Walking to wash bin and stepped in crack on the ground, rolled left foot.

The employee was startled by an attempted rattlesnake bite. As employee kicked leg outward and jumped away awkwardly in response, employee felt a sharp pain in left knee.

Employee was standing at regrind area when dust/debris was blown everywhere from the wind causing the dust/debris to enter employee's right eye. On 5/17/23 employee was prescribed oral antibiotics and eye drops. Employee has been referred to a specialist.

Employee was walking down ramp carrying a hose when EE lost their footing and hit left knee on the ramp. This caused a laceration. Wound closed with 17 stitches.

EE was performing housekeeping. EE was chipping a build up of dirt with a pry bar & struck the back of hand against a stairway edge. The edge of the metal stairway cut through EE's leather glove & created a puncture wound on the back of EE's left hand. Sutures were used to close the 1/2 inch wound.

While looking for pieces of scrap metal at the cement port on letter belt B at the crusher, employee grabbed a piece of metal and swung arm back to throw and right pinky finger made contact with corner of sign causing a laceration requiring stitches to finger.

Employee was complaining about kidneys hurting and not feeling good. Was treated for dehydration.

EMPLOYEE USED A HOT STICK TO TRIP #4 REGRIND MILL BREAKER. EE HEARD A BUZZ AND RAISED EE'S HANDS TO SHIELD FACE.

While employee was throwing a hanger bolt into discard bin and reported a shoulder sprain. Employee was treated as first aid but after MRI determined surgery was needed. Fist aid until 07/09/2019 at which time employee became a lost time injury.

Clean up on B ball mill washing material from behind mill, employee stepped back into an open launder (grating removed) stepping 2.5' down spraining ankle.

EMPLOYEE WAS ASSISTING IN CHANGING DRIVE BELTS ON #3 SCRUBBER AT THE CRUSHER. EMPLOYEE HAND SLIPPED IN GREASE ON THE BELT AS THEY WERE MANUALLY TURNING. LEFT HAND MIDDLE FINGER GOT CAUGHT BETWEEN BELT AND PULLEY CAUSING LACERATION REQURING SUTURES.

Employee was installing a throat liner on the discharge end of rod mill. While holding the cable with right hand, the liner spun and pinched left ring finger causing laceration.

Employee states that EE was cleaning the #5 scrubber before EE went on break. At break time, EE took glove off and noticed blood inside EE's glove from a laceration to right middle finger. EE is unsure how it occurred.

While employee was washing lime off of light vehicle #165, tripped and fell over a piece of wood causing a laceration to right ring finger that required stitches.

This injury initially was a F/A from 1/26/2017 until the thumb was x-rayed and a fracture was seen on 1/31/2017 at which time it became reportable. The employee was placing a guard on the west side of 7 belt on the short head crusher. EE put one side down and while placing other side the guard pinched thumb between the guard and support beam.

While climbing up the ladder of a loader, employee slipped and caught themself with right arm. Complained of right shoulder pain. Was treated as a first aid, was determined later by MRI employee needed surgery to repair a rotator cuff tear in shoulder. Employee lost no time or had restrictions until 5/4/2017.

Employee was working on 10 tripper and started experiencing cramps in legs and vomiting. Was treated for heat exhaustion at emergency room.

While removing a bearing cap on a 4-15/16" bearing that was about 4' above the #10 tripper, employee was working from floor level as the cap was lowered. It slipped from hands, striking employee in left leg causing laceration.

EE was removing a drain plug at the bottom of hydraulic tank for a loader. When the plug broke loose the employee felt a popping sensation in shoulder. This injury has been treated as first aid and employee was sent for an MRI when it was determined that surgery was needed. On 04/14/2016 were ee surgery and the first day of lost time.

Employee was removing a light bulb using an extended stick. While twisting to remove the bulb from the hugger with right hand, the bulb broke; causing laceration to middle finger.

On the above date, employee reported for audiogram in compliance with the Hearing Conservation program and a standard threshold shift was noted. The audiogram demonstrated a standard threshold shift with an average loss of 10dB or greater at 2,3, and 4kHz in the right ear from the initial baseline and an average hearing loss more greater than 25dB at 2,3 and 4kHz.

Employee was pulling a pry bar, and lost footing from standing onto of bracket to falling 2-3' to ground; impacting right shoulder on ground.

Employee fell; right hand hit kick plate.

