Employee walking on a catwalk tripped over piping running along side of catwalk grating floor impacting stomach with a water line valve at handrail level.
Employee tripped on stair step reporting to work striking elbow on handrail while attempting to catch balance. Elbow extension required doctor to render a shot of cortizone and prescribe physical therapy.
An gate between the lime kiln and the stone feed weigh belt was not closed and the stone feed weigh belt was exposed to hot kiln gas and caught on fire. Attempts were made to extinguish the fire but due to winds, the fire continued to smolder and required more lenghtly fire suppression as personnel could not disturb embers behind the conveyor belt guarding until cooled.
An employee was assisting with servicing operations of heavy equipment. While dismounting the lube/fuel truck, on the passenger side, the employee stepped into a rut directly below the truck's running board causing his ankle to twist. The employee was taken to a medical facility and treated for a sprained ankle.
An employee's previous back injury was re-aggravated when a rock rolled out of a loader bucket and struck the side of the haul truck bed he was operating.
While changing out a hammer on a track drill, a 48" pipe wrench slipped on rotation and grazed the maintenance tech's right side.
An employee had an allergic bronco-pulmonary reaction due to aspergillosis mold exposure while operating a Caterpillar 988-G front end-loader.
While driving down a haul road, the haul truck struck a soft spot causing the operator's seat to bottom out. When the seat bottomed out, it caused a lower back and neck strain to the operator. The soft spot was removed and filled in with dry material.
MINER WAS EXPOSED TO FUGITIVE DUST WHILE INSTRUCTING A TRUCK DRIVER ON LOADING A TRUCK AND EXPERIENCED A CHEMICAL IRRITATION TO THE EYE AS A RESULT OF THE DUST REACTION WITH RAIN WATER DURING HEAVY RAINFALL.
oPERATING A CAT 988G LOADER LOADING HAUL TRUCKS. "WENT INTO THE BANK" AND STRUCK A HARD SPOT. THIS CAUSED AN ABRUPT "STOP" RESULTING IN "WHIPLASH" LIKE INJURIES.
BACK STRAIN WHEN REMOVING METAL COVER FOR A CONVEYOR BELT.
SECURITY OFFICER WAS PATROLLING THE MINE SITE WHEN UNKNOWN SUSPECT(S)- PROBABLY ILLEGAL ALIENS- BEGAN STONING HIS TRUCK. WHEN HE TURNED TO LOOK IN THEIR DIRECTION, THE TRUCK DRIFTED TO THE SIDE OF THE ROAD, WHERE A HEAVY FORKLIFT BLADE WAS STORED. THE OFFICER TRIED TO STOP, BUT PRESSED ACCELLERATOR INSTEAD OF BRAKE PEDAL. THE TRUCK STRUCK THE BLADE, PITCHING DRIVER FORWARD.
EE WAS MARKING DRILL HOLE LOCATIONS IN PREPARATION FOR DRILLING. HE SLIPPED ON A ROCK CAUSING HIS RIGHT KNEE TO HIT THE GROUND SCRAPING IT & EXTENDING HIS LEFT LEG(IN A STRAIGHT POSITION).
EMPLOYEE WAS CHANGING A RETURN IDLER ON #2 BELT WHILE STANDING ON AN INCLINED BANK UNDER THE CONVEYOR FRAME. HE HUNG HIS 2# HAMMER BY THE HEAD ON THE CONV. BELT. HIS FEET SLIPPED ON THE BANK A ND HE PROBABLY HIT HIS HEAD ON THE HAMMER HANDLE WHEN HE FELL. THE HAMMER HIT THE BACK OF HIS RIGHT HAND BRUISING IT.
EMPLOYEE HAD PARKED PICKUP ON THE RAILROAD TRACKS AND WAS CLEANING OUT THE BED. HE THREW OUT THE MAT (ONTO THE RAILROAD TRACKS) AND SWEPT OUT THE BED THEN WHEN HE STEPPED OUT OF THE TRUCK BED ONTO THE MAT HIS FOOT LANDED ON THE RAILROAD TRACK TWISTING HIS LEFT ANKLE.
EE STEPPED ONTO A PALLET AND AROUND EQUIPMENT STORED ON PALLETS TO REACH A TUBE PART STORED ON UPPER SHELF. WHEN REACHING UP HE LOST HIS BALANCE AND FELL BACKWARD OVER THE EQUIPMENT ELECTRIC M OTORS ON PALLETS SPRAINING HIS RIGHT WRIST/ANKLE/KNEE.
EE STEPPED OUT OF WATER TRUCK AND HIS FOOT SLIPPED OFF THE RUNNING BOARD. HE GRABBED A BAR TO KEEP FROM FALLING HURTING HIS RIGHT SHOULDER.
EMPLOYEE WAS LOADING 40-LB. BAGS OF GYPSUM INTO WAIST-HIGH CHUTE. BAG FELL INTO CHUTE AND WHEN HE TRIED TO PULL IT LOOSE, IT WAS STUCK; HE INDICATED THAT HE GOT FRUSTRATED & PULLED HARDER ON THE BAG; WHEN IT LET GO, HE FELT HIS LEFT SHOULDER POP.
EE WAS SHOVELING SPILL INTO OPENED DOOR OF BIN WHEN HI FOOT SLIPPED AND HE FELL PART WAY INTO BIN AND AGAINST THE BIN DOOR WHICH SCRAPED HIS LEFT SIDE AND HIT HIS LEFT MIDDLE FINGER.
