Employee was removing a roller at the wash plant. The roller slipped causing a small cut to the employee's right ring finger. About 10-15 minutes later, after the finger was cleaned and a bandaid put on the cut, the ee passed out. By the time EMTs arrived, he had gained consciousness but was taken to the hospital to be checked. He was fine except for the small cut.

Isolated case of Employee misconduct where Employee failed to follow procedures and climbed onto the sand screw containment at the Wash Plant without the use of a ladder or a fall protection system and fell approximately 8.5 feet to the ground landing on his left shoulder.

EE WAS USING A RAZOR KNIFE TO CUT CONVEYER BELTING. THE BLADE BROKE CAUSING THE KNIFE TO SLIP AND CUT THE EE'S HAND BELOW THE THUMB. LACERATION REQUIRED 4 STITCHES TO CLOSE THE WOUND.