On the above date this employee was using a 1" impact gun and felt a sharp pain in upper left arm/shoulder. He has been treated for first aid since then and was out for personal illness for almost a year. Only now receiving any treatment for work injury. He had exploratory surgery and first day of lost time is 03/24/2016. The validity of this injury is in question.

Employee was pulling a turning cable and felt pain in left groin area.

Employee fell approx 24" off ladder resulting in a laceration to the back of the head.

The employee was lining up a liner onto a bowl for crusher repair. The liner made contact with the bowl, it slid down the bowl adjusting itself. The employees foot was below the liner, the liner came into contact with the steel toe of his boot. The liner slid over the steel toe and made contact with the employee's left foot.

Installing shell liner using air winch attached to shell of mill using an eye bolt 2 rows above row being installed. 2 men pushing liner into place a 3rd lowering it A bar was in the liner so it sat at the correct angle while lowering. When pressure was applied to this bar the supporting sling came out of the air winch hook and the load fell on the workers foot.

Employee was struck in the right eye by dust particles from an air blaster.

Employee was struck in the face by a hook on a pulley attached to an overhead crane.

Unknown, employee was found unconscious on stairway due to medical issue.

Low support beams in a work area outside/near filter plant maintenance.

Employee performing pre-op on equipment. When stepping down off bumper with dried mud he slipped and fell to the ground. Employee thought he was fine and continued working. The following the day (Saturday) his back started hurting at home and went to the doctor. Testing revealed employee had contusion of the back and one rib with a closed fracture.

EE was walking down stairway to lower level in crusher building. When employee stepped off the step onto the landing, he rolled his right ankle. During the examination of the accident, there was an uneven surface of built up dried mud on the stair landing. The uneven surface was crowned in the middle about 1 1/2" high & sloped down to both sides, the width of the stair tread.

Descending down ladder, slipped and struck her left side of rib cage on hand rail. Incident was first aid case, X-rays on 9/10/2012, revealed fracture of left rib.

Trying to load an acetylene bottle into a pickup, employee sustained an abdominal strain. Incident was first aid, incident became reportable when employee started to lose time on 6/12 due to surgical procedure.

Employee tripped over the pavement lip in parking lot.

Employee used excessive force in trying to open an ore car. Felt pain in left shoulder area. Sought outside medical attention on 10/03/2011.

Employee stepped over a pipe and felt discomfort in right knee. Incident was first denied as not being work related, comp carrier accepted claim and company was notified on 10/10/2011. Claim is still being contested by company.

Unknown, employee over a period of time working throughout the concentrator has sustained a standard threshold of the right ear in comparison to the baseline audiogram of 8/18/2008.

Unknown, employee over a period of time working throughout the concentrator has sustained a standard threshold for the right ear in comparison to baseline audiogram of 7/25/2000. Received reportable notice on 8/10/2011.

Employee stepped on blade end of hoe, handle came up and struck employee on the mouth. Knocked out two teeth and chipped one tooth.

While removing a portable vinyl partition(curtain), lifting the unit he felt pain in left elbow. Accident was first aid, became reportable on 5/31/2011 due to restrictions.

Doing demolition work, trying to knock down a loose piece of rock/mud, a 6 foot wooden plank with a piece of pipe on the end came loose striking employee on right shoulder, upper back.

Stepping off platform, misjudged distance, fell forward landing hard on right foot. Sustained sprain of foot. Incident was reclassified to Recordable Lost time on 5/25/2011, when restrictions could not be accommodated.

Employee had been climbing stairs, working on his knees. Sustained left anterior knee overuse tendonitis. Incident became reportable on 5/3/2011.

Unknown, employee over a period of time working throughout the concentrator has sustained a standard threshold shift for the left ear in comparison to baseline audiogram of 3/1/2006. Received reportable notice on 4/26/2011.

Working at tailing pump house she was standing in 1 foot of mud. When she attempted to free herself, she twisted her back. Sought outside medical attention on 4/19/11. Incident turned reportable on 4/19/11.

Employee was doing cleanup removing material from under conveyor belt. As he pulled on the rake,right to left, experienced low back discomfort.

Employee lost footing on soft berm material falling backwards. He landed on his left side with his left arm extended behind him.

Employee was operating dozer approximately 10 hours. Right wrist is sore from operating control lever for such a long period of time.

