The employee exited the fuel truck and slipped and hit his shoulder against the fuel tank on the truck. The employee was seen by a doctor on 11/16 for an MRI and was put onto modified duty on 11/17/04. The employee has a contusion of his left shoulder.

WHILE USING A 4" GRINDER TO CLEAN HANDRAIL, EMPLOYEE LOST CONTROL OF GRINDER. THE WHEEL CUT THROUGH HIS GLOVE CAUSING A HALF-INCH LACERATION ON HIS RIGHT HAND BETWEEN THE THUMB AND FOREFINGER. GRINDER WAS PROPERLY GUARDED, SWITCH WAS OF NON-LOCKING TYPE.
