Alternative fuels material storage began self heating. Result was a fire involving saw dust and plastic alternative fuels. No property damage or injuries.

While retrieving the blend silo #2 sample, there was a strong draft that caused the tech who was holding a 4ft sampling stick to be pulled against the air slide. Diagnosed with shoulder strain and provided prescription meds/light duty.

Employee tripped over a wheel chock rope and shattered their knee cap.

A Norrell Construction elevator technician fell while climbing preheater tower elevator car ladder. In an attempt to not fall, the technician grabbed the ledge above the ladder, resulting in lacerations to several fingers and 2 fractured finger bones.

Employee sustained a laceration to their left forearm from a dial pen that protruded further than the gate valve it was affixed to.

Employee walked through material and stepped on something under the material. Their reaction led to a soft tissue strain in their back that has progressively increased in pain over time.

Employee was walking and tripped, fell onto their left side and injured their left shoulder.

Jib from the mobile crane fell about 5' from the boom, bounced on the deck of the crane then struck an employee's foot behind the steel toe.

Employee was using a press in the maintenance shop. While holding a piece of metal plate and jogging the punch to ensure the die was in the correct position, part of the press made contact with the employee's left thumb.

An electrical disturbance caused a partial power outage, which caused ID fans to stop and allowed hot gasses from the precalciner tower to contact solid alternative fuels within the bucket elevator.

While rigging a walk-behind, EE was installing a shackle in order to fly equipment off the pre heater tower when another employee pressed the controls lowering the bucket on EE's hand causing a contusion to the hand. The device was de-energized but due to the manufactured settings the bucket can still be lowered without power.

Employee was descending stairs, when they misstepped two stairs from the bottom. The employee then fell two stairs to floor landing, where they struck their left knee. Incident became reportable on 02/10/21 when the employee was prescribed physical therapy.

Employee was struck on right knee by 5.5" Kiln brick that fell approx. 4-5ft. *Became recordable on Oct. 2, 2020 when employee received prescription medication.

An iron worker was performing welding tasks when during the work shift received arc flash burns to the eyes. It was determined that the light from the sun was behind the welder and entering EE's welding shield that caused the automatic lens to malfunction.

The employee was attempting to loosen a valve when the pipe wrench employee was using slipped, causing employee to fall and hit shoulder. Employee was working from the ground and not an elevated platform.

Employee stepped onto a concrete pylon and felt pull in right foot.

Employee was trouble shooting a problem on the tower around the O2 probe. While working around the probe a hose ruptured and sprayed steam and glycol on the employee. As a result the employee sustained burns to left hand.

Employee was moving a sample table. The table began to fall, employee tried to stop it from falling over, this action caused strain to the employee's shoulder. The employee was taken to the hospital and examined. It appears to be a strain possible slight tear. Employee was placed in a sling and will be examined again in one week.

Employee was on the 5th floor of the Pre-Heater tower conducting a visual walk around inspection when hot gases and hot material escaped unexpectedly near the tipping valve of the 4B vessel. Employee sustained burns to his head (nose, eyes, left ear, face and forehead), neck, forearms and left elbow.

During the bricking of the kiln employee was passing brick, placing them on the brick table. The Brick masons were in the process of keying out a ring of brick. employee had his hand on the brick table the employee turned to pickup another brick off the pallet. when his back was turned a brick fell from the key window and struck him on his hand he had resting on the table.

The following contributed to the injury: The weight of the inspection door, limited working room due to a 3" line, lack of help from a second employee and position of the employee's hand as he pushed the inspection door back into it's original position. The employee suffered from an avulsion to the soft tissue on the tip of the right index finger.

Slipped on a piece of metal and fell off the edge of it. When he fell he hit his leg on a piece of metal and bruised his leg.

While EE was actuating a slide gate valve in the raw mill, the EE was burned on both forearms when material flushed from silo. The EE was treated for 2nd degree burns to his forearms.

Employee started work finishing loading a cement barge with work partner. EE left work partner to go to the bathroom on the company tug boat (2nd time). When EE did not return, work partner went to look for EE finding him unconscious in the tug boat bathroom. Site First Responders rendered CPR until relieved by local EMS who pronounced EE deceased on site.

The employee was getting a rivet buster (jack hammer) to remove buildup off the 5th floor finish mill roof. He dropped the rivet buster and it landed on his left foot point first. cutting and fracturing his left big toe.