Employee was getting ready to cut vacuum hose. Supervisor told him to wait until truck was turned off. Employee continued to cut hose and in doing so got his face too close to end of hose. His face was sucked to the end and remained until truck was turned off - about 30 seconds.

Using a wrench to open a valve, wrench slipped off valve and struck employee on left knee. Employee did not seek medical attention until 12/28/10

Injured employee and a helper were loading scrap into a scrap bin. They both picked up a scrap idler and were in the process of throwing it into the bin, when the helper let go of his end. The motion of the piece of scrap being let go caught the injured employee's left thumb straining it.

Employee lifted a bucket full of dried cement, in doing so he experienced discomfort in his right shoulder resulting in a sprain.

Unknown, the employee over a period of time of working throughout the concentrator has sustained a standard threshold shift for the left ear in comparison to baseline audiogram of 7/20/2000.

Employee was mixing Nordbak to put in between liners. Mixer was raised up high enough that it spun out and up into his face under his glasses and into right eye.

Employee stepped on wet portion of lime, slipped, lost balance, as he fell grabbed support beam of slaker to stop fall. Sprained left shoulder. Incident became restricted on 9/29/10.

Unknown, employee over a period of time of working throughout the concentrator has sustained a standard threshold shift for the right ear in comparison to baseline audiogram of 7/19/2000.

Was not drinking enough fluids, got dehydrated at work.

While cutting rubber liner with utility knife, knife slipped and employee sustained a laceration of the left forearm.

Tightening a hose clamp with screwdriver. Screwdriver slipped, employee penetrated her left hand with hand tool. Incident became restricted on 6/21/10.Sustained laceration to left hand palm.

Employee was using a battering ram to remove lock nut from a crusher mantle. He felt sudden pain in left shoulder.

While lifting a plug valve to move it, employee experienced discomfort around his belly button. Sustained an umbilical cord hernia. **This turned out to be a strain only**

Employee was repairing cyclone box. He swung hammer to strike the head of a nail, the hammer struck against a pipe below this area, and in doing so, the hammer bounced up striking his left thumb.

Walking on floor, lost balance, fell to floor spraining left ankle.

Walking on uneven ground surface, twisted his right ankle.

Unknown, employee has sustained an STS of the right ear compared to baseline exam, he is a mobile equipment mechanic.

Employee twisted his right wrist when he lifted a wheelbarrow full of material in order to dump load into conveyor belt hopper.

Cut off 12" steel pipe, pipe and support bracket fell on right foot. Sustained fracture

Was washing down area around pumps, he started to move to walk and right foot got stuck in 1.5 inches of mud, caused a popping sensation in right knee. Restricted 11/19/09.

While moving out of the way of a scrap steel container, employee was walking backwards,fell down. Restricted duty 11/17/09

Employee had been lifting plug valves, experienced discomfort in back area

Employee was sitting in control room in front of computer in mill powerhouse .

Working on a motor, using a sledge hammer and a striking face wrench to tighten the bolts, as he was striking the wrench, the hammer slipped smashing left index finger between the wrench and hammer.

Employee was repairing the Section 7 air compressors, experienced some discomfort in neck area

Associate was found unconscious in his dump truck. Truck was stopped with engine off. Associate was given immediate medical attention. Associate was transported by a medical helicopter to a Phoenix hospital. No work related issues found.

While in the process of hoisting a sump pump out of the sump, the employee claims to have felt pain to his right shoulder as he pulled on the chain to the chain block he was using.

The employee was using a pry bar to help un-plug a feeder in the track vault. The bar slipped and he tried catching himself, causing him to wrench his lower back.

The employee leaned over the ledge of a sump pump, to check if the water was draining and in doing so he did not notice a piece of glass from a radiant light bulb inbedded in the accumulation of material built up on the ledge. As he leaned forward he accidentally placed his hand on the glass, lacerating the palm of his left hand.

Employee was washing down a concrete aisleway, started to walk down the incline, when he slipped and fell.

Equipment operator was preparing to dismount from a dozer. He stepped out of the cab onto the crawler track. As he turned to grab the handhold bar, he lost his balance and fell off the dozer onto the ground.

EE was atempting to open a bottom dump railroad car into the track hopper. When the employee pulled on the handle to open it, she felt the doors did not open, so she decided to close the lever and try again. As she pushed on the lever to the close position, the handle snapped back suddenly catching her right ring finger.

Coming down off a slope at the tailings, employee's feet slipped out from underneath him. He lost his balance and fell backwards, while trying to brace himself, he out stretched his arms back. When he made contact with the ground he felt something pop in his left arm.

