The screen needed more oil and to be greased. The injured employees added oil. The employee that got injured decided to put their left hand between the timing belt and sheave to check if the bearing was "locked up," the counter weight fell freely pulling their glove and left hand between the timing belt and sheave. Injured employee had 3 fractured fingers.

Rogers Group Inc · Over-exertion in wielding or throwing objects

Employee was a using a 8 pound sledgehammer to hammer on the outside of a bin to knock down material that had built up inside. In the process of swinging the hammer EE felt a pop in the right shoulder. Employee was taken to a clinic and observed and released to full duty, no medications, and no restrictions. MRI reviewed on 3/23/22 showed no damage, but PT was recommended.

Two EEs were carrying a piece of screen media up to the feed end of the scalp screen. One EE was walking backwards, injured employee walking forward. IE stumbled & stepped back a step, in doing so rt foot & leg went into an access hole for a chute gate lever. IE's lt leg went to the floor of platform, rt leg went thru the hole. IE's groin area struck toe board around access hole.