EE mounting 988G when step broke at weld. EE swung out and held onto handrail with left hand and all body weight. EE strained shoulder. EE given modified duty on 5/1/12.

EE was removing hood of S Elevator (FM2) with overhead hoist. Dust blew off hood as it was positioned in front of fan exhaust. EE attempted to move out of way and tripped over conduit in-between elevator housings. EE landed on shoulder. EE sought treatment on 4/4. Given pain medication and restricted duty for 4 days by clinic.

EE was removing lower air chamber section of 10' air slide with another EE onto a forklift. EE's thumb was caught between the fork and air slide when it was unexpectedly moved by other EE. EE had nail removed and sewn back on as a precaution. EE working with restrictions.

At 11:30PM, Holcim EE's noticed flames at Geocycle, LLC (ID# R216) alternative fuels shed. Fire department contained fire by 12:15AM. Fire out at 2:38AM when FD left site. Roof of shed was damaged beyond salvage. Building is being stabilized to allow removal of remaining material. RCFA initiated. AFR policies and procedures will be updated to prevent occurrence.

EE was assisting another EE cutting pipe on chop saw. While moving next section of pipe, EE right hand made contact with still spinning blade. Saw had been shut off, but blade continued to spin down from previous cut. Cut went through leather gloves and into EE right thumb which required stitches to close. EE on restricted duty.

EE's were pushing tail pulley of drag conveyor with hydraulic ram to remove shims. EE was hit on right thigh by hydraulic ram when it slipped out of position under pressure. EE sustained Medial Collateral Ligament Sprain to right knee. EE working with restrictions.

EE using portable grinder with cut-off wheel to remove section of dust collector duct when wheel got stuck in pipe. Grinder kicked back and hit EE in chest and left cheek under face shield. Treated at clinic, left cheek needed 9 stitches, return to work, no restrictions.

The miner was walking down a set of stairs in the clinker pit. When he stepped off the bottom step his foot hit an unlevel place on the floor and he rolled (sprained) his left ankle and fell on the floor. He was taken to the ER room.

EE was removing thimbles from swirl chamber and as he was pulling one out of the access hole, some hot material went into the top of his glove. The hot material (kiln feed) caused a burn to the top of EE's right hand (little larger than 1/2 dollar). EE was wearing proper PPE. EE did not report incident until 11/8/10, at which time he was sent to doctor. Wound care/antibiotic.

EE used the defeater mechanism to enable himself to open the energized 4160 VAC enclosure, reached in and touched the hot fuse with his right hand causing a 3rd degree burn to tip of right thumb & index finger. 1st degree burn on palm of left hand. EE was transported by mobile fire & rescue to medical clinic burn center. Paramedic said injury was not life threatening

EE was on a ladder repairing florescent light fixture in 9' ceiling in the plant conf. room. EE broke his right arm when he caught himself after missing last step of the ladder as he was climbing down.

EE cut his left index finger, requiring 3 stitches. EE was changing out stainless steel seal segments on the feed end of the kiln, when one of the segments riding (worn) edge cut through his glove and cut his finger.

EE was working on the ground when a beam clamp fell from the 4th floor & struck him in his head. He did have all required PPE. The clamp was being removed by employee on the 4th floor

Employee was utilizing an & acetylene torch to cut a hole in the cone of a limestone silo. Employee had safety glasses & burning sheild on. After employee completed burning 4" hole he flipped his face sheild up & limestone dust fell from the bill of hard hat behind his safety glasses into left eye. Employee was sent to Dr where eye was cleaned. Kept out of dusty areas.

EE was loading bulk trucks from rail loading bay. After loading one of the bulk trucks, a bulk loader went on top of truck to close and secure the trucks loading hatch. Truck driver left loading station, went to his truck, got inside, cranked it up and proceeded to extit the rail loading bay with EE still on top of truck. EE caught himself, cut over right eye, 5 stitches.

We had a near miss when cardox tube went off unexpectedly while being loaded by production worker. Tube was in loading fixture when it discharged.

EE was moving cable on a barge, when he twisted at the waist and felt a pain in his lower back. Strain, lumbar region. Preexisting degenerative disc disease/lumbar.

Employee was working on airslide which conveys portland cement and as he was removing the bottom of the airslide, cement dust got in both eyes. He was sent to doctor's office but since it was Saturday afternoon it was closed. Employee was taken to local hospital where eyes were washed out. No follow-up visits were needed.