Employee was replacing eroded floor grating alongside a belt conveyor. He accidentally stepped on a section of grating that gave way. He fell through grabbing himself on the handrail and arresting himself with his retractable fall arrest equipment. Saw doctor 05/21/2009, released with restrictions, employer able to comply.

Employee reports that while setting down a sheave on the bed of a truck, his left index finger tip was caught between the sheave and the truck bed. He sustained a contusion and open wound to the distal end of his left index finger.

Employee slipped on the first step of a stair case, due to an oil slip on the step.

Employee was climbing into pickup which was parked in a manner that created a higher than normal distance for climbing into the cab. He placed his left leg into cab and pushed off the ground with the right foot. He experienced discomfort behind the right knee. Sustained a right knee/upper leg sprain on the posterior side.

Employee was working, kneeling down preparing to hook up the "D" ring on a sling to lift a bearing pedestal when the 10 ton hoist hook assembly struck him on the left side of his shoulder and head.

Employee was working on sump pump using an oxy-acetylene torch to cut off a bearing. A flash occurred causing a ball of fire to come towards him. The ball of fire caused multiple burns to the face, neck and right forearm near the wrist.

Employee was walking down a dirt incline and stepped on some lose material. Her right leg gave way and she caught herself with her right leg. Employee felt a sharp pain in her right lower back that radiated down her right leg. At clinic employee told nurse that pain had been bothering her for the last several weeks.

EE operating the 243 Locomotive. He reported as he was approaching the concentrate hopper cars he went to apply the brakes & they did not work. He hit the hopper cars at approx. 5mph causing him to hit the control panel with his body, two facial cuts below his left eye from his safety glasses & he said his right chest area was sore.

Employee was stepping onto the bottom step of the stairs leading to the regrind deck with his right leg when he felt a sharp pain in his rigth knee.

Employee was walking back to control room in the Mill building when she stepped into a divit in the floor of the grinding area and twisted her right knee. Employee was force to work the overtime shift.

Employee reported accident on 8-15-08. On 8-8-08 Employee stated at end of shift that his knee was sore from working on his knees for two 12 hour shifts in a row. He said that no special event took place but that his knee became sore. He thought that it was just sore from constantly working on his knees. On 8-15-08 he felt that it wasn't improving and needed to see the nurse.

Employee was walking down a hill when she stepped into a wheel rut and twisted her left ankle.

Employee while guiding a load that was being lowered to the repair floor, slipped on a spot of grease twisting his left knee inward.

It was dark and not well lighted. While enroute to the shift supervisor's office he stepped through a 6"-8" gap in the grating covering the hole above row #3 cell #3 in the floatation area. He caught himself with his Rt wrist and shoulder to keep himself from falling.

Employee was in the process of cleaning the bolt on a 24 inch valve. He reported that he had his left hand on the pipe to stabilize himself. The impact wrench was sitting on top of the pipe and it slipped down were employee was working. the wrench landed on employees left hand pinching his small finger between the wrench and pipe. Employee's finger required 6 sutures.

On 4-21/08. Employee mechanic was working on a motor for the # 5 rod mill in the mill building. He reported a 12 pound hammer fell from above striking him in the left upper arm. His arm was between the hammer and motor coupling. He sustained contusions to the upper left arm. On 4-22-08 employee reported to clinic to see doctor and became a restricted case.

Employee was tighting bolts on A-mill with a 1 inch impactor. The air hose broke of at the male end of quick release coupling and the whip hose hit him on side of face. Employee sustained left facial contusion with whiplash type injury with neck strain.

Employee was standing at the discharge end of #3 rod mill. He stepped foward to grab a discharge box, the grading moved causing him to fall into the hole that was approx.20 inches deep. Employee sustained a sprain to the right knee and leg. Case was first aid, become restricted on 4/7/08 after seeing doctor.

Employee noticed a Skirtclamp that was dragging on a conveyor belt. Employee (while belt was running) pulled pin from clamp to adjust clamp. While adjusting clamp, trailing edge caught belt which caused it to jump foward pinning employees left hand between the skirt board and clamp. Employee sustained laceration with tendon, ligament and nerve damage.

trying to aligne launderer bolt hole, employee slipped and pinched left index finger between bar and frame work. No glove was being used due to the 1/4 " bolt that was in hand needed to be inserted

Employee was walking backwards, pulling a water hose and tripped on a pipe. Employee fell catching herself with left hand. Employee sustained a sprain to left wrist.

