Employee was walking toward the office at the end of shift and tripped on some plastic stripping straps that had blown out of the dumpster, injuring shoulder. EE received results from an MRI on November 26, 2019 and was diagnosed with a torn rotator cuff. EE is currently working full duty awaiting appointment with an Orthopedic specialist.

Employee was performing regular maintenance on the blending tower. After taking several water breaks he began feeling ill and sought medical attention. He was diagnosed with heat exhaustion.

Employee injured his right 5th finger while opening a sliding chain link gate. He was diagnosed with a mallet finger and it was placed in a stack splint. No restrictions or lost time. This incident is being reported within 10 days of seeking medical attention.

Employee was moving a boulder, when the boulder slipped off the bucket causing the loader to jerk and the employee bounced up losing his hard hat and bounced again and hit his head on the fan switch, lacerating the right side of his scalp.

A Vulcan Contractor Services employee ID #01-03226 fractured his R fibula while working at Tuscumbia Quarry #01-00045. Employee threw a tie-down chain over a load to be hauled. The chain swung back under the load toward employee; he jumped back and feet became entangled in hoses attached to trailer causing him to fall.

Injured employee and three others were installing a feeder pan onto the Hazmag feeder vibrator. Injured employee was prying feeder pan with a pry bar away from the feed box, so it could be picked up higher. The pry bar got caught against a piece of metal which pinched his right thigh.