Employee was holding a shaft to prevent it from turning while another employee was securing a locking collar on a bearing using a pin punch to tighten the collar. As the collar was struck a piece of the collar sheared off and embedded in the employee's left arm which required one stitch.

Employee and two maintenance mechanics were replacing flights and return rollers on pan feeder. The bolts securing one of the flights had been removed. Employee was pushing on flight with left hand. The flight dislodged and as it fell, it struck employee's left hand causing a fracture to middle finger and back of hand (3rd metacarpal).

EE was making a connection of scuba bottles to filter rack preparing for advie job to inspect ship hull. He connected the hose from these bottles into down steamed of the filter rack, by passing the high to low pressure regulator. When system was pressurized a pneumatic explosion occurred. Striking him in the face with debris

Employee was disconnecting a temporary electrical connection and was using a box blade knife to cut the electrical tape. The razor went through the tape, glove and cut about a one inch cut in left index finger, requiring five stitches.

At the start of employee's shift on 2/6/06 he reported that he woke up with irritated right eye. He had been grinding the day before and assumed he had gotten a foreign body in his eye. He was sent to doctor who flushed eye out but saw nothing. A prescription was given. The next day he still felt something in eye and went to an eye doctor who removed particle from under eyelid.

A loose connection on a transformer caused a power failure to the plant air compressor which caused exhaust valves to close on the Pyro main baghouse which allowed hot gases to reach the dryer feed belts and started a fire. The Mobile Fire Dept was called and they put the fire out in less than an hour. Valves will be installed to prevent future like incidents

Employee was working on kiln floor installing a monorail. There was welding in the area and clinker dust on the burner pipe. Employee was not sure when or what got into his left eye. Employee reported eye irritation 7:00 a.m. next morning. A small piece of material was removed from eye; EE's eye felt better. Next day still some irritation; sent to doctor. Eye drops prescribed.

Employee was hooking up portable air compressor while he was lifting the tung of the air compressor it rolled back causing his right pinky finger to be caught between tung of trailer and i beam it had been selling on. Employee injury was laceration requiring stitches and fracture. Both to 5th digit of right pinky finger tip.

Employee was walking down the inclined coal/iron ore welt walkway, he slipped on some loose material injuring his right knee. 9-27-05 employee was sent to doctor to be checked and doctor placed him on restricted duty - no stairs or prolonged standing.

Employee was assisting contractor pull a new 13,800 VAC Feeder when he pinched his left 5th finger (pinky) between the wire being pulled and the wire way. The fingernail was loose so employee was sent to orthopedic group where fingernail was removed, cleaned & stitched (5) back on. This was done so new nail would grow back straight.

EMLOYEE HAD COMPLETED HIS ASSIGNMENT FOR THE DAY & IT WAS CLOSE TO QUITTING TIME SO HE WENT TO THE WELDING SHOP WHERE THERE WAS SOME FABRICATION OF CRAWL GUARD BEING DONE. THE EMPLOYEE CUT HIS BACKSIDE OF RIGHT HAND WHEN IT STRUCK A PIECE OF ANGLE IRON THAT HAD A SAW CUT 45 DEG. END, CREATING A SHARP EDGE.

The freight elevator was jammed.EE was trying to get the elevator door unjammed. When the elevator door finally came loose, it shut on his right hand.

EE WAS IN THE PROCESS OF REMOVING THE CARRY DECK DROTT CRANE FROM THE KILN FLOOR. AS EE WENT TO DRIVE INTO THE FREIGHT ELEVATOR, IT WOULD NOT FIT. EE STARTED SLIDING THE PIECE OF PLATE AND PINCHED LEFT MIDDLE FINGER, CUTTING THROUGH GLOVE CAUSING A CONTUSION REQUIRING 5 STITCHES

A brick was dropped by a brick mason off the kiln rig template. While EE was setting a brick down for the mason to install. The brick was dropped about eighteen inches on to EE's left hand causing a hand contusion.

CHANGED BLADE ON SAW, BLADE DID NOT GET TIGHTENED. BLADE CAME OFF SAW.

