A Norrell Construction elevator technician fell while climbing preheater tower elevator car ladder. In an attempt to not fall, the technician grabbed the ledge above the ladder, resulting in lacerations to several fingers and 2 fractured finger bones.

While rigging a walk-behind, EE was installing a shackle in order to fly equipment off the pre heater tower when another employee pressed the controls lowering the bucket on EE's hand causing a contusion to the hand. The device was de-energized but due to the manufactured settings the bucket can still be lowered without power.

Employee was descending stairs, when they misstepped two stairs from the bottom. The employee then fell two stairs to floor landing, where they struck their left knee. Incident became reportable on 02/10/21 when the employee was prescribed physical therapy.

An iron worker was performing welding tasks when during the work shift received arc flash burns to the eyes. It was determined that the light from the sun was behind the welder and entering EE's welding shield that caused the automatic lens to malfunction.

Employee was trouble shooting a problem on the tower around the O2 probe. While working around the probe a hose ruptured and sprayed steam and glycol on the employee. As a result the employee sustained burns to left hand.

Employee was moving a sample table. The table began to fall, employee tried to stop it from falling over, this action caused strain to the employee's shoulder. The employee was taken to the hospital and examined. It appears to be a strain possible slight tear. Employee was placed in a sling and will be examined again in one week.

Employee was on the 5th floor of the Pre-Heater tower conducting a visual walk around inspection when hot gases and hot material escaped unexpectedly near the tipping valve of the 4B vessel. Employee sustained burns to his head (nose, eyes, left ear, face and forehead), neck, forearms and left elbow.

During the bricking of the kiln employee was passing brick, placing them on the brick table. The Brick masons were in the process of keying out a ring of brick. employee had his hand on the brick table the employee turned to pickup another brick off the pallet. when his back was turned a brick fell from the key window and struck him on his hand he had resting on the table.

The following contributed to the injury: The weight of the inspection door, limited working room due to a 3" line, lack of help from a second employee and position of the employee's hand as he pushed the inspection door back into it's original position. The employee suffered from an avulsion to the soft tissue on the tip of the right index finger.

Employee started work finishing loading a cement barge with work partner. EE left work partner to go to the bathroom on the company tug boat (2nd time). When EE did not return, work partner went to look for EE finding him unconscious in the tug boat bathroom. Site First Responders rendered CPR until relieved by local EMS who pronounced EE deceased on site.

EE was removing hood of S Elevator (FM2) with overhead hoist. Dust blew off hood as it was positioned in front of fan exhaust. EE attempted to move out of way and tripped over conduit in-between elevator housings. EE landed on shoulder. EE sought treatment on 4/4. Given pain medication and restricted duty for 4 days by clinic.

At 11:30PM, Holcim EE's noticed flames at Geocycle, LLC (ID# R216) alternative fuels shed. Fire department contained fire by 12:15AM. Fire out at 2:38AM when FD left site. Roof of shed was damaged beyond salvage. Building is being stabilized to allow removal of remaining material. RCFA initiated. AFR policies and procedures will be updated to prevent occurrence.

EE was assisting another EE cutting pipe on chop saw. While moving next section of pipe, EE right hand made contact with still spinning blade. Saw had been shut off, but blade continued to spin down from previous cut. Cut went through leather gloves and into EE right thumb which required stitches to close. EE on restricted duty.

EE's were pushing tail pulley of drag conveyor with hydraulic ram to remove shims. EE was hit on right thigh by hydraulic ram when it slipped out of position under pressure. EE sustained Medial Collateral Ligament Sprain to right knee. EE working with restrictions.

EE using portable grinder with cut-off wheel to remove section of dust collector duct when wheel got stuck in pipe. Grinder kicked back and hit EE in chest and left cheek under face shield. Treated at clinic, left cheek needed 9 stitches, return to work, no restrictions.

The miner was walking down a set of stairs in the clinker pit. When he stepped off the bottom step his foot hit an unlevel place on the floor and he rolled (sprained) his left ankle and fell on the floor. He was taken to the ER room.

EE was removing thimbles from swirl chamber and as he was pulling one out of the access hole, some hot material went into the top of his glove. The hot material (kiln feed) caused a burn to the top of EE's right hand (little larger than 1/2 dollar). EE was wearing proper PPE. EE did not report incident until 11/8/10, at which time he was sent to doctor. Wound care/antibiotic.

