Employee adjusted the grizzly bars at the primary crusher and failed to close the access door before the feeder started back up. While closing it a rock fell onto the door and injured EE's finger, requiring two stitches.

Employee was helping with maintenance on feeder. While climbing on the second step of a four foot , the ladder shifted and caused him to fall back against the hitch on the welding truck. Employee suffered from a cracked rib and contusion to his back.