CONVEYOR BELT FIRE (25' BELT) AT THE FEED END OF THE RAW MATERIAL DRYER WHEN THE PLANT EXPERIENCED A POWER FAILURE DUE TO AN ELECTRICAL FEEDER UNDERGROUND SHORTING OUT, TAKING DOWN ALL POWER I N THE PLANT. LOCAL FIRE DEPT EXTINGUISHED WITHIN 45 MINS. NO ONE WAS IN AREA WHEN FIRE STARTED. MSHA WAS CALLED. LOOKING AT INSTALLING SLAM GATE AT FEED END.

EE WAS ALIGNING BELTS ON A CLINKER COOLER COMPARTMENT FAN & WHILE HE WAS MANUALLY TURNING THE BELTS HE GOT THE TIP OF HIS MIDDLE FINGER CAUGHT BETWEEN THE SHEAVE AND BELT. THE PINCHED FINGER REQUIRED STITCHES AND THE BONE IS CRACKED.

Another employee was operating a hoist on 8th floor by lowering trash to ground. A piece of 3/4 inch plywood fell off load and struck injured on 6th floor. He was wearing proper PPE.

EMPLOYEE WAS ON A SCAFFOLD AND WAS NOT TIED OFF. EMPLOYEE FELL FROM A LYING DOWN POSITION WHICH ALLOWED HIM TO FIT BETWEEN THE DECK BOARDS AND THE HANDRAIL. IT IS STILL UNDETERMINED WHY HE WAS LYING DOWN.

Employee was assisting with an I-beam as a spotter and guide. The beam hit the wall which caused the rope to slip from his hands dropping the beam on his right foot.

EE REMOVED A PLATE COVERING A 2'X 2'OPENING IN THE FLOOR GRATING TO LOWER A GEAR BOX SHAFT THROUGH. THE POWER CABLE TO THE CRANE WAS NOT RETRACTING PROPERLY SO EE MOVED TO CORRECT THE CABLE & TURNED AROUND & STEPPED INTO THE OPENING HE CREATED. HIS RT. LEG WENT INTO THE OPENING CAUSING A CUT ON THE FRONT SIDE OF LOWER LEG, 20 STITCHES.

BEARING BOLT ON A SYMETRO GEARBOX WAS CUT WITH A TORCH. THE BOLT BECAME AIRBORNE ONCE THE CUT WAS MADE DUE TO STORED ENERGY FROM TORQUE. THE BOLT STRUCK EMPLOYEE'S LEFT HIP.

EE WAS USING A 4 1/2 INCH GRINDER WITH A WIRE WHEEL. HE WAS CLEANING AN ELEVATOR DOOR. MATERIAL SETTLED ON THE RIM OF FACE SHIELD AND WHEN HE LOOKED UP MATERIAL SLID OFF THE SHIELD INTO HIS FA CE. EE WAS WEARING FACE SHIELD AND SAFETY GLASSES AT TIME OF INCIDENT. EE SOUGHT MEDICAL ATTENTION. TRASH IN RIGHT EYE- EYE SCRATCHED.

EE DROVE THROUGH PLANT PARTS WAREHOUSE ON A THREE WHEEL BICYCLE AS HE EXITED WAREHOUSE A TRUCK WAS IN ROADWAY SO EE TURNED TO AVOID TRUCK CAUSING BICYCLE TO COME UP ON 2 WHEELS. HIS HAND WAS PINCHED BETWEEN THE HANDLE AND A PORTABLE LOADING RAMP IN THE AREA.

ON SUNDAY MAY 25TH EE WAS WORKING IN CLINKER COOLER AND GOT CLINKER DUST IN EYES. OFF MAY 26TH. MAY 27TH EE S RIGHT EYE LID FELT IRRITATED SO HE WAS SENT TO THE DR. THE DR SAW NOTHING IN HIS E YE AND PRESCRIBED DROPS FOR IRRITATION. MAY 28THIRRITATION HAD CLEARED UP.

THE INJURED EE REMOVED A PLYWOOD SHEET COVERING A HOLE IN THE THIRD FLOOR GRATING OF THE RAW MILL SILO BLDG TO RETRIEVE A BURNING TORCH. AFTER RETRIEVING THE TORCH HE FORGOT THE REPLACE THE PL YWOOD COVERING AND FELL THRU THE OPENING A MINUTE OR TWO LATER.

EE WAS INSTALLING FLOOR GRATING BY THE SCALPING SCREEN WITH ANOTHER EE AND WHILE TRYING TO PLACE A PIECE OF GRATING WHEN SOMETHING IN HIS SHOULDER SNAPPED.

