Employee was on the dredge cutting a cable when he realized the did not have his hammer. While retrieving his hammer he tripped over a lifting eye that was mounted to the floor of the dredge, falling against the cab injuring his left shoulder.
WHILE HANGING A ROOT RAKE ON A DOZER, EMPLOYEE WAS HAMMERING ONE PIN IN WITH ANOTHER PIN SMASHING HIS RIGHT HAND LITTLE FINGER BETWEEN THE TWO PINS, SPLITTING IT OPEN.
