Black Warrior Minerals Inc · Struck against a moving object

Bulldozer operator was preparing a drill bench on top of an approx. 50' highwall. Operator was pushing consolidated material over the edge of the highwall. The bulldozer traveled over the edge of the highwall and fell to the pit bottom. Operator was found unresponsive. 1st aid and CPR were performed until the coroner arrived and pronounced the victim dead at 1404 hours.

Blaster's Helper was driving/operating Blasting Agent Bulk Truck when he missed a gear descending a 14% grade ramp. Driver was unable to stop or slow truck resulting in truck crashing into highwall. Employee suffered contusion to head and upper body.

The employee was in a truck with a crane attached. He was lifting a pump with the crane. The crane fell and jerked the truck up and slammed it down. The employee hurt his back with the impact of the truck when it fell.

Coal truck driver for Black Gold Trucking had got- ten loaded with coal and was leaving out of the coal yard. He got almost to the gate when he died with a heart attack. The truck rolled back down a slight grade and turned over on its left side. Damage to truck 7000.00 approx & 3000.00 approx. to the dump trailer.

HE WAS OFF WORK THE FOLLOWING DYA, CAME BACK 4-20-00, WAS CALLED BACK TO DR'S OFFICE ON 4-24-0
