Employee was changing screen wires and hit finger with a hammer, causing a laceration. Employee sought medical attention on May 26, 2021, received five sutures and placed on restricted duty. Employee was released to full duty on June 2, 2021.

A repairman was installing a rotor shoe in the crusher and asked for assistance. Another employee was holding the shoe with both hands while the repairman used an eight pound sledge hammer to hit it in to place and struck the employee's right hand. Employee suffered a fracture to the right little finger and was placed on restricted duty.

Employee was working from a small boat performing maintenance on the frame of pump barge. Around 4pm lightning struck a power pole about 50 yards away from the pumps. The employee tensed as the lightning struck, straining a muscle in right shoulder. EE was taken to the doctor and released to restricted duty.

Two employees were removing shafts on a feeder pan housing at the primary. While pulling out one of the shafts, the injured employee put his hand between the chain of a come-a-long and the feeder frame, smashing his 5th right finger. He sought medical attention, resulting in about 1/4 inch of bone amputation of his 5th right finger.

While an (Vulcan Contractor Services #01-03226) employee was performing annual ground checks at the Tuscaloosa Quarry #01-03087, he experienced an electrical arc flash. He sustained 1st and 2nd degree burns to his left hand, forearm and face. Employee did not follow electrical procedures and was not wearing the proper PPE that had been provided for him.

Employee was adjusting setting on jaw crusher while turning jam nut with pipe wrench, adjustment (bolt) rod broke. Spring was under pressure, spring came off and hit employee in chest.

While gassing up the air compressor, another employee tossed a socket approximately 15' - 20' up to a work area. The socket missed the work area and fell on the employees left foot.

Employee was dragging a 2" hose and slipped on a patch of ice. He took it easy and put ice on his R knee for several days. He later decided to go to the doctor and was diagnosed with a strained R knee and wrapped with ace bandage. On 1/28/08 it was decided because of his size and weight he needed to have a special brace ordered to immoblize the knee to aid with healing.

Employee became trapped between F-250 pickup and haul truck when he walked between the vehicles. Driver of haul truck accidentally disengaged the park brake and rolled into injured employee. Th injured employee was switching between vehicles to receive training on haul truck.