Employee was descending concrete at Admin building. Near bottom of stairs employee stepped on debris twisting her ankle and fell catching herself on handrail.

Employee was helping unplug tail sample pump in the mill. He was up on the pump, had positioned a flex hose over the discharge pipe and was holding it in place. Pump was turned on, he was unable to keep flex hose in place and consequently hot mud sprayed on his right arm and right side of face resulting in first and second degree burns.

Employee reportedly was helping other workers move grinding rods in the mill. The crane pulled on the cables causing the rods to shift and in doing so, the cable and hook pinched employee's right ring finger resulting in an open wound that required sutures to close.

Employee reported that while trying to pull cable from under rebar tube, cable got stuck between ground and rebar structure. Employee tried to throw remaining 10 feet of cable over structure, in doing so sustained a right anterior thigh strain-acute.

Employee reported installing liners at ball mill; while handling a liner his right hand was caught between the outer and intermediate liners, smashing his right hand. This resulted in a contusion of the hand.

Employee was cutting a 2 inch hose, overhead. When the hose fell to the ground the employee tried to hold on to it, causing a left shoulder sprain.

Employee was using a come along to change belts on a scrubber. He was backing off come along when it released, causing him to hit his left elbow on a metal plate. Received a 1 cm x 1/2 cm laceration.

Employee was walking to foreman's office when she stepped on uneven concrete floor, twisting right ankle. Employee did not have her work boots laced up.

Employee was walking down stairs to 4-5 feeder pit, stepped onto concrete floor, twisting right ankle.

Employee was dumping ore from a wheelbarrow into a chute. While lifting wheelbarrow, lost center of gravity, causing wheelbarrow to tip over to the right side, striking left knee with handle.

While EE was repairing a wooden table he pinched his right thump between the pry bar and the table support.

While dismounting loader employee slipped on ladder with right foot. Stopped his fall but caught his left foot on ladder. After about 10 minutes started to feel pain in left groin area.

Contact made with employee on 7-12-07, confirmed that he did have surgery. Still do not have any medical data to support claim nor does our workers' compensation carrier. On 1-18-07 employee was walking down stairs, carring a box of tape, slipped on bottom step, fell to ground on right knee and right elbow.

Shoveling spillage when shovel handle struck her right hand.

While adjusting the packing of C-1 Tailings pump, employee pushed 10" wrench to tighten the packing. Employee felt a "pop" and then experienced a lot of pain to his left arm and left shoulder.

Employee sought medical treatment on 1/3/2007, at this time his injury became a reportable injury. Employee was walking through mud, slipped and twisted right knee, while trying to catch himself, twisted left knee.

Employee stepped on a piece of grating. Grating slipped causing him to fall through approx. 2 feet. He sustained a left shoulder sprain, contusions to left 4th and 5th fingers, and a right knee abrasion.

Employee was walking up stairs, when left boot got caught on step. She tried to catch herself to avoid falling, felt discomfort in lower left back area.

EE was repairing discharge screens. His feet were planted, when he turned to his left, twisting his right knee. He was picking up a discharge screen.

Employee was taking a piece of 1.5" pipe out of the storage rack and a piece of flat bar .5" x 2" x 6' caught on the pipe and was pulled out from rack. The flat bar struck the employee on the top of the left foot.

Employee while cleaning track hopper feeder, was struck by a small piece of rock. Employee sustained a contussion to his right 5th finger.

EE was cutting a piece of rubber skirting with a razor knife. The knife slipped and he sustained a one inch laceration to the posterior side of the middle joint on the right thumb.

EE ws cleaning a lube pump, while standing on a wooden slat. He did not step off completely, catching his right foot on the edge of the wooden slat and the floor causing his right knee to buckle outwards.

While operating a 553 Bobcat, employee sustained a crushing injury to her right foot. Claims she was bounced around in the cab and when she tried to reposition her body, her foot slipped off the cab frame. When she lowered the bucket, it pinched her foot between the cab and the bucket arm assembly.

Employee was assisting pipefitters in the installation of a 4" plug valve. The valve slipped pinching left 5th digit between plug valve and saddle. This resulted in a laceration from the tip of the finger to the first joint.

Pinched left fifth finger while unloading grinding rods onto holding rack.

