THE WAS ASKED TO REMOVE SCREWS FROM A METAL ROOF DECK ON THE EAVE OF A ROOF. FOR REASONS UNKNOWN, THE MINER REMOVED HIMSELF FROM THE ELEVATED WORK PLATFORM (JLG AND GOT ON THE ROOF. HE ALSO VI OLATED COMPANYPOLICY IN PROCEEDING TO WORK IN AREAS OF THE ROOF WHILE NOT TIED-OFF. WHILE WORKING ON THE ROOF DECK, NOT IN FRONT OF THE JLG AND WHILE NOT TIED-OFF, HE FELL TO THE GROUND.

Employee fell while walking through a doorway. They reached out to catch themselves, but their hand gave way and they struck their elbow onto the concrete, resulting in a hairline fracture of the elbow. Fall was from same elevation with no obstructions or material in their path.

Employees were on the ground laying a spreader bar down to begin break and as it was laying down EE had hand on the underside as opposed to on top and EE's first instinct was to push it and it pinched EE's hand between the spreader bar and saw horse smashing three fingers.

On April 3, 2025, at 3 PM in Arizona, an employee cutting a liner plate got it stuck. After securing and cutting it, the plate twisted, pinching their hand. They iced it, continued work, and later saw a doctor who diagnosed a left hand metatarsal fracture. Supervisor was notified; swelling reduced.

While installing a door liner in a tight space, a failed hook keeper caused the plate to swing, pinning the employee against a scaffold pole. The impact injured the employee's left knee. Witnesses operated comealongs from ladders. Employee had restricted duty. Inspect all rigging equipment, chains, and hooks. Enhance safety training and protocols to ensure a safer work environment.

Unloading material from a skid. On the last pick EE found a nylon strap still attached. EE reached in with the cordless grinder to cut the strap. When the strap was cut the load shifted and EE's thumb was caught between the load and the diagonal brace on the skid.

Visiting corporate engineer was climbing down the ladder to the woodchip hopper when they slipped and fell approx. 5 feet onto the concrete. They sustained a fractured left ulna and radius as well as a contusion on their back and shoulder.

Employee was standing on part of the structure, tied off, with working safety choker around anchorage point. EE repositioned to reach the work when doing so EE misjudged the step and fell onto the handrail striking the ribs on the right side.

Employee was removing the bearing cap from a belt conveyer pulley for inspection. EE forgot to remove the tension from the counterweight and when EE removed the bearing cover bolts it shifted smashing EE's hand between the bearing cap and support structure. The result was multiple lacerations and fractures of the left ring finger.

During demolition of a project, EE was holding a piece of steel and after the tube was cut, they pushed more out on the bottom piece then the top piece which caused the top to come down and hit their hard hat and glasses, causing the glasses to cut their eyebrow.

Employee was checking the shear pin on the K-4 coal mill. They reached for a wrench to leave the area and the coal mill clean out door nuts vibrated loose and the door pinched their right index finger between the door and the rotary feeder housing. This resulted in a minor fracture and a laceration of the right index finger.

Employee was positioning a sill for the D4 roller mill, weighing approx. 200 lbs, into position. While bringing it in the employees hand became caught between the sill and the bearing housing of the D4 mill resulting in a fracture of EE's right hand pinky finger.

Employee had irritation on back when cement dust fell down the back of EE's shirt on Feb 18. EE decided to get it looked at on Feb 22 and was treated with first aid and released to full duty. On Mar 5 EE went to an Urgent Care clinic and was treated for an infection

On 11-4-15 EE was hit on right shoulder by a wire cutter dropped from above. On 12-7-15 EE stated that the shoulder was sore and was transported to the clinic. EE was diagnosed with impingement syndrome and placed on modified duty and therapy.

EE reports a spider bite, does not know when or where it happened. He says it probably happened when he opened a bottom door on a rail car. Informed his supervisor on 5-6-15, asked to see a physician on 5-7-15. The physician gave him a shot and a prescription. Returned to full duty, no restrictions

EE standing 5' away from another EE swinging a 4# hammer trying to break a bearing loose. A piece of the hammer head broke off and impaled in the right arm of the injured employee. EE removed metal shavings from steel stock being machined on the lathe. The shavings cut through his glove causing a laceration to his finger. EE transported to emergency care & received six stitches, returned to regular duty. EE will have outpatient procedure on 7-25-14 (regular scheduled time off) & will be on modified duty on his next scheduled day.

The employee who was holding the cardboard also had the razor he used to cut the cardboard in his hand causing a laceration to the injured employee on his left ring & small finger requiring stitches. Installing a burner pipe in a kiln when the cable of the manual hand winch failed causing winch to move, pinching the finger of the worker between the handle and a steel I beam. Pinched top of middle finger on right hand, hairline fracture seen in X-Ray when worker taken in for medical treatment.

Received burns from working in a bag house. He received the burns from lime dust in the bags. According to the employee the burns turned into a fungal infection. Claim is in doubt. (reported 11/19/07)

EE caught his right ring finger between two steel tubes on 10/4/07. On 10/10/07, the EE saw an industrial doctor and was diagnosed with a fracture on the tip of his right ring finger. Placed on modified duty.

On 6/5/07, EE claims that while he was working on the Bucket Elevator, torching off huck pins, his foot rolled over some of the pins on the floor and twisted his right knee. EE did not report incident until the next day. Refused medical treatment as well as filling out injury form. EE stated he wanted to go see his Dr and get a cortisone shot, continued to work rest of week.
