Employee experienced left hip pain while replacing a hydraulic hose on an excavator. Doctor reviewed MRI results on November 30th 2022 and placed employee on restricted duty.

Employee was walking around conveyor investigating cause of a belt failure. While looking up at the structure EE's ankle rolled, causing EE to fall to the ground. EE sought medical attention on June 25, 2019 and is currently on restricted duty.

On November 23, 2009, employee strained his back while performing work activities in the plant. Employee felt pain in his lower back after shoveling, lifting a rock out of the roadway, and pulling a water hose. Employee did not request to seek medical attention until 12/9/09. This accident is being reported on 12/16/09 which is within 10 days of seeking medical attention.

Employee and co-worker were attempting to reline the chute underneath the primary crusher. After heating the plate injured, employee instructed co-worker to hit the plate with a hammer which caused a rock to become loose from a shelf underneath the crusher and strike employee's head above his right ear.

Employee was straightening a head lamp gaurd. While heating and bending the guard in a vise he was pulling with a cresent wrench. The wrench slipped causing employee to fall on his left wrist.

EMPLOYEE WAS ATTEMPTING TO ATTACH A HAUL CABLE TO A BARGE FOR LOADING. THE SWIVEL WAS LAYING AGAINST THE BARGE AS IT WAS LIFTED ONLY ONE END OF THE SWIVEL RELEASED ITS ENERGY. AS THE EMPLOYEE WENT TO ATTACH THE CABLE THE BARGE MOVED CAUSING THE OTHER END TO OPEN. EMPLOYEE RECEIVED A DEEP BRUISE TO THE LEFT INDEX FINGER.

ON FRIDAY 4/2/04, HE DECIDED HE NEEDED TO SEE DR. & WAS DIAGNOSED AS LOW BACK STRAIN & PLACED ON LIMITED DUTY.