EE used the defeater mechanism to enable himself to open the energized 4160 VAC enclosure, reached in and touched the hot fuse with his right hand causing a 3rd degree burn to tip of right thumb & index finger. 1st degree burn on palm of left hand. EE was transported by mobile fire & rescue to medical clinic burn center. Paramedic said injury was not life threatening

EE cut his left index finger, requiring 3 stitches. EE was changing out stainless steel seal segments on the feed end of the kiln, when one of the segments riding (worn) edge cut through his glove and cut his finger.

Employee was utilizing an & acetylene torch to cut a hole in the cone of a limestone silo. Employee had safety glasses & burning sheild on. After employee completed burning 4" hole he flipped his face sheild up & limestone dust fell from the bill of hard hat behind his safety glasses into left eye. Employee was sent to Dr where eye was cleaned. Kept out of dusty areas.

EE was loading bulk trucks from rail loading bay. After loading one of the bulk trucks, a bulk loader went on top of truck to close and secure the trucks loading hatch. Truck driver left loading station, went to his truck, got inside, cranked it up and proceeded to extit the rail loading bay with EE still on top of truck. EE caught himself, cut over right eye, 5 stitches.

EE was moving cable on a barge, when he twisted at the waist and felt a pain in his lower back. Strain, lumbar region. Preexisting degenerative disc disease/lumbar.

Employee was working on airslide which conveys portland cement and as he was removing the bottom of the airslide, cement dust got in both eyes. He was sent to doctor's office but since it was Saturday afternoon it was closed. Employee was taken to local hospital where eyes were washed out. No follow-up visits were needed.

Employee was holding a shaft to prevent it from turning while another employee was securing a locking collar on a bearing using a pin punch to tighten the collar. As the collar was struck a piece of the collar sheared off and embedded in the employee's left arm which required one stitch.

Employee and two maintenance mechanics were replacing flights and return rollers on pan feeder. The bolts securing one of the flights had been removed. Employee was pushing on flight with left hand. The flight dislodged and as it fell, it struck employee's left hand causing a fracture to middle finger and back of hand (3rd metacarpal).

He connected the hose from these bottles into down steamed of the filter rack, by passing the high to low pressure regulator. When system was pressurized a pneumatic explosion occurred. Employee was disconnecting a temporary electrical connection and was using a box blade knife to cut the electrical tape. The razor went through the tape, glove and cut about a one inch cut in left index finger, requiring five stitches.

At the start of employee's shift on 2/6/06 he reported that he woke up with irritated right eye. He had been grinding the day before and assumed he had gotten a foreign body in his eye. He was sent to doctor who flushed eye out but saw nothing. A prescription was given. The next day he still felt something in eye and went to an eye doctor who removed particle from under eyelid.

A loose connection on a transformer caused a power failure to the plant air compressor which caused exhaust valves to close on the Pyro main baghouse which allowed hot gases to reach the dryer feed belts and started a fire. The Mobile Fire Dept was called and they put the fire out in less than an hour. Valves will be installed to prevent future like incidents

Employee was working on kiln floor installing a monorail. There was welding in the area and clinker dust on the burner pipe. Employee was not sure when or what got into his left eye. Employee reported eye irritation 7:00 a.m. next morning. A small piece of material was removed from eye; EE's eye felt better. Next day still some irritation; sent to doctor. Eye drops prescribed.

Employee was hooking up portable air compressor while he was lifting the tung of the air compressor it rolled back causing his right pinky finger to be caught between tung of trailer and i beam it had been selling on. Employee injury was laceration requiring stitches and fracture. Both to 5th digit of right pinky finger tip.

Employee was assisting contractor pull a new 13,800 VAC Feeder when he pinched his left 5th finger (pinky) between the wire being pulled and the wire way. The fingernail was loose so employee was sent to orthopedic group where fingernail was removed, cleaned & stitched (5) back on. This was done so new nail would grow back straight.

EMLOYEE HAD COMPLETED HIS ASSIGNMENT FOR THE DAY & IT WAS CLOSE TO QUITTING TIME SO HE WENT TO THE WELDING SHOP WHERE THERE WAS SOME FABRICATION OF CRAWL GUARD BEING DONE. INJURY REQUIRED MEDICAL ATTENTION WHICH CONSIST OF REMOVING THE 3/16" PIECE OF METAL FROM THE GUM AND EIGHT STITCHES INSIDE THE LIP.