EE WAS WORKING ON PREHEATER PLUG UP WHEN HOT MATERIAL BLEW OUT A POKE HOLE AND HIT HIM 6'AWAY WHERE HE WAS STANDING. THE HOT MATERIAL HIT HIM IN THE FACE CAUSING 1ST & 2ND DEGREE BURNS TO THE FACE AND RT. EAR. EE WAS TAKEN TO USA BURN CENTER BY AMBULANCE. RELEASED FROM HOSIPITAL ON FRIDAY NOVEMBER 29TH.

EE WAS WORKING ON PREHEATER PLUG UP WHEN HOT MATERIAL BLEW OUT OF A POKE HOLE & HIT HIM SIX FEET AWAY WHERE HE WAS STANDING. THE HOT MATERIAL HIT HIM IN THE FACE CAUSING 1ST & 2ND DEGREE BURNS TO THE FACE & RIGHT EAR. EE WAS TAKEN TO USA BURN CENTER BY AMBULANCE. RELEASED FROM HOSPITAL TUESDAY DECEMBER 3RD.

EE WAS DRILLING A HOLE IN THE SOUTH SIDE OF THE CLINKER COOLER WHEN HE SAT AGAINST THE HOT BIT CAUSING HIM TO MOVE QUICKLY TWISTING HIS LEFT KNEE. EE'S KNEE CONTINUED TO BE SORE MONDAY A.M. SO HE REPORTED INCIDENT TO SUPERVISOR. EE WAS SENT TO DOCTOR AND PLACED ON FIVE DAYS LIGHT DUTY.

EMPLOYEE WAS BEATING ON THE BOTTOM OF A HOPPER & THE NEXT MORNING HIS SHOULDER WAS SORE. HE REPORTED TO ACTING SUPERVISOR APRIL 3 WHO REPORTED IT TO ME. I DISCUSSED WITH EMPLOYEE THE SAME DAY & ASKED HIM TO SCHEDULE A DOCTOR APPOINTMENT. ON5/6/03 SENT EMPLOYEE TO DOCTOR.

EE WAS REMOVING A NUT FROM A SPARE 4160 VAC SWITCH GEAR TO REPAIR THE GANTRY CRANE 4160 VAC SWITCH GEAR HE WAS REPAIRING. WHILE REMOVING THE NUTTHE CABINET FLASHED CAUSING 1ST DEGREE BURNS TO HIS FACE AND 1ST,2ND,& 3RD DEGREE BURNS TO HIS LT. FOREARM. THE EE WAS WEARING HIS HARD HAT ANDSAFETY GLASSES, WHICH PREVENTED FURTHER INJURY. EE DID NOT MAKE CONTACT WITH THE 4160 VAC.

EE WAS REMOVING A DUST COLLECTOR MOTOR & TWISTED CAUSING LOWER BACK PAIN. ON 6/13/02 HE REPORTED INCIDENT & WAS SENT TO DOCTOR WHO PRESCRIBED A MUSCLE RELAXER & RESTRICTED HIS LIFTING/BENDING.

EE WAS REMOVING FIRE BRICK FROM THE KILN DURING PLANT OUTAGE WITH AN 8 GAUGE INDUSTRIAL GUN. THEBRICK HE WAS SHOOTING WAS 55FT. INTO THE KILN. APIECE OF BRICK OR FRAGMENT OF SOME KIND FLEW BAC K, WENT BETWEEN THE BARREL ON THE GUN AND THE SHIELD. THE FLYING OBJECT STRUCK THE EE'S SAFETYGLASSES BREAKING THE RIGHT LENSE. EE WAS SENT TO EMERGENCY ROOM TO BE CHECKED OUT.

THE EE WAS ASSISTING IN REMOVING BROKEN LIFTERS AND WASHING RAW MATERIAL FROM THE DRYER APPRON FEEDER WHEN MATERIAL FLUSHED OUT AND HE WAS PULLED ONTO THE PILE OF LIFTERS THAT HAD BEEN REMOVED AND STACKED TO THE SIDE. WHEN THE EE FELL OVER THE PILE IT BRUISED AND TWISTED HIS LEFT KNEE.

EE WAS WALKING UP A GANGWAY AND WHEN HE REACHED THE END, IT DROPPED APPROX. THRE TO FOUR FEET (DUE TO SLACK IN LIFTING CABLES). EE FELL BACK ON THE GANGWAY IN A TWISTING MOTION WHICH RESULTED IN A LUMBAR STRAIN.