Employee was stringing welding cable, he threw the cable over a seven foot high guard he experienced discomfort.

Employee observed a co-worker standing with his back to a radiant heater. The co-worker's coveralls had caught on fire. Employee along with another employee assisted the co-worker in extinguishing the fire and removing his coveralls.

Employee reported he was trying to unchoke cone of the lime tank. Dry lime sprayed and got inside the employee's boots on his feet. Note: Injury became Restricted Reportable on 06/01/05 after doctor placed work restrictions on employee.

EMPLOYEE REPORTED HE WAS BOLTING A FLANGE USING AN IMPACT WRENCH. WRENCH SLIPPED OFF OF NUT & EE INJURED HIS BACK.

EE REPORTED HE WAS REMOVING AN OLD BEARING. EE'S PARTNER STRUCK THE BEARING RACE WITH A HAMMER AND A PIECE OF STEEL FLEW OFF AND HIT EE IN THE LEFT UPPER SHOULDER AREA. A SMALL PIECE OF STEEL (APPROX. 3/8" X 1/8") WAS REMOVED BY DOCTOR. EE WAS PLACED ON RESTRICTED DUTIES AT THE TIME OF THE REMOVAL OF THE STEEL.

Employee reported he was loading the silo ball bin with 5,6,7 overhead crane when dust fell into his eyes. Note: Injury became reportable restricted on 4-29-05 when Dr gave ee work restrictions.

EE WAS CLIMBING DOWN VERTICAL LADDER, SLIPPED AND FELL APPROX 4 FEET. EE LANDED ON METAL GRATING, SUSTAINING A RIGHT ELBOW CONTUSION.

It was determined this date that said miner sustained hearing loss in his left ear. Employee works in various areas of the plant performing Boilermaker activities.

Employee reported he was walking in front of the Air Conditioning shop when he fell on his right side hurting his left side/back. Note: Injury became a Reportable Restricted Injury when doctor gave employee restrictions on 03/09/05.

EE reported he was walking on a wooden floor when an improperly secured trap door allowed EE to fall through to the next level. Note: EE lost one day of work, because of additional required x-rays by doctor.

EE WAS DIAGNOSED WITH A STANDARD THRESHOLD SHIFT BY DOCTOR IN HIS LEFT EAR WITH AN AVERAGE LOSS OF 10 dB OR GREATER AT 2, 3 AND 4 kHz FROM HIS INITIAL BASELINE. EE RECIEVED WRITTEN NOTIFICATION AT WORK.

Employee was diagnosed with a standard threshold shift in left ear with an average loss of 10 db or greater at 2,3, and 4 khz from the initial baseline. EE is presently off of work due to a personal surgery.

EMPLOYEE REPORTED HE WAS TAKING A SHOWER AT THE MEN'S HOURLY CHANGE ROOM AND WAS STUNG ON THE END OF THE RIGHT INDEX FINGER BY A SCORPION. DOCTOR TOOK EMPLOYEE OFF OF WORK FOR TWO DAYS.

Employee reported he was welding inside of rod mill. Employee's right foot was against the shell of the mill. Employee's right foot received burns where the right foot was in contact with the shell of the mill. ee did not feel the burn when it was happening. accident was a restricted duty accident, because of the restrictions given by the attending physician.

EMPLOYEE REPORTED HE WAS DRILLING A HOLE NEAR THE END OF A PUMP SHAFT FOR A COTTER KEY WITH A HAND HELD DRILL, WHEN THE 9/64 INCH DRILL BIT BROKE. EMPLOYEE RECEIVED A PUNCTURE WOUND AT THE BASE OF THE MIDDLE FINGER ON HIS RIGHT HAND. EMPLOYEE RECEIVED SUTURES AND RESTRICTION FOR USE OF RIGHT HAND, WHEN HE WAS TAKEN TO THE DOCTOR.

EE REPORTED HE WAS WALKING DOWN STAIRS AND FELT HIS KNEE "POP".

EMPLOYEE REPORTED HE WAS HOOKING UP THE TURNING CABLE FOR 6A BALL MILL. HOOK CONTACTED EDGE OF CYCLONE STAND CAUSING DUST TO FALL INTO HIS LEFT EYE. IMBEDDED FOREIGN BODY WAS REMOVED THE NEXT DAY FROM HIS LEFT EYE WHEN HE SAW THE DOCTOR FOR THE FIRST TIME.

