANT, LEG AND FELL INSIDE OF HIS BOOT. OPERATOR SUSTAINED 2ND DEGREE BURN ON TOP OF HIS RIGHT FOOT.