ON 8/7/01 AT 11:30 AM, EMPLOYEE CLIMBED ON TOP OF THE CEMENT BAG PALLETIZER THAT HAD SHUTDOWN DUE TO A BAG MISALIGNMENT. AFTER ALIGNING THE MACHINE AND BAG THE MACHINE STARTED AUTOMATICALLY AN D CAUGHT HIS LEFT FOOT BETWEEN THE UPPERPUSHER AND BAG PRESS. WHEN THE OPERATOR WAS ASKED WHAT HAPPENED HIS REPLY WAS THAT HE FAILED TO LACK OUT. SURGERY WAS NEEDED.

EE WAS THROWING SCRAP METAL INTO METAL DUMPSTER WHEN HE CUT HIS RIGHT INDEX FINGER WHICH REQUIRED FOUR STITCHES. THE EE WAS NOT WEARING GLOVES.

EE WAS CLIMBING A LADDER OUT OF #1 CARGOHOLD OF A DIXIE CARRIER LIMESTONE BARGE WHEN ONE OF THE LADDER RUNGS BROKE LOOSE CAUSING EE TO FALL APROX 3' ONTO THE BOTTOM OF THE BARGE. HE LANDED ON HIS RIGHT WRIST & LEFT SHOULDER. EE WENT TO THE HOSP TO BE EXAMINED & LATER CHECKED BY CO DR. HE WAS RELEASED FOR FULL DUTY WHEN ASKED HE WAS FINE. 3/11/02 EE COMPLAINED OF SHOULDER SENT FOR

EMPLOYEE WAS WORKING INSIDE THE CLINKER COOLER REMOVING GRATES FROM THE FIRST COMPARTMENT. A PIECE OF COATING CAME LOOSE AND STRUCK EMPLOYEES HARD HAT, KNOCKING IT OFF AND CUT HIS HEAD WHICH R EQUIRED 11 STICHES. THE PIECE OF COATING STRUCK THE RIGHT BOTTOM EDGE OF HARD HAT.

EE WAS GREASING A LOAD OUT VALVE ON THE 3RD FLOOR OF THE TRUCK SILO WHEN HE LOST HIS FOOTING ANDCAUGHT HIMSELF WITH HIS ARMS CAUSING A SCRAPE ONHIS LEFT ARM.

EE WENT TO CHECK NORTH CLINKER BUCKET ELEVATOR WHICH HAD DROPPED OUT. HE WAS CLIMBING UP STEPS TO CHECK (STOP FLAGS) ELEVATOR. WHILE COMMING BACK DOWN HE SLIPPED & CAUGHT HIS FALL WITH HIS RI GHT HAND. THE TIPS OF HIS RING FINGER & SMALL FINGER WERE PINCHED BETWEEN RIDING RAIL & WHEEL ONSOUTH BUCKET ELEVATOR.

EE WAS LIFTING THE CAGE DOOR ON A FREIGHT ELEVATOR WHEN SHE STRAINED A BACK MUSCLE. SHE WAS TAKING SOME SAMPLES FROM THE GROUND LEVEL TO THE 2ND LEVEL WHERE THE LAB IS LOCATED. THE SAMPLES WER E IN 5 GAL BUCKETS.

EMPLOYEE WAS USING A 6' PIPE TO BREAK UP CHUNKS IN A CEMENT SILO. THE PIPE RAN FROM THE OUTSIDE OF THE SILO TO THE BOTTOM CONE WHERE THE CHUNK WAS. EMPLOYEE STRAINED HIS BACK WITH A TWISTING MOTION.

EMPLOYEE ATTEMPTED TO LIFT A 4'X8'X1/4" SHEET OF STEEL WITHOUT ASKING FOR HELP OR USE OF EQUIPMENT; WHILE LIFTING THE PLATE, HE MADE A TWISTING MOTION & FELT A SHARP PAIN TO THE LEFT SIDE OF H IS MIDDLE BACK. HE WAS SENT TO THE DOCTOR FOR EXAMINATION, WHERE HE WAS GIVEN A SHOT AND PUT ON LIGHT DUTY.

