Employee was installing a roof bolt when the employee got his right index finger pinched between the roof bolt plate and the mine roof. Employee did not become a lost time accident until he had surgery on his right index finger on 10-29-08.

Employee was helping the roof bolter operator install a 6 foot resin bolt, as the employee was putting the head of the bolt in the bolt wrench, a rock fell and hit the emloyee on the head and shoulder area.

Employee was helping re-direct the fire suppression sprsy outlets at #2 belt header, when employee slipped and landed on his rescuer on his right side. The area at the header is on a steep grade and wet.

A roof fall occurred on the No. 1 section in the number 3 entry cross cut at spad 7135. The roof fall measured approximately 30 feet long, 30 feet wide, and approximately 9 feet in height. The roof fall fell on the No.2 MBC, No. 3 MBC,and portion of the belt tailpiece. The No.2 MBC operator was trapped inside the operator compartment for a brief time span but suffered no injuries.

The employee was helping make a belt splice on the #1 belt, when the belt splicing tool got jammed. As the employee was helping to free up the belt splicing tool, his left hand got caught, causing a fracture to his left thumb.

The height and distance of the roof fall can not be determined due to geological conditions and accumulation of water in the face area.