EMPLOYEE WAS HELPING REPLACE A STANDARD CRUSHER HEAT EXCHANGER WHEN A FOREIGN BODY WENT INTO HIS EYE. IMBEDDED FOREIGN BODY WAS REMOVED 3/29/04.

EMPLOYEE REPORTED HE WAS WELDING AND SOMEHOW GOT AN OBJECT IN HIS EYE. NOTE: FOREIGN BODY WAS REMOVED ON 03/26/04 AT THE HOSPITAL EMERGENCY ROOM.

Employee reported he was lifting an outrigger pad onto a crane and felt his right shoulder pop. Note: Injury became reportable-restricted on March 17, 2004 when dr gave EE restrictions.

EMPLOYEE REPORTED HE WAS WALKING UP AND DOWN STAIRS AT THE MILL BUILDING. EMPLOYEE'S KNEE STARTED TO HURT AND STARTED TO SWELL. NOTE: INJURY WAS ORIGINALLY DENIED INDUSTRIAL STATUS, BUT WAS ACCEPTED BY WORKERS COMPENSATION AS AN INDUSTRIAL INJURY ON APRIL 01, 2004.

EE REPORTED HE WAS INSPECTING THE CRUSHER, STEPPED BETWEEN THE GAPS ON THE FLOOR COVERS ON THE TAKE-UP, LOST HIS BALANCE, ALMOST FELL THROUGH, AND STOPPED HIMSELF CAUSING A RIGHT THIGH STRAIN. INJURY BECAME A RESTRICTED REPORTABLE ON MARCH 01, 2004.

EMPLOYEE REPORTED HE WAS SHOVELING IN THE FEEDER PIT & EXPERIENCED PAIN IN HIS MIDDLE BACK. NOTE: INJURY WAS ORININALLY DENIED BY WORKERS CONPENSATION, ADVISED ON 09/27/04 THAT THIS CLAIM HAD BEEN ACCEPTED. THE CLAIM WAS NOT ACCEPTED AS ON THE ORIGINAL INJURY OF 7/9/03, BUT AS A NEW INJURY WITH THE ABOVE DATE.

EE REPORTED HE WAS WALKING, TRIPPED & TWISTED HIS LEFT KNEE. NOTE: INJURY BECAME REPORTABLE-- RESTRICTED ON 12/31/03.

EE REPORTED HE WAS ATTEMPTING TO DISLODGE STEEL BALLS OUR OF A SUMP WITH A METAL BAR WHEN A PIECE OF STEEL SPLINTER HIT HIM IN THE LEFT CHEEK.

EE REPORTED HE WAS PULLING A TOOL BOX OUT OF A TRUCK, LATER IN THE DAY HE FELT LOWER BACK PAIN

NOTIFICATION WAS RECEIVED FROM THE MEDICAL DEPT. THAT A STANDARD THRESHOLD SHIFT HAD OCCURRED. DATE OF NOTIFICATION WAS 8/25/2003. NOTIFICATION WAS RECEIVED ON 9/8/2003.

EE WAS REMOVING LIGHT BULB, WHEN BULB BROKE CUTTING HIS FINGER.

EMPLOYEE WAS WORKING ON THE THROAT LINERS ON #7 ROD MILL, CHIPPING CEMENT WITH A CHIPPING GUN. THE CHIPPING GUN SLIPPED AND THE BLADE OF THE CHIPPING GUN STRUCK THE EMPLOYEE ON THE INSIDE OF HIS LEFT KNEE. INJURY BECAME REPORTABLE ON 8/27/03 WHEN THE EMPLOYEE RECEIVED A DOCTOR'S CARE.

EMPLOYEE WAS SHOVELING ON #2 BELT, STARTED TO HAVE BACK PAIN.

EMPLOYEE REPORTED MOVING AC UNIT ON DOLLY, DOLLY WAS STUCK IN DOORWAY, WHEN PUSHED, DOLLY MOVED FORWARD CAUSING EE TO SLIP AND HIT R KNEE ON FLOOR.

EE REPORTED HE WAS WALKING DOWN STAIRWAY TO #2 BELT, CARRYING A SHOVEL AND A RAKE ON HIS SHOULDER; HIS FOOT SLIPPED CAUSING HIM TO FALL ON HIS BUTTOCKS AND SLIDE DOWN STAIRS (8-10 STAIRS) WITH HIS RIGHT LEG BENT BACK UNDER HIM.