A GROUP OF EES WERE CLEANING OUT TRUCK SCALE BASEMENT WITH VAC TRUCK AND BUCKETS. THEY REMOVED TWO SECTIONS OF FLOOR GRATING (APPROX 4'X4') BARRICADING THE NORTH ENTRANCE. THROUGH THIS OPENING THEY LOWER A 55 GAL DRUM, FILLED DRUM AND WITH CARRY DECK CRANE REMOVED FULL DRUM. THIS PROCESS CONTINUED UNTIL COMPLETION OF JOB. EES WERE ROLLING UP, EE RUNNING UPSTAIRS FROM BASEMENT, CALL

EE WAS REMOVING AIR FILTER FROM 1 KILN FEED CONPRESSOR. WHEN HE LIFTED THE FILTER OFF ITS STAND, IT SHIFTED DUE TO IMPROPER RIGGIN, AND RESULTED IN A CUT TO HIS RIGHT FOREARM, HE WAS SENT TO D R. AND CUT REQUIRED 4 STITCHES.

WHILE WORKING IN DUST COLLECTOR EE GOT CEMENT DUST ON ARMS AND HE DEVELOPED A SKIN RASH ON BOTH ARMS OVER THE WEEKEND SEN 4 EE TO DR 7 AM MONDAY MORNING AND HE WAS GIVEN A SHOT FOR ITCHING.

WHEN EE STEPPED UNEXPECTEDLY TO UNEVEN SURFACE HE PULLED A BACK MUSCLE. EE WAS EXITING PERSONNEDL ELEVATOR AND WHEN HE STEPPED OUT HE DORPPED 5-6" BECAUSE ELEVATOR HAS STOPPED 5-6" AOBVE DEDIC ATED SPOT MAKING UNEVEN SURFACE AT LANDING.

EMPLOYEE WAS WORKING 5' OFF GROUND LEVEL USING 10' STEP LADDER. HE WAS INSPECTING A SERIES OF THREE SHUT OFF VALVES ON A KILN FEED AIRSLIDE BY INSPECTION HATCHES. WHEN HE OPENED THE SECOND INS PECTION HATCH KILN FEED BLEW OUT CAUSING EMPLOYEE TO FALL CUTTING HIS LEFT ELBOW. KILN FEED HAD TO BE WASHED FROM RIGHT EYE AND CUT, REQUIRED 17 STAPLES.

EE WAS OPENING A FIVE GALLON BUCKET BY LIFTING THE SEAL TABS WITH A WELDING ROD. THE WELDING ROD SLIPPED CAUSING MIDDLE FINGER ON HIS LEFT HAND TO STRIKE TAB WHICH RESULTED IN A CUT WHICH REQU IRED TWO STITCHES.

EE WAS USING A SHOVEL TO PLACE CLNKER IN A WHEELBARROW WHEN HE TWISTED AT THE WAIST AND PLACED CLINKER IN WHEELBARROW HE FELT A SHARP PAIN IN HIS BACK. EE WENT TO DR NEXT DAY AND DR DETERMINED IT WAS A PULLED MUSCLE, WAS GIVEN MUSCLE RELAXER.

EE WAS USING A PIPE WRENCH TO FREE UP A STUCK ROTARY FEEDER (VALVE) WHEN THE FEEDER BROKE LOOSE HE JAMMED HIS HAND & ARM Y

EE WAS CLIMBING INTO APRON FEEDER WHEN HE LOST HIS FOOTING AND CAUGHT HIMSELF WITH HIS LEFT HAND WHICH CAUSED HIM TO TWIST HIS WRIST.

EE WAS BURNING BOLT HEADS OFF IN OVERHEAD POSITION WHEN A PIECE OF HOT SLAG WENT DOWN SHIRT SLEEVE STOPPING AT ELBOW RIGHT WHICH LEFT 2 BURNS 3/8" IN DIAMETER ON ELBOW

EE WAS USING A JACK HAMMER TO REMOVE THE RAW MILL FEED END MANDOOR WHEN A 3/16" PIECE OF THE HAMMER BIT BROKE LOOSE AND TRAVELED APPROX. 6 FEET STRIKING HIM IN THE LOWER LIP PASSING THROUGH AN D IMBEEDING IN HIS GUM. INJURY REQUIRED MEDICAL ATTENTION WHICH CONSIST OF REMOVING THE 3/16" PIECE OF METAL FROM THE GUM AND EIGHT STITCHES INSIDE THE LIP.
