A bolt from the cheek plate in the primary jaw crusher dislodged and struck the employee in the mouth, causing a laceration to lip and broken teeth.

Employee shift started at 4PM on July 10, 2024. Incident happened July 11, 2024 at 1:25 am. Employee fell on right knee, causing a laceration, while inspecting screens and wash deck. Employee received sutures and was released to full duty.

Employee was assisting a coworker that was drilling through a piece of copper bar. EE was holding vise grips that were clamped to the metal when the drill slipped and hit the back of their hand, causing a laceration. EE sought medical attention and is currently on restricted duty.

Employee was changing a belt on conveyor when EE slipped on the toe board and scrapped EE's left shin. EE refused treatment at the time but reported to EE's supervisor on January 5, 2018 that EE felt like the wound on EE's left shin was infected and requested medical attention. EE was diagnosed with lower extremity cellulitis and is currently losing time.

Employee was lifting a piece of metal that he was replacing on a conveyor when he lost his footing. As he stepped back to regain his balance he felt a pop in his upper back near his shoulder. He sought medical attention on December 5, 2014.

EE was operating an excavator in the pit constructing a catch berm. Rocks fell from the wall and struck the employee on the knee. The accident resulted in a broken knee cap and lacerations to his right leg.

Alledged Claim 2/19/09 changed from Code 5 (under investigation) to Medical Treatment 05/01/09. Employee asked for someone to relieve him; felt he was going to pass out due to blood pressure, wanted to go home & take meds. Next morning suggested he may have hurt back when boulder shifted in his bucket and jarred him. Did not mention back to his supervisor on day of incident.

Employee was unlocking rail car brake with a brake stick. The brake stick slipped off of the brake wheel as the brake broke free. This atcion caused employee to shift off balance, twisting his knee. This was a very tall car cauing employee to have to stretch to reach. Internal investigation was conducted by Risk Management; claim has been accepted as compensible claim.

Increased back pain, returned to Dr. 12/12/06. Dr. rx MRI, time off for 2 weeks. Emp. was re-eval. 12/14/06, following reading of MRI,was released to return back to work 12/18/06.
